Driver 1- stated he was travelling south bound on N Delsea Dr. when he began experiencing his throat closing up. Driver 1 advised he has a medical condition and uses an inhaler for it. Driver 1 stated that as he was experiencing not being able to breathe his vision got blurry and before he knew it he looked up realized he had crashed. EMS was on scene as well as the fire company. Driver 1 refused medical treatment and claimed no injuries. While on scene I was able to observe how the collision took place and in what sequence it happened. Driver 1 drove off of the road way hitting the curb and then hitting the utility light. Driver 1 then struck the fire hydrant and kept driving through the metal fencing of International auto sales. After hitting the wire fence driver 1 then strikes the parked vehicle at the dealership, which is where the collision ended. The water utility company responded on scene as well as the NJ Department of Transportation for the utility light. The wire fence is owned by the car dealership (International auto sales). I issued driver 1 two tickets at the scene: 0614-VC-009479 (Unlicensed Driver 39:3-10) 0614-VC-009480 (Careless driving 39:4-97) During the collision there were multiple points of impact. 1st impact was the curb side 2nd impact the utility light 3rd impact fire hydrant 4th impact wire fence 5th impact Honda (minivan) Damage to the fence was estimated at $500.00 Damage to the utility light standard is $2,500.00 Damage to the fire hydrant is $1,000.00

Driver #1 stated he was inside the driver seat, vehicle was running but it was in park. Driver #1 advised he was dropping off a patient at 80 S Main Rd. prior to him exiting the vehicle, Vehicle #1 was struck by Vehicle #2 on its rear bumper. Driver #1 and Passenger #1 claimed no injuries. Driver #1 stated he didn’t observe Vehicle #2 backing out of the parking stall.
Driver #2 stated his vehicle was in a parking stall, facing northbound at 80 S Main Rd. Driver #2 stated as he began to back up he did not observe Vehicle #1 in his rearview. Vehicle #2 then struck Vehicle #1 on its rear bumper. Driver #2 claimed no injuries.
No summons issued.

Driver #1 stated she was stopped in traffic just east of the intersection of W. Sherman Avenue and Orchard Road. Furthermore, stating she was placing her coffee in the center console when released her foot off the brake striking Driver #2 in the rear of his vehicle. Driver #1 said she witnessed the light turn green but did see the vehicle in front of her move. Driver #2 stated he was also stopped in traffic at the intersection in front of Driver #1. Furthermore, when the light turned green he stated his vehicle was struck in the rear by Driver #1. Driver #3 stated he was also stopped in traffic in front of both Driver #1 and #2. Furthermore, when the light turned green he stated his vehicle was struck in the rear by Driver #2. Driver #3 claimed minor injuries to his back due to the impact of the collision. EMS responded to the scene, at which time Driver #3 stated he was in good condition and did not need further treatment. No summonses were issued in this case.

Driver #1 stated while traveling south bound on S. Delsea Drive he attempted to make a left hand turn onto W. Chestnut Avenue when his air brakes to the tractor trailer did not engage as the truck started to slide. Driver# 1 stated that he attempted to control the vehicle due to heavy traffic conditions as he guided the tractor and trailer into a fixed sign located at 521 S. Delsea Drive (Riggins Gas Station). Vehicle #1 struck the Riggins Gas support that held up the gas prices causing damage. I spoke with a representative of 521 S. Delsea Drive, Farrukh Khawaja, who indicated that he did not witness the crash, however, showed all of the new damage to this Officer. I observed a small bush has been run over by Vehicle #1 as well as a large 2 legged support which holds up the business sign and gas prices. Khawaja indicated that the damage is approximately $30,000.00 in damage. He stated that this is an estimate, but they will have an official estimate taken at a later date. *Insurance Policy for Vehicle# 1- All Star Insurance (2821 S. Parker Road Aurora CO 80014) Contact information- Yelena Tyevs (303) 755-2378 (all-ins@hotmail.com) NAIC# 35939 There were no injuries reported to this Officer while on scene. Extensive damage was to the sign of 521 S. Delsea Drive was created by the crash. This Officer did not witness this crash. Driver#1 was issued Summons# VC-009552 for careless driving. Driver #1 stated he was making a right turn (heading west bound) onto West Park Avenue from North East Avenue when he accidently sideswiped Vehicle #2. Vehicle #1’s front driver side mirror struck Vehicle #2’s front driver side mirror.
Driver #2 stated he was at the intersection of West Park Avenue/ North East Avenue facing east bound in the left turn lane when Vehicle #1 sideswiped Vehicle #2 while making a right turn onto West Park Avenue. Vehicle #1’s front driver side mirror struck Vehicle #2’s front driver side mirror.
All the occupants of the Vehicle #1 and #2 had no complaint of pain. John Morris (Supervisor from Board of Education) responded to the scene and was advised.
Vehicle #1 sustained minor damage to the front driver side mirror.
Vehicle #2 sustained minor damage to the front driver side mirror.
No summon issued. Sgt. C. Garrison approved a non-reportable collision.

On 05/05/2016, at approximately 2:41pm, this department received a call that a female (driver #1) was involved in a collision in the area of South East Avenue and East Almond Street, where she struck a bicyclist. The driver related that after the collision, the bicyclist fled the area on foot when she responded to this department to report the incident, due to the fact she did not have a cell phone. The bicyclist was unable to be located and the call was cleared.
On this date at approximately 5:22pm, this department received a call for a medical emergency at the address of 557 North East Avenue. Vineland Dispatch was advised that the subject was riding a bicycle earlier on this date and he was struck by a vehicle. It was reported that the subject was bleeding from his head and Vineland EMS was dispatched to the scene. Due to the prior incident, Vineland Dispatch believed that this subject was involved in the earlier incident when I was dispatched to the address. Prior to my arrival, I was informed by Vineland Dispatch that Vineland EMS was transporting the subject to Inspira Health Network when I diverted and responded to Inspira Health Network.
At the hospital, I met with Vineland EMS who was escorting the patient (later identified as Oscar Malave) into the emergency room. Malave’s girlfriend (Virgin Martinez) was also with him when she advised me of the following. Martinez said she was at her residence (557 North East Avenue) when Malave arrived at her house. She said he was bleeding from his forehead and nose when he told her he was hit by a car while riding his bicycle in the area of South East Avenue and East Almond Street. Martinez said Malave refused to go to the hospital at that time and she treated his injuries as best as she could. Martinez said after a while, Malave was still in a lot of pain and bleeding when her friend called for an ambulance to respond to the house.
Once Malave was checked by the hospital staff, I attempted to speak with him regarding the accident. Malave said he was riding his bicycle across South East Avenue when he was struck by a car. He said he does not remember anything else from the collision. I asked Malave why he left the scene when he told me that he was scared and ran to his girlfriend’s house. Malave sustained several minor cuts and abrasions on his head, arms and legs which he stated were caused from the collision. I asked Malave where his bicycle is when he said he left it at the scene and he ran on foot. Malave could not provide me with any information for his bicycle. I asked Martinez if Malave had his bicycle when he arrived at her house but she said he did not and she does not have any information for the description of the bicycle.
I then left the scene and responded to police headquarters when I was able to locate the information for drive r#1 when I contacted her via telephone. Speaking with driver #1, she advised me that she was not in the city of Vineland at this time and that she would respond to this department on her way home tonight to speak with me and to provide me with her vehicle information.
Speaking with driver #1, she stated she was traveling north on South East Avenue in the area of East Almond Street. Driver said she observed the bicyclist traveling south on the east side walk of South East Avenue, traveling towards her. Driver said when she approached the bicyclist, he appeared to lose control of his bicycle when he turned into her lane of travel and struck the passenger side of her vehicle. Driver said she immediately stopped when she saw that the bicyclist was bleeding from his face and head. Driver said when she was going to report the incident to police the subject fled the area on foot. I asked the driver for a description of the bicyclist when she said that she only saw that he was an older Spanish or Mexican male with a dark shirt. She could not provide me with any additional information for the bicyclist and she could not give me any description of his bicycle.
The driver said when the subject left the area, she did not have a cell phone when she drove to this department to report the collision. She related when she went back to the scene with Ofc. Scalzi, she was unable to locate the bicycle from the collision.
Malave was issued the following traffic summonses with a court date to appear for May 17th 2016 at 9:00am.

Driver#1 stated that he was traveling west on Industrial Way approaching DeMarco Dr. He said a car was stopped at DeMarco Dr. south bound. Driver #1 said the car suddenly pulled into the intersection turning left (east) on to Industrial Way. Driver#1 said to avoid collision he applied his brake and veered to the right, sideswiping a parked car (vehicle#2). Driver#1 said he was trying to avoid collision and cannot give a description of the driver or the make of the car that pulled out in front of him. He said his forklift was carrying a pallet which was stacked with heavy metal plates. He said the plates are wider than the pallet. Driver#1 said when he veered to the right, the metal plates on the pallet sideswiped vehicle#2. Investigation at the scene revealed that vehicle#1 had moved from point of impact prior to my arrival. Vehicle#2 was parked and was still in the original position of impact. Point of impact was approximately 30 feet east of the east curb line of DeMarco Dr. and approximately 6 feet south of the north curb line of Industrial Way. Driver#1 stated he was not injured from the accident. Vehicle#1 sustained no damage. Vehicle#2 sustained major damage to the left side from the front fender to the rear fender. The owner of vehicle#2 was located at her job, Lucca Freezer. She was informed of the accident and came outside to view the damage to her car. Note that the collision was in front of Lucca Freezer which is where construction was being done. Driver#1 and his vehicle#1 are part of the construction crew (Fabbri Concrete Masonry). Below is the information for driver#1 and the forklift he was driving. Both are part of a construction crew. Fabbri Concrete Masonry 1589 Gallagher Dr. Vineland, NJ 08360. On May 6, 2016, I was dispatched to 543 Park Dr., for a hit and run collision. Upon my arrival I met with the home owner, Craig Kelley, who made the following statements. Craig stated he was in his home sleeping when he heard a loud bang. Craig then went to the front door and opened it to see the 8 inch thick tree that was in front of his house, now lying across his driveway. Craig and I investigated the damage to his property, and observed additional damage to his chain linked fence, as well as the chain linked fence of house number 541 Park Dr. During my investigation, I observed large tire tracks going across the front of the property of 543 and 541 Park Dr, along with debris from the vehicle. I also located a large Ford grille lying on the ground by the fence. I then looked down the road and could see a large trail of oil in the road way. At that time K9-Officer D. Adams followed the trail which led him to a Ford F150 bearing NJ REG: D48FXJ, which was pulled over to the side of the road with its hazard lights on at Maple Ave & Brewster Rd. Upon my arrival to Maple Ave & Brewster Rd, I observed heavy damage to the front of the vehicle which was driven by, Christopher Sosa-Diaz. The Ford F150 had a 6ft fence post impaled into the front of the truck along with some bushes and shrubs. There I performed a series of Sobriety Tests where Christopher was found to be D.W.I. Christopher stated he was driving home from the Oak Tavern after having a couple drinks and took the turn a little too fast. He stated he wasn't sure what he hit and kept going. Christopher was then placed under arrest for D.W.I. and issued the following summonses:

Driver #1 indicated at that time she side swiped Vehicle #2 on the rear driver side bumper. Driver #1 stated, “I thought I had enough room.” Vehicle #1’s passenger side sideswiped Vehicle #2’s rear driver side bumper.
Driver #2 stated he was stopped at the intersection of East Chestnut Avenue and South Brewster Road facing east bound when Vehicle #1 sideswiped his vehicle (#2). Driver #2 stated he did not know what happened as he was stopped at the red light.
Both drivers did not have a complaint of pain.
Vehicle #1 sustained minor damage on the rear driver side bumper.
Vehicle #2 sustained major damage to the passenger side.
Driver #1 was issued summons #0614-VC-010659 for careless driving (39:4-97).

Driver #1: Edith Figueroa 74 of the 700 block of S. West Ave.
On 05/06/2016 at approximately 0906hrs, I was dispatched to the area of East Grape Street / South Sixth Street. Police Dispatch relayed the caller observed a gray Honda bearing NJ UXL77S backed out of a driveway on East Grape Street and backed into a white vehicle bearing unknown registration which was parked across the street. The caller also stated the accused vehicle fled the area west bound on East Grape Street. Police Dispatch advised the caller wish to remain anonymous and left the area.
I began to check the area and located a white vehicle (#2) bearing NJ registration D55FSX on East Grape Street. I noticed damage to the front passenger side fender (above tire). The damage was a dent and it appeared fresh as the center had the dirt removed from it while the surrounding area was still dirty. It should be noted it was raining at the time and has been for several hours prior.
I checked the NJ registration and the registered owner came back to 526 East Grape Street, Apartment A. Therefore, I walked over and knocked on the door. Shortly after knocking, I met with the registered owner of Vehicle #2; an Alphonse Printemps. I advised Printemps of my investigation and the damage on Vehicle #2 that I discovered. Printemps stated he does not remember seeing the damage and suspects someone did in fact strike his vehicle.
While I was on scene with Printemps, the anonymous caller responded back to the area. Speaking with the concerned citizen regarding the collision, he/she (Witness #1) related the following:
Witness #1 was traveling west bound on East Grape Street, when Witness #1 saw Vehicle #1 back out of the driveway to the residence of 524 East Grape Street. Therefore, Witness #1 stopped and let Vehicle #1 continue to enter the roadway. As Vehicle #1 was backing up, it backed into Vehicle #2 which was parked (unoccupied). Vehicle #1 then fled the area and Witness #1 took down the tag number before contacting this police department.
Witness #1 stated she/he knows that Driver #1 was aware of the collision as Driver #1 “looked back” after the collision occurred. Witness #1 could not provide a description of Driver #1 but, stated the driver is either white or Hispanic. Witness #1 described Vehicle #1 as a gray Honda. Witness #1 could not provide anything further.
I provided Printemps with the case number to the collision and responded to the accused registered owner’s address; 753 South West Avenue, Apartment B. It also should be noted, the registered owner of Vehicle #2 is a 74 year old Hispanic female by the name of Edith Figueroa. Upon my arrival, I did not see the accused vehicle (#1) in the driveway and knocked on the door of Apartment B with no answer. I then knocked on Apartment A and met with Gladys Diaz. I asked Diaz who lives in Apartment B at which time she stated an Edith Figueroa with her son. Diaz also stated Figueroa’s son has “down syndrome.” I asked Diaz what type of vehicle Figueroa drives and she stated, “A gray Honda.” Diaz indicated Figueroa is not home at this time and did not have a contact number for her.
I asked Police Dispatch if they could locate a contact number for Figueroa at which time Dispatcher E. Santiago advised we have nothing on file. Investigation pending…
On 05/10/2016, I responded to 753 South West Avenue, Apartment B in an attempt to meet with the driver of Vehicle #1. Upon my arrival, I observed the accused vehicle unoccupied in the driveway. I knocked on the door and met with Edith Figueroa. Figueroa was advised of the incident and why I was there. Figueroa indicated she did back out of a driveway in the area and heard a noise when she backed up. At the time, Figueroa did not know she struck anything and drove off.
Looking at Vehicle #1, there was damage to the rear passenger side bumper but according to Figueroa, that is old damage. Figueroa was issued summons #0614-VC-10622 for improper backing (39:4-127).

Driver #1 stated she was traveling south bound on South Delsea Drive approaching West Sherman Avenue when she stopped in traffic as she was attempting to make a left hand turn into McDonalds (2707 South Delsea Drive). Driver #1 indicated there was a vehicle (X) traveling north bound that stopped in the roadway and “waved for her to go.” Vehicle X reportedly waved hastily and Vehicle #1 did not initially make the turn. Driver #1 stated, “He (Vehicle X) was blocking a lot of cars so I went.” When Vehicle #1 made the left turn, Vehicle #2 traveling north bound in the eastern lane struck Vehicle #1 head on.
Driver #2 stated he was traveling north bound on South Delsea Drive crossing over West Sherman Avenue in the most eastern lane. Driver #2 indicated he passed a vehicle (X) to merge from two lanes into one. Driver #2 stated it was at that time he struck Vehicle #1 head on. Driver #2 stated Vehicle #1 turned in front of him and was unavoidable. Driver #2 advised he tried to stop but, “did not see her.”
Both drivers and their corresponding passengers did not have any complaint of pain.
Vehicle #1 sustained moderate damage to the front passenger side bumper area.
Vehicle #2 sustained moderate damage to the front end.
Driver #1 was issued summons #0614-VC-010660 for careless driving (39:4-97). It should be noted Vehicle X left prior to my arrival and I could not speak with that Driver.

On Saturday May 7th, 2016 I was dispatched to 790 Fox Lane in regards to a damaged car and mail box. Upon my arrival I met with the residence of 790 Fox Lane, Tamika Garrett who said that her father’s vehicle (vehicle 2) was damaged overnight and so was her neighbor’s mailbox (711 Fox Lane). After speaking with Garrett her father Isaac Simpkins came out and said that he parked his vehicle in his daughter’s driveway last night and it was in perfect condition. Simpkins said this morning he came outside for an unrelated reason and saw that the back passenger side tail light on his vehicle was smashed and the rear bumper on the same corner had fresh scuff marks on it. After speaking with Simpkins and Garrett I walked across the street and examined the mailbox at 771 Fox Lane. I could see that the mailbox was struck by a vehicle due to tire marks preceding the mailbox which indicated the vehicle hit the car at 790 Fox Lane first then proceeded to the mail box. After reviewing the report made by Simpkins and the evidence at the scene I quickly related this incident to a two car collision I handled shortly before this investigation. It involved a female identified as Alexandra Harr who rear ended a vehicle on Main Road and was transported to her residence which is Located at 740 Streamview Lane, which is in the same neighborhood that this investigation is taking place in. I back tracked the potential path of the accused vehicle towards Briar Trail and located a front license plate bracket with New Jersey registration T55FHW screwed in it laying in the roadway in front of 1517 Briar Trail which is also the intersection of Briar Trail and Streamview Lane. I knew the registration belonged to Harr's vehicle because I was familiar with it from the prior collision report, see case number 2016-26879. I spoke with the resident of 1517 Briar Trail, Lynn Elliott who said her children's portable basketball hoop that was in front of her house was struck. Elliott valued the basketball hoop at $349.00. Next I responded to Harr's residence and was not able to illicit a response. After numerous attempts I finally made contact with Harr who gave a very vague statement regarding the collision. Harr admitted to hitting the basketball hoop and mailbox but said she did not realize she hit the car. Harr told me that she was up all night due to her Bi-Polar disorder. I asked Harr about her medications and she said that she only takes what is prescribed to her and nothing that would affect her ability to operate a motor vehicle. As a result of this investigation Harr was issued summons number 0614-VC-008456 for violation of 39:4-97 Careless Driving and 0614-VC-008457 for violation of 39:4-129 Failure to Report.
