Driver 1 stated he stopped at the
intersection of W Landis Ave and N Orchard Rd waiting for traffic to be
clear to turn left (north) onto N Orchard Rd from W Landis Rd.
Driver 1 stated the intersection was clear and the light began to turn from
green to yellow when he began to make the turn. Driver 1 stated once he
began the turn vehicle 2 "came out of no where" striking his
vehicle. Driver 2 stated he was traveling west on W Landis Ave
approaching the intersection with N Orchard Rd. Driver 2 stated the light
was green as he entered the intersection when vehicle 1 suddenly turned in
front of him causing the collision. Driver 3 stated he was waiting on the
red light at the intersection of N Orchard and W Landis. Driver 3 stated
he observed vehicle 1 turn in front of vehicle 2 causing the initial
collision. Driver 3 stated the impact from that collision caused vehicle
1 to strike his vehicle.
Driver 2 was transported to Inspira for minor abrasions to his wrists from the
airbags. Drivers 1 had no visible signs of injury or complaints of
pain. Driver 3 stated the impact from the collision caused some dirt to
get into his eyes, but he declined medical treatment at the scene.
Damage to vehicle 1 & 2 was major. Damage to vehicle 3 was moderate.
There were no summons issued.
Driver 2 & 3 were advised of their options for complaint action at the
Vineland Municipal Court.

On 09/14/2016 I was dispatched to
the area of North Main Road and East Park Avenue for a two vehicle collision
with no reported injuries. Prior to my arrival dispatch advised one of
the vehicles (Vehicle #2, NJ HU1716) left the scene travelling west on
Park Avenue.
Upon my arrival I spoke to Driver #1 who stated the following: Driver #1
explained she was stopped at the red light in the south bound lane
on North Main Road. Driver #1 informed me that while she was stopped,
she was struck by Vehicle #2 in the rear. Driver #1 advised after she was
struck, she exited the vehicle and spoke with Driver #2. Driver
#1 stated Driver #2 explained there was no significant damage, and the
police did not need to be involved. Driver #1 said she got back into her
vehicle, made a right turn, and began travelling west on Park Avenue when
she discovered there was an issue with her vehicle. Driver #1
advised she was able to obtain Driver #2's "tag" number as it
traveled past her, and she contacted this department. Driver #1 had a
complaint of pain to her neck, but declined medical attention. The
occupants inside vehicle #1 did not have a complaint of pain.
Note: Driver #1 described Driver #2 as a white male approximately twenty
years of age.
Vehicle #1 had very minor damage to the rear bumper and was driven from the
scene by the driver. Note: there was a scratch mark on the rear bumper of
vehicle #1, but Driver #1 stated it was "old" damage".
Note: While utilizing the Pro Phoenix "in-house" record system,
it was discovered vehicle #2 was registered to Marc Rousseau of 10 Shingle
Oak Drive, Voorhees, NJ, 08043. I was unable to locate a contact
number for Marc, and I contacted the Voorhees Police Department
to respond to the residence to locate Marc. It should also be
noted that Driver #1 described Driver #2 as a white male approximately
twenty years of age, and Marc Rousseau is Sixty One years of age. At
the time of this report, Voorhees Police Department has not contacted me with
any further information.
On 09/16/2016 I was contacted by Voorhees Police Officer Macchili who informed
me he spoke with Melanie Rousseau (Driver #2). Officer Macchili informed
me he observed a SUV with a license plate bearing (NJ HU1716) in the driveway
of the registered owners residence. Officer Macchili said
while observing the SUV, he did not observe any significant damage on the
vehicle. Officer Macchili provided me with Driver #2's contact
information, and I contacted Driver #2 who stated the following: Driver #2
explained on 09/14/2016 she was stopped at an unknown intersection in Vineland,
NJ. Note: Driver #2 said she does not reside in Vineland and she was
unfamiliar with the roads.
Driver #2 advised she was at a complete stop behind Vehicle #1, and Vehicle #1
was also at a complete stop because the traffic signal was red. Driver #2
said while stopped, Driver #1 exited the vehicle and looked at Vehicle #1 and
Vehicle #2. Driver #2 stated she did not hit Vehicle #1 and she did not
know why Driver #1 was looking at both vehicles. Driver #2 advised Driver
#1 got back into Vehicle #1, then made a right on to Park
Avenue. Driver #2 said she also turned on to Park Avenue, and
noticed Vehicle #1 pulled over to the shoulder of the roadway, and the hazard
lights were on. Driver #2 advised she continued past Vehicle #1,
because she did not believe she was involved in a collision.
While speaking with Driver #2 she stated there was no damage to her vehicle,
and she did not sustain any injuries. Driver #2 informed me she was the
only occupant inside her vehicle, and stated there was two small children
inside vehicle #1 at the time of the incident. Driver #2 provided me with the
necessary information to complete this collision investigation.
No Summonses issued

Driver of vehicle #2 stated she
was stopped at the stop sign on E Butler Avenue and Lincoln Avenue when vehicle
#1 struck her from behind.
Driver of vehicle #1 stated he was driving east on E Butler Avenue and was
approaching the stop sign at S Lincoln Avenue. He stated he misjudged his
distance and to vehicle #2 and struck her rear end.
Vehicle #2 sustained minor damage to the trunk area and rear bumper.
Vehicle #1 had damage to the front bumper of his
vehicle which caused the front license plate to fall off.
No complaint of pain to either driver.

The driver of vehicle number one
stated, he does not believe that his trailer collided with vehicle number two.
The driver of vehicle number two stated
while his vehicle was parked, vehicle number one entered the parking lot and
collided with the front of his vehicle.
At the time of my arrival there were no visible
signs of injuries and no complaint of pain. Both driver refused medical
attention.
Vehicle number one sustained minor to no damage and
was left at the scene. Vehicle number two sustain minor damage to the front and
was also left at the scene.
Due to the collision occurring on private property,
the minimal amount of damage, and no one being injured, this collision was
deemed non-reportable so no summonses were issued at this time.

Driver 1 stated he was traveling
West on Sheridan Ave and went to turn right onto the Boulevard and his car slid
on the wet roads. Driver 1 was not injured as a result of the collision. Driver
1 was issued summons VC 7340 for careless driving as well as E ticket E16 2267
and 2268 for expired registration and bald tires (rear tires).
Driver 2 stated he was traveling South on the SE Boulevard and was slowing down
to turn left onto Sheridan Ave when vehicle 1 slid around the corner and struck
the front of his truck. Driver 2 stated he was not injured as a result of the
collision. Due to extensive front end damage, vehicle 2 was towed from the
scene by Rulyn's.

Driver 1 said he was traveling
south on the SE Blvd and attempted to turn right (west) onto Walnut Road but due
to the wet roadway his vehicle slid and struck Driver 2's vehicle in the front
driver side fender. Driver 1 did not claim any injuries as a result of
this incident and his vehicle sustained minor front end damage and was driven
from the scene.
Driver 2 said that he was stopped at the yield sign on Walnut Road and the SE
Blvd when Driver 1's vehicle slid into the front fender of his
vehicle. Driver 2 did not complain of any injuries and his vehicle
was towed from the scene by Good Fellas towing, who was contacted by the owner
of the vehicle. Driver 2's vehicle sustained minor damage to the
front fender however it rendered it disabled.

Driver 1 stated she was
stopped at the stop sign, north bound at Third St/Wood Street with the
vehicle's yellow flashing overhead lights on. She stated she didn't see
any opposing traffic therefore she entered the intersection and then stopped a
short distance away to drop off her passengers, switching the flashing yellow
lights to flashing red lights. She stated as all of the occupants were
getting up from their seats Vehicle 2 crashed into the rear of her
vehicle. Driver 1 was issued summons 0614-VC-12653 for Failure
to Yield ROW at Intersections, 39:4-90, with an appearance before the
Vineland Municipal Court.
NOTE: Vehicle 1 is a 2012 IC School Bus yellow
color with flashing lights visible from the front and rear of the vehicle with
a red stop arm that protrudes from the drivers side when the vehicle is stopped
and the red lights are flashing.
Driver 2 stated she was west on Wood Street approaching
Third Street when Vehicle 1 entered the intersection in front of her and then
quickly stopped. She stated she attempted to stop however was unable to
and crashed into the rear of Vehicle 1. While speaking to Driver 2 at her
vehicle I noticed that her vehicle inspection sticker appeared to be
fraudulent. The silver reflective tape on the bottom of the sticker was
missing in several locations and the month indicator showing "4" was
a thinner size then I've seen on many other stickers. I had dispatch
check the DMV/MVC data base for an "inspection inquiry" and learned
that according to the database the vehicle had never been through NJ MVC
inspection under the registration displayed. I removed the sticker from
the window, it easily pulled off, and saw that the "4" had been taped
on the sticker with clear tape and the edges of the white paper displaying the
"4" were not cut evenly.
The sticker was tagged and turned into the ID Section
of the VPD as evidence. Driver 2 was issued summonses 0614-VC-12651 for
Failure to Inspect, 39:8-1 and 0614-VC-12652 for Fraudulently Obtaining an
Inspection Sticker, 39:8-9c both with court appearances before the Vineland
Municipal Court.

D2: Joshua Carrasquillo, 5/18/1992, Oak Lane, Vineland NJ
Driver 1 said she was traveling
north on Delsea Drive when the vehicle in front of her suddenly executed a
right turn. Driver 1 said she braked while the car in front of her
turned at which time she was struck from behind by Driver 2. Driver 1's
vehicle sustained minor damage to the back bumper and was driven from the
scene. Driver 1 did not claim any injuries as a result of this incident.
Driver 2 said he was traveling north on Delsea Drive behind Driver 1 when
she stopped suddenly. Driver 2 said he tried to avoid striking Driver 1
by swerving towards the center of the roadway but due to the rain and wet road
conditions was unable to avoid the collision. Driver 2 did not
complain of any injuries as a result of this incident nor was he issued
any summons. Driver 2 drove his vehicle from the scene as it sustained
minor front end damage.

D2: Johnathan Ramos, 9/30/1986, E Weymouth Rd, Vineland NJ
Driver #1 stated she was stopped in the east lane of Elm Rd at the
intersection of South Orchard Rd, for the stop sign. Driver #1 said while
checking the intersection for traffic her vehicle was struck from behind by
vehicle #2.
Driver #2 stated he was traveling east on Elm Rd towards the
intersection of South Orchard Rd. Driver #2 said he began slowing down as
he approached the intersection when he attempted to stop behind vehicle
#1. Driver #2 said while stopping his vehicle slid on the wet pavement
and struck the rear of vehicle #1.
No visible damage was observed on either vehicle involved in the
collision.
At the scene driver #1 said her neck felt “stiff” as a result of
the collision. Vineland EMS arrived however she refused any further
medical treatment. Driver #2 and his passenger both stated they were not
injured and refused medical treatment.
No witnesses to the collision were located and no summonses were
issued at the scene.

Driver # 1 stated he was south
bound on South Delsea Drive, approaching Brown Road, following behind vehicle #
2. Driver # 1 said he observed vehicle # 2 slowdown in traffic for a
vehicle waiting to turn onto brown Road, at which time he unsuccessfully
attempted to slow down and stop for traffic as well, striking into the rear of
vehicle # 2.
Driver # 2 sated she was stopped in traffic on the south bound lane of North
Delsea Drive for a vehicle in front of her waiting to make a left turn onto
Brown Road. Driver # 2 said that it was when she stopped for traffic her
vehicle was struck in the rear by vehicle # 1.
Investigation revealed that Driver # 1 failure to maintain control of his
vehicle was the cause of this crash. Driver # 1 was issued summons # 0614
E16 002289 for careless driving (39:4-97). No reported injuries were made
by either party involved in this incident.
