Driver #1: Alfredo Hernandez-Flores 47 of the 700 block of E. Butler Ave.
On April 10, 20016, I was dispatched to South Main Road and East Butler Avenue in regards to a one car collision. Upon my arrival, I observed Vehicle # 1 a Honda Odyssey LX, Minivan, Gold with PA Registration KCA7208 with extensive damage on the front area. The utility pole (#VE8149) was split in half and the electric wires was laying on the roadway (Butler Avenue). The Vineland Fire Department and Vineland E.M.S were also at the scene.
Driver # 1, Alfredo Hernandez Flores stated he was driving east on East Butler Avenue when he lost control of his vehicle and struck utility pole number VE8149.
Driver #1, suffered a superficial scratch on his right hand and complained of pain on his chest. Driver # 1 was transported by Vineland EMS to Inspira Medical Center, 1505 West Sherman Avenue for further Medical Treatment.
Driver # 1, Alfredo Hernandez Flores was issued with the following traffic summonses 0614-VC-010151, Careless Driving 39:4-97 and 0614-VC-010152, unlicensed driver, 39:3-10.
Vehicle # 1 sustained damage on the front area and it was towed from the scene by Giancola Towing. Vehicle # 1, Insurance Information: Access Insurance Company, OB. Box 105866, Atlanta GA 30348 Policy Number: APA003753979 Phone number: (215)745-6900 P.O.I ( Point of Impact) was approximately 75 feet east of the east curb line of South Main Road and approximately 4 feet south of the south road edge of East Butler Avenue.
Angel Conde Jr of 3221 South Main Road #1, stated the electrical wires were pulled away from his residence due to the accident. The Cumberland County Road responded to the scene and calculate the damages (See Supplement compiled by Officer T. Hall for further details).
A DWI report was completed by this officer due to driver # 1 was suspected to be under the influence of alcohol beverage (See DUI report). Charges pending DWI at this time.

Driver 2 stated she was traveling South on N Main Rd when driver 1 did not stop at the stop sign for Fairmount Ave and hit the side of her vehicle causing her vehicle to flip onto the roof at which time this collision occurred. Vehicle 2 sustained heavy damage to the roof, hood and both drivers side and passenger side of the vehicle. Vehicle 1 was removed from the scene by Joe's Bug Repair. Driver 3 stated she was traveling South on N Main Rd behind driver 2 when driver 1 failed to stop at the stop sign on Fairmount Ave hitting driver 2 and causing her vehicle to flip over on to his roof. Driver 3 stated she was unable to stop in time and struck vehicle 1 very soft at which time this collision occurred. Vehicle 2 sustained minor damage to the front passenger side bumper and was driven away from the scene. Driver 1 stated he struck driver 2 but doesn't really know what happened. Driver 1 was later determined to be intoxicated and arrested for DWI see DWI report for further information. Vehicle 1 was removed from the scene by South Side Auto Body at which time they were also advised to place a 12 hour hold on same. The following summons were issued to driver 1:

Driver #1 stated he was leaving the Vineland Public Library (1058 E. Landis Avenue) making a left turn onto East Landis Avenue when he struck Driver #2. Driver #1 stated he checked the lanes for any vehicles but didn't see anyone. Driver #1 stated he was unable to stop his vehicle when he saw Driver #2 due to the weather conditions. Driver #1 claimed no injuries while on scene and declined medical attention. Driver #1's vehicle sustained moderate damage to the front end bumper and was able to be driven from the scene. Driver #2 stated she was traveling east on East Landis Avenue when Driver #2 pulled out of the driveway of the Vineland Public Library and struck her vehicle. Driver #2 stated she then contacted this department. Driver #2 claimed no injuries while on scene and declined medical attention. Driver #2's vehicle sustained moderate damage to the driver's side door and was able to be driven from the scene.
No summons issued at this time.

Driver #1: Kyle Wynder 38 of the 2000 block of Mayslanding Road
Driver # 1 advised he was north bound on Snyder Avenue, approaching Mays Landing Road, when he suddenly lost control of his vehicle, leaving the west side of the roadway, partially rolling over, and then coming to rest onto the property of 5346 Snyder Avenue. When questioned about the collision, Driver # 1 advised me his vehicle may have suffered some type of malfunction causing him to lose control. Driver # 1 denied any pain relating to this collision. Driver # 1 advised he contacted his own tow service and they would be en route to the crash to retrieve the vehicle. Driver # 1 was unable to provide me with a name for the tow service. I then spoke with a person who resides at 5346 Snyder Avenue, Brandi Martin. Ms. Martin advised me she was aware of the crash that occurred on her property. Ms. Martin was advised that she could contact the insurance company representing vehicle # 1 for the damage done to her property. Investigation revealed that Driver # 1 failure to maintain control of his vehicle was the cause of this crash. At this time of this investigation, I was unable to determine if vehicle # 1 suffered from any vehicle defects prior to the crash which could have contributed to this collision. Driver #1 was cited for careless driving.

Details: Driver#1 stated she was traveling north bound on East Avenue when she stopped in traffic. Driver#1 stated while stopped in traffic, Driver#2 rear ended her vehicle causing the collision. Driver#2 stated she was traveling north bound on East Avenue behind Driver#1 when she turned around to check on her dog who was in a crate in the back seat. Driver#2 stated when she turned back around a moment later, she saw Driver#2 stopped in traffic. Driver#2 stated she did not see Driver#1 stopped in traffic until the last moment causing her to rear end her vehicle causing the collision. Driver#2 was issued summons #VC10419 for Careless Driving (39:4-97). Driver#1 was transported to InSpira Health for complaint of pain to her chest. Both vehicles sustained minor to moderate damage.

On this date, I was dispatched to the area of Mill and Garden Road in regards to a collision. The caller reported a male subject got into a collision and left the scene. The caller described the subject as a male having a black in color shirt walking east on Garden Road. A short time later I arrived on scene. On my way to the location, I took Delsea Drive and turned west onto Garden Road after being advised the subject might be walking toward that direction. I did not see any foot traffic as I traveled west. I arrived the intersection to find a grey in color vehicle on the property of 1872 W Garden Rd. It was inside of a large bush, almost not visible from the roadway. Although difficult to get to I was able to see the vehicle was not occupied. There was a subject/witness #1 on the side of the road. Witness #1 stated that he was driving and saw the vehicle inside of bush. Witness#1 stated a male subject walked out of the vehicle and just started walking away from the collision. Martin stated the subject did not run he "just walked away". He described the subject as wearing a dark in color shirt with a stocky build. Martin stated he tried to maintain visual of the subject as he walked away but it was too dark. He was not able to get a good look at the subjects face for identification. Also on scene was another Witness#2. Witness #2 stated she was in her residence when she heard a noise. She went outside to find that there was a vehicle in her yard. Witness#2 stated she walked over to see of anyone was in the vehicle and noticed a Hispanic male in the driver's seat. Witness #2 stated she went over and even assisted the driver out of the vehicle. She stated the vehicle seemed to be smoking so fearing of a fire, she reached in and shut the vehicle off. The subject at that time stated he was looking for his phone and was combative. He then exited the vehicle and left walking. Witness#2 stated she got a good look at the subjects face and was able to identify him if she saw a photo. She described him as a Hispanic male, with facial hair and about 5-08" or 09" wearing a dark shirt, possibly black. An inquiry on the tag revealed the registered owner to the vehicle to be a David Figueroa. Olin's Tow responded to the scene and pulled the vehicle out. Once it was pulled out to a safe location, I was able to check the vehicle safely for any paperwork. Inside of the vehicle, I located the registered owners, David Figueroa's Bank of America Visa debit card. This card contained a photo of the subject. There was also the vehicle registration card. On the passenger side floor, I located a black in color iphone6. This phone continued to ring as I was on scene conducting my investigation. I obtained the phone believing that this may be the owner's. Because witness #2 stated she had direct contact with the subject that was in the vehicle, I showed her the photo on the debit card and concealed the cards name and identification number as I showed her the photo. Witness # 2 positively identified the subject as the subject that was in the vehicle. She pointed out that the subject had visible facial hair. The vehicle had extensive damage to the front driver's side due it hitting the curb edge prior to entering the property of 1872 W Garden Rd. The driver's side front wheel was canted After the vehicle was towed I advised Witness # 2 of her options due to the damage on the property caused by the vehicle. I then responded to headquarters. Once at headquarters, the cell phone I obtained continued to ring. Although the phone was locked I was able to answer calls. I received a call from a male who did not identify himself. I explained the situation and asked him if he can get in contact with David Figueroa. The male stated David was his cousin but doesn't know where he would currently be at. I attempted to get this males subject names but he hung up on me. Several more calls came in but I repeatedly got hung up on when I asked about David and identified myself as a police officer. Several hours later, David Figueroa suddenly showed up at police headquarters. He asked where his vehicle was. He was very uncooperative and did not wish to tell me how he found out about his vehicle and to respond to police headquarters. I immediately noticed his facial hair as the witness described. David stated he was never driving. He stated he was with his friend and lent his vehicle to a "Jay". I told him if this was the case to provide me with "Jays" full name and where does he live. He stated he did not wish to provide me with it and said, "I don't want to talk". A short time later a female walked into headquarters. She stated she was the mother of David. This female stated David was with her the whole time. I asked David why did he state that he was with his friend. He became upset again and stated he lives out of his vehicle. This female also stated that she did not know David's phone number. I found this odd since she stated she was the mother. His statements were very inconsistent. I showed him the phone I located and told him if it was his. He stated, "No". Based on my investigation, I issued David Figueroa the following summons: VC-005512 (Leaving Scene Accident) VC-005513 (Failure to report), (Uninsured MV) VC-005515 and (Reckless Driving) VC-005514. The cell phone was tagged and placed in evidence locker #78. I made a copy of David's Visa card which contained the photo and his registration card. I then turned those items over to him. On the next work day, I was advised by Dispatcher E. Santiago that a male subject repeatedly called police headquarters in order to attempt to know how retrieve his iPhone. Dispatcher Santiago stated the subject refused to provide his name and became upset when asked to identify himself.

Driver # 1 she was east on East Landis Avenue, approaching East Landis Avenue. Driver # 1 advised the traffic light controlling her lane of travel was green. As Driver # 1 entered the above mentioned intersection, she stated vehicle # 2 disregarded a red traffic light controlling its lane of travel, entering onto the intersection and causing this crash. Driver # 2 stated that he was south on North Valley Avenue, approaching East Landis Avenue. Driver # 2 stated the traffic light controlling his lane of travel was green. Driver # 2 stated that as he entered the above intersection, vehicle # 1 disregarded a traffic light controlling its lane of travel, causing the crash. A witness to the collision, Diana L. Ganci, advised she witnessed the crash. Ms. Ganci stated she was south bound on North Valley Avenue, travelling behind vehicle # 2. Ms. Ganci stated she observed the traffic light controlling vehicle # 1 lane of travel turned from yellow to red, as vehicle # 1 entered the intersection. Ms. Ganci said Driver # 1's disregard of the red traffic signal controlling its lane of travel was the cause of this crash. A passenger inside vehicle # 1 complained of pain in her lower leg and face from this crash. A passenger inside vehicle # 2 complained of pain in the left shoulder. Vineland EMS responded to the scene for the reported injuries. Vehicle # 2 passenger was transported to Inspira Hospital, 1505 West Sherman Avenue. After speaking with all persons involved, it was determined that Driver # 1 was the cause of this crash. Driver # 1 was issued summons # 0614 VC 010589 for failure to observe traffic signal.

Driver #1 was traveling west on West Elmer Road when she stated she didn't see Driver #2's turn signal due to the sunlight and rear ended the vehicle. Driver #1 stated she then pulled over to the shoulder lane and checked on Driver #2. Driver #1 claimed no injuries while on scene and declined medical attention. Driver #1's vehicle sustained moderate damage to the front driver's side bumper. Driver #1's vehicle was towed from the scene by J& C Auto Towing Company due to her vehicle being unregistered. Driver #2 was traveling west on West Elmer Road when she stopped to make a left turn into her workplace when Driver #1 struck her vehicle from the rear. Driver #2 stated Driver #1 had been following too closely for a length of time before the collision. Driver #2 claimed no injuries while on scene and declined medical attention. Driver #2's vehicle sustained moderate damage to the rear bumper on the passenger side. Driver #2's vehicle was able to be driven from the scene. Driver #1 was issued traffic citation 0614-VC-010039 for Unregistered Vehicle (39:3-4).

Driver#1 stated she was stopped in traffic heading north bound on Delsea Drive just before Park Avenue when Driver#2 rear ended her vehicle causing the collision. Driver#2 stated she was stopped in traffic behind Driver #1 when she thought Driver 1 began to move forward when she rear ended her vehicle causing the collision. Driver#1 had complaint of pain to her back, but refused medical treatment from EMS. Driver#2 was issued Summons # VC10420 for Careless Driving in violation of NJS 39:4-97. Minor damage to both vehicles. No other injuries were reported.

On April 13, 2016, I was dispatched to Wawa, located at 501 North Delsea Drive, in regards to a motor vehicle collision. Upon my arrival on scene, driver #1 stated while backing up, traveling in a North direction, she looked prior to backing up and did not see vehicle #2 behind her. Driver #1 was further upset and said that driver #2 was making this collision seem a lot worse than what it was. While talking to driver #2, he stated that he was traveling West in the parking lot. Driver #2 said that he saw a friend of his and stopped to talk. Driver #2 said he then noticed that driver #1 was backing up which he blew the horn prior to driver #1 hitting his vehicle. Driver #2 complained of neck pain. At this time, I requested that Vineland Emergency Medical Service respond to the scene. Upon their arrival, driver #2 refused medical treatment at this time. No other injuries were reported. During this investigation, driver #2 reported that vehicle #1 damaged the back and driver side rear bumper areas. Also, driver #2 reported that his driver side rear tail light was damaged from this collision. Upon checking, broken pieces from the tail light was unable to be found. The damage on the back of the bumper also did not match up and appeared to be unrelated to this collision. Driver #2 was advised of my findings. However, both vehicles did in fact sustain visible/minor paint transfer damage during this incident. I further met with a male subject who identified himself as Bruno Sansalone (Friend of driver #2/Witness). Sansalone stated while walking in the parking lot area, driver #2 stopped to talk to him and driver #1 hit his vehicle. No traffic summons(s) issued.

Driver 1 stated he was travelling south on S East Ave approaching the intersection of S East Ave and Fava Dr. Driver 1 stated as he was travelling, a turkey flew into the roadway flying directly towards his vehicle. Driver 1 stated he applied his brakes as the turkey then turned left, striking the passenger side view mirror. As a result, the mirror broke away from the vehicle. Driver 1 claimed no injuries at the scene. The vehicle was able to driven from the scene.

Driver 1 stated he was approaching the intersection of S Main RD and E Chestnut Ave. Driver 1 stated he heard sirens of an emergency vehicle but could not see where it was coming from. Driver 1 stated he was slowing down to begin to pull over while at the same time looking through his mirrors and around the immediate area searching for the emergency vehicle. Driver 1 stated while he was looking around he struck the rear bumper of vehicle 2. Driver 1 claimed no injury as a result of this collision. Damage to vehicle 1 was moderate. Driver 2 stated she was approaching said intersection and heard audible sirens of an emergency vehicle. Driver 2 stated she was in the right turning lane of E Chestnut Ave to turn onto S Main RD. Driver 2 stated when she heard the sirens approaching from the north bound traffic of S Main RD she came to an immediate stop at which time vehicle 1 struck her vehicle in the rear bumper. Driver 2 claimed no injury as a result of this collision. Vehicle 2 had moderate damage to the rear bumper. It is the determination of this officer that other factors were involved in regards to this collision and no summons to be issued at this time.

Upon my arrival I observed a truck which struck 2 parked cars. I spoke to driver 1 who immediately began giving me attitude when asking for his license, registration, and vehicle insurance. Driver stated when he was leaving his parking spot his vehicle steering tightened up causing him to strike the two parked vehicles. Driver 1 stated he wanted to call his lawyer at that time and did not want to answer any further questions and was being very uncooperative. Driver 1 was issued two summons, one for careless driving 0614VC008558 and improper mounting of license plate, 0614VC008559. Note: I checked the surrounding area of the vehicle and asked if the driver had the license plate inside of the vehicle which he stated he did not. Vehicle 2 had moderate amount of damage to the front of the vehicle and was towed by All Tow towing. Vehicle 3 had a moderate amount of damage to the rear passenger side of the vehicle causing the axle to bend and was towed by Olin’s Towing.

On this date, 04/14/2016, I was dispatched to the area of Brewster and Landis Avenue in regards to a (1) vehicle collision report. The caller, Driver #1 stated she was involved in a collision at her neighbors' fence. She advised dispatch that her vehicle was still there. I was initially dispatched to 2642A E Landis Avenue (residence of Driver #1). Once I arrived to the location, the driver, Sheri Berman came out of her apartment. At this moment I did not see the vehicle involved in the collision. I am familiar with her and knew she was an older female who frequently drinks alcohol. I spoke with Sheri who was clearly upset. She stated she wanted to go to the Wawa located on S Brewster Road to buy some cigarettes. Once she left her driveway she turned and she hit her neighbor’s fence. She pointed across the street. Across the street was the property of 2649 E Landis Avenue. This property sits on the southwest corner of the intersection. Her vehicle seemed to have left the roadway while it was on Landis Avenue and turned extremely early southbound onto the property. It struck a wooded cedar fence that blocked the Landis Avenue side of the road then continued south on the property and struck a small maple tree. The vehicle then reversed and struck the wooded cedar fence which blocked the Brewster side of the road. It seemed she attempted to leave by going through this side of the fence however her vehicle became disabled before getting back onto the roadway. Sheri did not have any complaint of pain or visible signs of injury. She refused medical attention. Her vehicle sustained damage to the front end of the vehicle. It also had two front flat tires. JC Tow responded to the scene and removed the vehicle from the roadway. The damage to the property of 2649 E Landis Avenue consisted of approx. 2 sections of the cedar post fencing and damaged grass. The small maple tree, although struck was not damaged. The owner of the property, Salvatore Serra was advised of the incident. Sheri could not explain why she made this turn onto the property. I evaluated her and at this moment I did not suspect her to be under the influence of any alcohol and or drugs. Sheri was issued her traffic summons # VC-05516 (Careless Driving) and advised a police report will be completed for the incident.

Driver #1 stated he was traveling east bound on Maple Avenue when he came to a complete stop at North Brewster Road. Driver #1 indicated he looked both ways and observed a bus (WITNESS #1) traveling north bound and Vehicle #2 traveling south bound on North Brewster Road. Driver #1 advised he thought he had sufficient time to get through the intersection and proceeded straight. Driver #1 stated Vehicle #2 struck his vehicle (#1) at a right angle on the rear driver side. Driver #1 stated, “She (Driver #2) was going really fast.”
Driver #2 stated she was traveling south bound on North Brewster Road approaching Maple Avenue when Vehicle #1 “ran” the stop sign and caused a collision. Driver #2 stated she could not avoid Vehicle #1. Driver #2 stated if she tried avoiding Vehicle #1 in the opposite direction she would have struck a bus traveling north bound.
Witness #1 provided a statement that corroborated with Driver #2. Witness #1 stated Vehicle #1 never came to a complete stop and “ran” the stop sign.
It should be noted after Vehicle #1 was struck it struck the fire hydrant located at the southeast corner of the intersection. The Water Utility was contacted regarding the struck fire hydrant.
Driver #1 had a complaint of pain to his left shoulder, arm and leg. Driver #1 also had a complaint of pain to his head. Emergency Medical Service (EMS) responded to the scene and Driver #1 went with them to the hospital to get further evaluated.
Driver #2 had a complaint of pain to both her wrists and hands. Driver #2 also had a complaint of pain to her left shoulder. Driver #2 signed a refusal form and did not go with EMS to the hospital.
Vehicle #1 sustained major damage to the rear driver side and was towed by All Tow.
Vehicle #2 sustained major damage to the front end and was towed by J&C Auto.
Driver #1 was issued summons #0614-VC-010516 for failure to stop (39:4-144). Driver #2 and Witness #1 were issued a subpoena with this report.
