Case #: 16-68723


Location: Park Avenue and 8th Street


Date: 10/31/16 @ 1449hrs


Driver #1: Malikah Rothmaller (F-19) Vineland


Driver #2: Tammy Demasse (F-43) Bridgeton

Driver #1 stated as she was stopped in traffic on west bound Park Ave, vehicle #2, which was also west bound behind her, collided into the rear of her vehicle.

Driver #2 stated as she was traveling west bound on Park Ave, she was blinded by the sun and was unable to see that vehicle #1 was stopped in traffic. As a result vehicle #2 collided into vehicle #1 causing the collision.

As a result of the collision both the driver of vehicle #1 and her passenger, Veronica Bonano, complained of back and neck pain. Vineland EMS responded to the scene and transported both to Inspira Hospital for treatment. Driver #2 related she was not injured.

Joanne Burgos, who witnessed the collision, related the following. Burgos explained she was stopped in traffic due to a school bus several vehicles in front of her. While stopped, Burgos saw vehicle #1 in her rear view mirror, which was also stopped directly behind her, get collided into by vehicle #2.  

Vehicle #1 sustained minor / moderate damage to its entire rear end. Vehicle #2 sustained minor / moderate damage to its front end.

Vehicle #1 was towed by Caldero's towing. Vehicle #2 was towed by Rulyn's Towing.

Driver #2 was issued a summons for careless driving / VC-011818.

Location: Main Road and Chestnut Avenue


Date: 10/31/16 @ 1450hrs


Driver #1: Safiyah Givens (F-33) Vineland


Driver #2: Yolanda Moreno (F-42) Vineland


Driver #1 stated she was traveling south on Main Rd. in the outside lane.  She stated vehicle #2 made a left hand turn in front of her to turn into Memorial School (424 S. Main Rd.) at which time vehicle #2 struck her vehicle.

Driver #2 stated she was traveling north on Main Rd.  She stated she stopped in traffic to make a left turn into 424 S. Main Rd.  She stated the traffic in the opposite lane of travel was stopped at which time a vehicle waved her in to make her turn into the school.  She stated she did not see vehicle #1 traveling north in the outside lane at which time they collided.  Driver #2 stated she believes vehicle #1 was traveling at a high rate of speed.

Witness #1, Yanitza Cardona, related she observed the collision.  She stated vehicle #1 was driving at a high rate of speed on Main Rd. when vehicle #2 attempted to turn into the school at which time both vehicles collided.

Vehicle #1 sustained damage to the front end and was towed from the scene by Speedys.  Vehicle #2 sustained damage to the passenger side of the vehicle and was towed from the scene by Williams.

Driver #1 had complaint of pain to her back but declined medical attention.  Driver #2 had complaint of paint to her jaw and arm and was transported to IHN by Vineland EMS.

Driver #2 was issued the following summons.

0614 VC 011859     Improper Turn     39:4-123

A subpoena were issued in regards to the above summons.

Location: Delsea Drive and Chimes Terrace


Date: 11/1/16 @ 1300hrs


Driver #1: Jessica Davis (F-34) Millville


Driver #2: Gary Gibson (M-38) Pittsgrove


Driver #3: Albert Collini ( M-72) Millville


Driver #4: Jonathon McClintock (M-23) Port Norris NJ


On November 01, 2016, I, Officer Fredrick DeMary, responded to the area of South Delsea Drive, approximately 225 feet north of Chimes Terrace, for a four vehicle crash.  When I arrived on scene I observed vehicles involved in this incident were still on the roadway of South Delsea Drive. 


Driver # 1 stated only thing she can remember about the crash is prior to the collision she was north on South Delsea Drive.  Driver # 1 told me she doesn’t remember how the collision occurred, only that she was injured because of the incident.  Driver # 1 complained of pain in her head, chest and abdomen from the collision.  Driver # 1 had a minor laceration to the face, with small amount of bleeding. 


Driver # 2 stated he was northbound on South Delsea Drive, traveling in front of vehicle # 1.  Driver # 2 said he was slowing in traffic for a red traffic signal at the intersection of South Delsea Drive and West Chestnut Avenue, controlling his lane of traffic.  While slowing, Driver # 2 said he observed vehicle # 1 approach from the rear and did not appear the vehicle was going to stop.  Driver # 2 said he tried to avoid the collision by driving his vehicle off the roadway, but was unsuccessful.  Vehicle # 2 was struck in the rear by vehicle # 1, causing vehicle # 2 to leave the west side of the roadway.  I was able to confirm Driver # 2’s involvement with the collision by a portion of vehicle # 2’s bumper being on the roadway, in the area of the crash scene.  Driver # 2 complained of pain in his neck after the crash, but later stated the discomfort went away. 


After the crash vehicle # 1 reportedly continued northbound on South Delsea Drive, striking the rear of vehicle # 3. 


Driver # 3 stated he was slowing down in traffic on the northbound lane of South Delsea Drive when his vehicle was struck in the rear by vehicle # 1.  Driver # 3 stated that after his vehicle was struck in the rear by vehicle # 1, his vehicle was forced forward into the rear of vehicle # 4.  Driver # 3 had no complaints of pain from this crash.   


Driver # 4 stated he was stopped in traffic on the northbound lane of South Delsea Drive, north of Chimes Terrace for a red traffic signal controlling his lane of travel.  Driver # 4 said that while stopped his vehicle was struck in the rear by vehicle # 3.  Driver # 4 indicated that he believed vehicle # 3 was struck in the rear by vehicle # 1 prior to the crash. 


Vineland EMS responded to the scene of the crash for the reported injuries.  Driver # 1 was transported to Inspira Hospital 1505 West Sherman Avenue, Vineland, New Jersey 08360, for the injuries she sustained.  Driver # 2 refused any medical attention at the scene.   


Investigation revealed that Driver # 1’s actions was the cause of this multiple vehicles collision.  A check of the roadway was negative for any brake tire marks prior to the crash scene.  Driver # 1 was issued summons # 0614 VC 013102 for careless driving.  Vehicle # 1 was removed from the scene by Caldero’s Towing.  Driver # 3 contacted his own tow service, A-1 Towing, for the removal of his vehicle.   


 


Case#: 16-69173


Location: Main Road and Butler Avenue


Date: 11/2/16 @ 0950hrs


Driver #1: John Davis (M-51) Millville


Driver #2: Bailey Sutton (M-65) Millville


Dispatch to the area of S. Main Rd. and E. Butler Ave. for a minor collision.  Upon my arrival, I spoke with driver #1 who related the following about the incident.  Driver #1 stated that he was stopped at the red light on S. Main Rd. at the intersection with E. Butler Ave.  He stated that as he waited for the light to change, he was struck from behind by vehicle #2.

Driver #2 stated that he was stopped behind vehicle #1, and he believes that his foot slipped off of the brake.  He stated that his vehicle went forward and he struck vehicle #1.

The occupants in vehicle #1 indicated that they were not hurt and did not need EMS to respond. However, EMS was already on scene and spoke with Driver #2.  Driver #2 stated that he was not hurt and signed the refusal from EMS.

Vehicle #1 sustained very minor damage to the rear of the vehicle.  Vehicle #2 sustained minor damage to the front of the vehicle.

Driver #2 was issued the following summons:

Case #: 16-69301


Location: Sixth Street and Washington Avenue


Date: 11/2/16 @ 1750hrs


Driver #1: Miguel A. Padilla Jr. (M-42) Vineland


Driver #2: Martha Ana Ordaz-Izquierdo (F-42) Vineland


On 11/03/2016 I was dispatched to the area of Sixth Street and Washington Avenue for a two car motor vehicle collision with no reported injuries. Upon my arrival I spoke with Driver #1 who stated the following: Driver #1 explained he was travelling south on Sixth Street, and began to slow down to turn onto Washington Avenue.  Driver #1 advised while making a left turn, Vehicle #2 passed him on the left side and he was struck by Vehicle #2 as he was turning.  Driver #2 did not have a complaint of pain while on scene.

On scene I spoke with Driver #2 who stated she was travelling south on Sixth Street approaching Washington Avenue.  Driver #2 explained Vehicle #1 was at a complete stop in the roadway, and it did not have a turn signal on.  Driver #2 stated she passed Vehicle #1 on the left, and while passing she was struck by Vehicle #1.  Driver #2 had a complaint of pain to the stomach and EMS was called to the scene.  Driver #2 declined medical treatment, and the occupants inside Vehicle #2 did not have a complaint of pain. 

Vehicle #1 sustained damage to the front driver side bumper and quarter panel, and was driven from the scene by Driver #1. Vehicle #2 sustained damage to the passenger front and rear door, and was left at the scene. 

Driver #2 was issued the following summonses:
Summons #0614-VC-12267 for Expired Registration (39:3-4)
Summons #0614-VC-12268 for Improper Passing (39:4-85)
Summons #0614-VC-12267 for Unlicensed Driver (39:3-10)
