Driver #1: Neiswender, Marissa (F-21) Millville
Driver #2: Petrosh, Alfred (M-61) Mays Landing NJ
Driver #1 stated she was traveling north bound on North Lincoln Avenue and had her left turn signal on to turn left. Her passenger told her at the last second she needed to turn right on Maple Avenue. She stated she quickly turned and thought vehicle #2 was further away and collided. Driver #1 and her passenger claimed to have no injuries. Vehicle #1 was towed from the scene by A1 Towing.
Driver #2 stated he was traveling north bound on North Lincoln Avenue and observed vehicle #1 turn left onto Maple Avenue. He stated vehicle #1 then turned around going straight across the intersection east bound on Maple Avenue. Driver #2 stated he attempted to avoid striking vehicle #1 by braking and pulling to the right. He stated vehicle #1 struck his vehicle. Driver #2 claimed to have no injuries. Vehicle #2 was towed from the scene by M&R Towing.
I spoke to a witness, Linda Correa, who advised she was in her yard, 462 North Lincoln Avenue. She stated she observed vehicle #1 north bound on North Lincoln Avenue and then turn left onto Maple Avenue. She stated vehicle #1 then turned right onto Maple Avenue when vehicle #2 attempted to go to the right and the vehicles collided.

On this date, August 8, 2016, I was dispatched to the intersection of East Chestnut Avenue and South Sixth Street, in regards to a motor vehicle crash. I arrived on scene, and located both vehicles in the parking lot of 607 East Chestnut Avenue. I pulled into the lot, exited my Patrol vehicle (809) and activated my Department issued Body Worn Camera (BWC.) I met with both drivers and the following information was related to this Officer. Driver #1 stated he was traveling east on Chestnut Avenue, next to driver #2, and his vehicle came into contact with vehicle #2. Driver #2 stated she was traveling east on Chestnut Avenue, in the passing lane, next to vehicle #1, when he came into her lane and struck the side of her vehicle. Driver #2 added that she believes a vehicle was turning into the parking lot in front of vehicle #1 and that may be why he came into her lane. Neither driver had any complaints of pain and Vineland EMS was not needed on scene. Vehicle #1 sustained minor damage to the driver side front fender, and driver's door, but was able to be driven from the scene. Vehicle #2 sustained minor damage to the passenger side doors (four door vehicle). Both vehicles were able to be driven from the scene. Vehicle #2 is a company car which is registered to Quest Diagnostics Inc. At this time no summonses were issued and both drivers were sent on their way.

Driver #1: McGarry, John (M-86) Buena
Driver 1 stated that after leaving Newfield National bank he got into his vehicle to leave and his foot got caught between the brake and gas pedal and he inadvertently pushed the gas pedal which caused his vehicle to jump the curb and crash into the west side of the building and main doors. Driver 1 said he was able to back his vehicle off the curb. Driver 1 did not complain of any injuries as a result of this incident and he drove his vehicle from the scene. Driver 1's vehicle sustained minor front end damage and minor damage to the front passenger side door and he was not issued a summons in connection to this investigation. I spoke with Newfield National Bank Branch Manager James Rizzo who indicated that he would be contacting someone to come out and provide an estimate for the damage done to the door and post struck by Driver 1. I also contacted the City of Vineland License and Inspections and informed them of the incident in order to have the damage evaluated for any possible structural damage, although none was readily apparent.

Driver #1: Drian, Stephanie (F-51) Millville
Driver#1 stated she was stopped in traffic on Hennis Rd, facing west when Driver#2 began honking her horn. Driver#1 stated Driver#2 was attempting to pass her, but traffic was backed up. Driver#1 said Driver#2 then backed up to gain some distance and attempted to go around her. Driver#1 stated as Driver#2 passed her, she side swiped her vehicle, causing damage to the driver’s side rear bumper and driver’s side. Driver#1 stated Driver#2 fled the scene and was only able to describe the vehicle as a grey car with a NJ registration. Driver#1 had no injuries or complaints of pain on scene.

Location: East Avenue and Wood Street
Date: 8/10/16 @ 0217

Driver #1: Cruz, Jason (M-43) Vineland
Driver #1 stated he was traveling west bound on East Wood Street, approaching North Eighth Street. Driver #1 stated as he was traveling, he fell asleep behind the wheel. Driver #1 stated he accidently struck vehicle #2.
The owner of vehicle #2 was not located on scene. I spoke with Billy Cannon of 822 Wood Street, who was in possession of the vehicle. Billy was advised of the collision.
Investigation on Scene:
Driver #1 fell asleep while traveling and veered to the right of the road. Vehicle #1 side swiped vehicle #2, causing the driver to wake up. Nothing else was struck.
Driver #1 claimed no injury. Vehicle #1 sustained moderate damage to the front right corner of the vehicle and the front right tire. Vehicle #1 was towed by M and R towing.
Vehicle #2 was unoccupied. Vehicle #2 sustained moderate damage to the front and rear left doors.
Driver #1 was issued a summons for reckless driving, VC-011541.

Location: Landis Avenue and Mill Road

Driver #2: Lorito, Nichole (F-42) Bridgeton
Vehicle #3: Parked vehicle
Driver #1 stated he was traveling east bound on West Landis Avenue in the area of Mill Road when he changed lanes from the inside lane to the outside lane. Driver #1 indicated he put on his right signal light and gradually changed lanes when Vehicle #2 reportedly was traveling at a high rate of speed and “came out of nowhere (in outside lane).” Driver #1 advised that the roadway was clear prior to him changing lanes. Vehicle #1’s passenger side struck the front driver side of Vehicle #2.
Driver #2 stated she was traveling east bound on West Landis Avenue in the area of Mill Road in the outside lane. Driver #2 indicated Vehicle #1 was traveling in the inside lane heading in the same direction and suddenly turned in front of her as he was attempting to enter private property on the south side. Driver #2 advised she was unable to avoid the collision. Vehicle #1’s passenger side struck the front driver side of Vehicle #2. As a result of the initial collision, Vehicle #2's front passenger side then struck Vehicle #3's rear bumper (driver side). Vehicle #2 continued on to strike the fence on the property of 1835 West Landis Avenue head on. Vehicle #2 continued and sideswiped a tree on the driver side. Vehicle #2 finally came to a rest while traveling east bound striking another tree. I met with Lorraine McCarthy (Manager of 1835 West Landis Avenue) who brought to my attention the damage to Vehicle #3. McCarthy was also advised of the damage fence and provided the case number for the report.
Driver #1 had a complaint of neck pain. Therefore, Emergency Medical Service (EMS) was requested and responded to the scene. Driver #1 went with EMS to the hospital to get further evaluated.
Driver #2 had a complaint of pain to her back and left knee. Driver #2 declined further treatment with EMS.
Vehicle #1 major damage to the passenger side and was towed by M & R Auto Body.
Vehicle #2 sustained major damage to the front driver side and was towed by A1 Towing. Vehicle #3 sustained minor damage to the rear bumper on towards the driver side.
Driver #1 was issued the following summons: 0614-VC-011394 (D.L. Suspended; 39:3-40); and 0614-VC-011395 (Unsafe Lane Change; 39:4-88b).
