On 3/7/16 I was dispatched to West Sherman Avenue/Wilson Avenue for a 2 car collision. Upon arrival I asked both Parties if they were injured and needed Vineland EMS to respond, they responded "No". I then proceeded to talk to each driver separate. Driver 1- Gabriella Rivera stated she was driving west bound down Sherman Avenue when she passed Driver 2 on the right side. Rivera stated that she was able to make it completely in front of driver 2, but suddenly the BMW that was in front of her stopped. Rivera came to a sudden stop as well which is when she states driver 2 hit her and spun her vehicle the opposite direction facing east bound on Sherman. Rivera stated that she hit her head on the driver’s side window at which time her parents decided to take Rivera to urgent care. Driver 2- Donald Wilson stated that he was driving west bound down Sherman Avenue when Rivera tried passing him on the right side. Wilson stated Rivera was not able to make it all the way at which time he hit her vehicle on the driver’s side with his right front end and Rivera's car spun now facing East on Sherman Ave. Wilson had no complaints of pain and no signs of visible injury. Driver 2's insurance company # 18023 and Policy # CA0273749 A summons was issued to Rivera for Improper passing (39:4-85). Summons number 0614-VC-009470.

On March 11, 2016 at approximately 1:28am I responded to 2159 Sunset Ave, for a motor vehicle crash into a fence.
Upon my arrival, I observed a burgundy Honda Accord Wagon, approximately 15 feet south of Sunset Ave off the roadway on the property of 2159 Sunset Ave. I also observed several fence panels that were broken lying next to the vehicle from the vehicle driving through them. The driver of the vehicle, Emmanuel Ayala, was standing in the roadway with his hands on a police cruiser while we investigated the crash.
I spoke to Emmanuel Ayala and asked him what happened tonight, and he was uncooperative, and did not wish to speak to me. I could immediately smell the odor of an alcoholic beverage emanating from his breath. At that time I asked Emmanuel if he would be willing to take a series of Standard Field Sobriety Test’s (SFST’s), which he declined to take. At that time he was placed under arrest for DWI and transported to the Vineland Police Department. See case report #16-14710 for further details.
Moises Afanador of 2159 Sunset Ave, did not wish to sign any complaints or make any report of the damage to his fence at this time.
As a result of this investigation, Emmanuel Ayala was issued the following motor vehicle summonses and given a mandatory court date of March 28, 2016 at 09:00am: 0614-VC-009850 39:4-50/DWI 0614-VC-009851 39:4-50.4a/REFUSAL 0614-VC-009852 39:4-96/RECKLESS DRIVING 0614-VC-009853 39:3-40/DRIVING WHILE SUSPENDED 0614-VC-009854 39:6B-2/UNINSURED MOTOR VEHICLE 0614-VC-009855 39:4-88/ FAILURE TO MAINTAIN LANE
Emmanuel didn’t complain of any injuries at this time. His vehicle sustained minor to moderate damage to the front bumper and undercarriage, and was towed from the scene by Rulyn’s Garage.

Driver #1 stated that she was driving North on South Main Road when driver #2 came to a stop in front of her. Driver #1 stated that she could not react in time to stop and hit driver #2 in the back bumper with her front bumper. Driver #1 claimed no injuries and there was moderate to heavy damage to the front of vehicle #1. Vehicle #1 was towed from the scene by Larcher’s Body Shop. Driver #1 had a 3 year old passenger sitting in his car seat in the back passenger side and he claimed no injuries. Driver #2 stated that she was driving North on South Main Road when traffic in front of her started to stop. Driver #2 stated that when she came to a stop that is when driver #1 ran into the back of her. Driver #2 claimed no injuries and there was minimal damage to the bumper of vehicle #2. Driver #2 was issued no summons. Driver #2 drove her vehicle from the scene. I issued the following summons for driver #1 0614-VC-009294 for careless driving.

Driver #1 stated she was turning into her residence (1976 N East Ave #64), when she felt impact to the rear of her vehicle. Driver #1 stated Driver#2 hit her vehicle with his motorcycle. Driver#1's vehicle sustained moderate damage to the rear passenger’s side quarter panel. Driver#1 had no complaints of pain or injuries on scene. Driver#2 stated he was riding his motorcycle north in the trailer park and did not see Driver#1 pulling into her residence. Driver#2 stated he collided with the rear of Driver#1's vehicle. Driver#2's motorcycle sustained moderate front end damage. Driver#2 had no complaints of pain or injuries on scene. Driver#2 was subsequently arrested for DWI 39:4-50. Driver#2's motorcycle was towed from the scene by Rulyn's and placed on a 12 hour hold. Driver #2: Megan Mortillite 17 of the 700 block of Florence Ave.
Driver #1, Guarionex Reyes, stated he was traveling south bound on S State St, approaching the light at E Chestnut Ave. Reyes stated he stopped at the light and waited for it to turn green. Reyes indicated while waiting for the light to change, he observed Driver #2's vehicle approach the intersection in the opposite direction and stop at the light. The light finally turned green and Reyes related he began to cross over E Chestnut Ave. As he got into the center of the intersection, Reyes stated driver #2 pulled out into the intersection as well and began to turn, and struck his vehicle on the driver rear door. Reyes indicated he observed out of the corner of his eye driver #2 beginning to turn into him, but was unable to avoid the collision. Driver #2, Megan Mortillite, stated she was traveling north bound on S State St, approaching the light at E Chestnut Ave. Mortillite indicated the light was red so she stopped and waited for the light to turn green. Mortillite stated she observed driver #1's vehicle at the light in the opposite direction, waiting as well. Mortillite related the light finally turned green and she was waiting for driver #1 to cross the intersection since she had her left signal on, signaling that she wanted to turn left onto E Chestnut Ave. Mortillite indicated she was waiting for driver #1 to cross E Chestnut Ave, but he didn't move from his stopped position at the light. After waiting a couple seconds, Mortillite stated she then pulled out into the intersection and began to turn. During her turn, Mortillite related that driver #1 then quickly began to accelerate into the intersection causing her vehicle to strike his vehicle. Both vehicles sustained minor to moderate damage, and none of the parties complained of any injuries at this time. Driver #2 was issued summons 0614-VC-009857, 39:4-90 Failure to yield at intersection.

Driver 1 stated he had stopped at the stop sign at the intersection of N Seventh St and E Peach St. Driver 1 stated he observed Driver 2 travelling north towards the intersection and thought he was going to continue travelling north through the intersection. Driver 1 stated he began to pull into the intersection when he felt the way was clear and struck the driver side of Driver 2's vehicle with the front bumper of his vehicle. Driver 1 claimed no injuries at the scene. Driver 1's vehicle sustained no visible injury to his vehicle. Driver 2 stated he was travelling north on N Seventh St approaching the intersection of N Seventh St and E Peach St. Driver 2 stated he was making a left turn onto E Peach St and observed Driver 1 come into the area of the intersection. Driver 2 stated that as he attempted to make the left turn he was struck by Driver 1 on the driver side door of the vehicle. Driver 2 claimed no injuries at the scene. Driver 2 sustained moderate damage to the driver door (dent/paint transfer). Driver 2 was able to drive his vehicle from the scene.

Driver #2: Michael Weeks 42 of Linwood NJ
I spoke with Driver#1 who related he was traveling West on East Landis Ave. He was stopped at the red traffic signal on East Landis Ave and Valley Ave and thought the light had turned green. He stated he then suddenly collided with vehicle #2, and stated he could not stop in time. I then spoke with Driver# 2 and he stated he was stopped for the red traffic signal. As he was stopped, he stated he was collided into from behind. As a result of the collision, none of the occupants were injured. The damage to vehicle #1 consisted of damage to the front portion of the vehicle. Driver of this vehicle called to have his vehicle towed at his own expense by a private towing company. The damage to vehicle #2 consisted of minor damage to the back portion of the vehicle.
No witnesses were located at the scene. A summons (#VC-009572) was issued for driver #1 for careless driving. A subpoena was issued to Driver #2.

Dispatched to 3849 S. Delsea Drive in regards to a collision, I was advised the location occurred on Hennis Road, near Home Depot. At the scene I spoke with Driver #1 who stated he was at the stop sign and proceeded into the roadway when another unknown vehicle cut him off and he struck vehicle #2. Driver #2 stated she was traveling west on Hennis Road when vehicle #1 struck her vehicle. Driver#2 complained of pain to her neck. Emergency Medical Service was contacted and responded. EMS #627 transported driver#2 to Inspira Health Network Hospital for further observation. Driver#1 was issued summons number # 0614 V 063207 for failure to stop, 39:4-144. Subpoenas were submitted with this report. Driver #2 later advised she had witness information which she did not provide at the scene. The name of the witness was "Rolon" and the contact phone number 856-332-7571, I attempted to contact the subject but there was no response so I left a voice mail. Later, Evelyn Rolon, witness contacted this department regarding the collision. Rolon advised me she was in the area when the collision occurred and she observed driver #1 stopped at the stop sign but he may have misjudged driver #2 distance when he proceeded into traffic and collision occurred. She related driver #2 was not speeding.

On March 12, 2016, I was dispatched to 4186 East Sherman Avenue, in regards to a Hit and Run collision. Upon my arrival at the residence, I met with a Lois Smaniotto (Property owner). Smaniotto reported on 03/11/2016 at approximately 8:30PM, an unknown driver entered her property. Upon backing up, traveling in a North direction, the unknown driver struck a driveway wood post and metal security swing arm/gate in the driveway. On this date, 03/12/2016, upon checking, Smaniotto noticed that her property was damaged and contacted the police. Smaniotto further said the car was possibly gray in color and had no additional identifiers. Upon checking, no debris was found at the scene from vehicle #1. However, the metal swing arm appeared to have a white and/or gray colored paint transfer on the end of it. During this incident, a wood post and metal swing arm were damaged. Date: 3/12/2016

Driver #1 stated she was making a left turn from South Delsea Drive onto West Chestnut Avenue when she was struck by Driver#2. Driver #1 stated she didn't see Driver #2 when making the left turn onto West Chestnut Avenue. Driver #1 stated she pulled into the Riggins Gas station and waited for police arrival. Driver #1 claimed no injuries while on scene and declined medical attention. Driver#1's vehicle had minor damage rear passenger fender and was able to be driven from the scene. Driver #2 stated she was traveling north on South Delsea Drive when Driver#1 turned in front of her when she struck Driver #1. Driver #2 stated she pulled over on the east side of South Delsea Drive and waited for police arrival. Driver#2 claimed no injuries while on scene and declined medical attention. Driver#2's vehicle had moderate damage to the front passenger side bumper and was able to be driven from the scene.

Driver # 1 advised she was west bound on Roberts Boulevard, approaching Glenn Terrace. Driver # 1 stated she was searching for a specific residence on Glenn Terrace, but was unsure if she was going to turn left or right onto Glenn Terrace. Driver # 1 said that when she reached the above mentioned intersection, she determined that she needed to execute a left turn. While in the process of executing her turn, Driver # 1 stated that vehicle # 2 passed her on the left causing both vehicles to collide. Driver # 1 admits that prior to starting her turn, she failed to activate her left turn signal. Driver # 2 stated that she was west bound on Roberts Boulevard, travelling behind vehicle # 1. Driver # 2 stated she observed vehicle # 1 slowdown in traffic, but she believed vehicle # 1 was going to pull to the right side of the roadway. Driver # 2 sated that she decided to pass vehicle # 1 on the left, but while doing so, vehicle # 1 began to make a left turn onto Glenn Terrace. Driver # 2 advised me vehicle # 1 did not have its left turn signal activated prior to making its turn. Investigation revealed that Driver # 1 failure to signal her turn was a contributing factor to this crash. Driver # 1 was issued summons # 0614 VC 009922 for failure to signal turn. Driver # 2 also appeared to play a role in this crash. Driver # 2 was issued summons for improper passing of vehicle # 1, summons # 0614 VC 009923, and failure to comply with the New Jersey Graduated Driver's License requirement for the number of occupants inside her vehicle, summons # 0614 VC 00924.
