Driver #1: Ashley Petitie 24 of the 990 E. Oak Road

Driver #2: Edith Ruiz 20 of the 1900 block of N. East Ave.

Driver#1 stated she was traveling north bound on Orchard Road when Driver#2 disregarded the stop sign at the intersection of Elm Road striking her vehicle and causing the collision.

Driver#2 stated she was traveling west bound on Elm Road when she stopped at the stop sign at the intersection of Orchard Road.  Driver#2 stated she proceeded to make a left turn south bound on Orchard Road when Driver#1 struck her vehicle.  Driver#2 stated she did not have a NJ Driver’s License.

Driver#1 had complaint of pain to her neck, but refused medical treatment from EMS.  Both vehicles sustained moderate damage.  Driver#2 was issued the following summons:  VC-00008016 39:4-144 for failure to stop and  VC00008017  39:3-10 unlicensed.

Driver #2: Ramon Irurita 21 of the 600 block of E. Almond Street

Driver #1 stated she was at the intersection of North Delsea Drive/ West Park Avenue facing south bound waiting in the turn lane (left turn). Driver #1 indicated the traffic signal was green at the time but, she was awaiting traffic to pass as she was several vehicles back waiting to make a left hand turn. Driver #1 stated after waiting briefly she decided to change lanes and go straight on North Delsea Drive (south bound). Driver #1 stated at that time she struck Vehicle #2 which was already in the lane going straight. Driver #1 stated, “I looked in my mirror before I went but, did not see him.”  

 

Driver #2 stated he was traveling south bound on North Delsea Drive approaching West Park Avenue when Vehicle #1 changed lanes from the turn only lane and collided with his vehicle (#2). Vehicle #1’s front passenger side fender struck Vehicle #2’s front driver side fender.   

 

On scene, no one had a complaint of pain.

 

Vehicle #1 sustained minor damage to the front passenger side fender.

 

Vehicle #2 sustained minor damage to the front driver side fender.

 

Driver #1 was issued summons #0614-VC-009687 (39:4-88b; Unsafe lane change). Driver #2 was issued a subpoena with this report.

Driver #2: Cindy Kruise-Downey 56 of the 1000 block of Junior Drive

Driver 1 stated that she was traveling west on Oak Road when her vehicle was struck by vehicle 2. Driver 1 added she was stopped at the stop sign, waiting for another vehicle to proceed through the intersection, before she proceeded. When she was half way through the intersection her vehicle was struck.

 

Driver 2 stated at the scene that she was traveling south on Brewster Road when she struck vehicle 1. Driver 2 indicated that she was stopped at the stop sign, and when she started to proceed through the intersection she struck vehicle 1.

 

Investigation at the scene revealed a point of impact (POI) to be within the intersection of N. Brewster Road and E. Oak Road. POI was determined by driver statements and debris in the roadway.

 

Vehicle 1 sustained moderate damage to passenger side of the vehicle. Vehicle was driven away from the scene by driver 1.

 

Vehicle 2 sustained moderate damage to the front end of the vehicle. Vehicle 2 was driven away from the scene by driver 2.  

 

No injuries were reported during this investigation.

 

No summons were issued at this time.

Driver #1: Zilia Perez 38 of the 600 block of N. Fourth Street

On this date, 27 January 2016, I was dispatched to a 2 vehicle collision involving a parked car.  

 

 

Upon arrival I was met by driver 1 who stated she did not need medical attention. Driver 1 stated she was heading East on Oxford Street when she did not notice a vehicle parked to the side of the road and she collided with it. Driver 1 stated the vehicle was sticking into the roadway which caused her to collide with it. Vehicle 1 sustained heavy damage to the front passenger side and was towed by Rock Towing (contacted by operator).  

 

 

Vehicle 2 (parked) owner arrived on scene. She was advised of the incident and stated her son parked the car on the side of the road. Vehicle 2 sustained heavy damage to the front passenger side and was pulled into an adjacent driveway.  

 

 

The scene provided no recent tire marks indicating breaking of vehicle 1. I also observed no tire marks on the snow where vehicle 2 was parked, indicating it was approximately 2 feet into the roadway prior to impact.   

 

 

Summonses Issued: 

 

(1)   Driver 1 was issued a summons for Careless Driving (39:4-97, 0614-VC-008982) 

(2)   The owner of Vehicle 2 was issued a summons for improper parking (39:4-135 0614-VC-008981)

Driver #2: Donald Jones 48 of the 200 block of Summit Street

On this date, January 28, 2016, I was dispatched to the area of Philip Street and North Main Road, in regards to a motor vehicle crash.  I arrived on scene and observed both vehicles pulled to the west side of Main Road, south of Philip Street facing south bound.

As I parked my Patrol vehicle behind the two vehicles, with my emergency lights activated, I was approached by driver #2 and he related the following information.  Driver #2 indicated he was traveling north on Main Road and has he passed Philip Street, he observed vehicle #1 coming off of Philip Street. Driver #2 indicated he initially he thought vehicle #1 did a "rolling stop" and struck his vehicle.  Driver #2 indicated when he observed vehicle #1 coming towards his vehicle, he swerved to the left to avoid the crash, but it wasn't avoidable.

Driver #2 stated after the crash, he turned around to return to the scene, and thought that vehicle #1 was possibly leaving the area, since she pulled out onto Main Road facing south.  Driver #2 indicated soon after the crash occurred, he took photographs of vehicle #1's ice (frost) covered windows. When I first observed vehicle #1, the rear windows were frosted, but the windshield was clear and the side windows were defrosting.  It is unknown if driver #1's vision was obstructed by the partially frosted windows.

I then spoke with driver #1 who indicated she was stopped on Philip Street waiting to enter Main Road, when vehicle #2 may have slid on the ice towards her vehicle and struck her car in the front.
 
It should be noted:  Where Philip Street meets Main Road, there was ice from when the snow in the area melted and then refroze overnight. The lane of travel on Main Road was complete dry, and the ice on Philip Street extended from the edge of Main Road to approximately 8-10 feet east down Philip Street, which would make an approaching vehicle on Philip Street have difficulty stopping.  

Driver #1 was adamant that she was stopped on Philip Street and vehicle #2 slid into her vehicle.  Driver #2 was adamant that he was traveling on Main Road, in his lane of travel when vehicle #1 struck his vehicle.  

No injuries were reported and Vineland EMS was not needed on scene. 

Vehicle #1 sustained minor damage to the front bumper on the left (passenger) side of the front license plate.  Vehicle #2 sustained minor damage to the passenger rear door in the area of the back wheel.  Both vehicles were able to be driven from the scene, even though they both sustained minor damage.  Based on the location of the damage on both vehicles, it is this Officer's belief, that vehicle #1 entered the roadway of Main Road and came into contact with vehicle #2. 

Driver #1 was issued summons # 0614-VC-8860 for careless driving and was given a Court date set for February 16, 2016 at the Vineland Municipal Court.  A subpoena was generated for driver #2, to appear in Court to testify on behalf of the State, in the event driver #1 contests the summons issued against her.

Vehicle #2 is registered to the State of New Jersey, under the Department of Treasury. Vehicle #2 is self-insured through the State of New Jersey through the Division of Risk Management, PO Box 620, Trenton, N.J. 08625

Driver #1: Ian Jackson 18 of the 2000 block of Mayslanding Road

Driver # 1 advised he was east on a roadway located on private property, specifically 2110 Mayslanding Road, when he collided into a parked vehicle located on the south side of the roadway.  Driver # 1 advised he didn't see vehicle # 2 parked because of sun glare.

I then spoke with the driver of Kelly A. Novick, the driver of vehicle # 2.  Ms. Novick advised she was not inside her vehicle at the time of the collision but was advised of the incident prior to my arrival.

Due to the collision occurring on private property and no reported injury, Driver # 1 was not issued any summons.

Driver #1: Terry Coker 56 of the 700 block of Harding Highway

Driver #2: Angel Toledo 34 of the 600 block of E. Walnut Road

Driver#1 stated that he was stopped at the red light west bound at Magnolia Rd and S. Main Rd. He said while sitting there, vehicle#2 attempted to make a left turn on to Magnolia Rd. Driver#1 said while turning, the back tire of vehicle#2 struck the left front of his car. 

Driver#2 stated that he was traveling south on S. Main Rd. and started to make a left turn on to Magnolia Rd. He said while turning, vehicle#1 started creeping forward and struck the left rear of his bus while turning. 

Investigation at the revealed that both vehicles had moved from point of impact prior to my arrival. Vehicle#1 sustained damage to the left front. Vehicle#2 sustained only a scuff mark to the left rear tire. Both drivers stated they were not injured. Driver#2 said his passengers were not injured. 

I was unable to determine who was at fault due to the statement of both drivers and there were no independent witness. I advised driver#1 that based on his statement he could sign a complaint against driver#2 at the Vineland Municipal Court for Improper turn at a Traffic Signal. I also informed driver#2 that based on his statement he could sign a motor vehicle complaint against driver#1 for Careless Driving.

Driver #1: Malcom Rease 21 of Millville NJ

Driver #1 stated he was traveling south bound on South Spring Road when he struck a snow pile that was partially in the roadway south of East Chestnut Avenue. Driver #1 stated due to him striking the snow pile it caused him to lose control of his vehicle (#1) and collide with the mailbox and fence belonging to the property of 540 South Spring Road.

 

On scene, I observed the said snow pile with was partially in the roadway (on other side of fog line) which reportedly caused the collision. The snow pile was in front of the residence of 540 South Spring Road. The owner, Ronald C. Walker, was made aware of the incident and the damaged property. The collision caused damage to the asphalt around the mailbox, mailbox and fence on the property of 540 South Spring Road. Walker was provided with the case number of the report.

 

Driver #1 did not have a complaint of pain.

 

Vehicle #1 sustained moderate damage and was towed by Johnson’s.

 

Driver #1 was issued the following summons:

#0614-VC-009692 (39:3-40; D.L. Suspended);

#0614-VC-009692 (39:3-4; Unregistered Vehicle); and

#0614-VC-009693 (39:6B-2); Uninsured Vehicle).

Driver #1 stated at the scene she was exiting the Shop Rite parking lot (3600 E Landis Ave) and thought vehicle #2 had it's right turn signal on to turn into the parking lot, so she pulled into the roadway and crashed into vehicle #2. Driver #1 said she was attempting to make a left turn on to Landis Ave. Driver #1 was not injured.

Driver #2 stated at the scene he was traveling West on Landis Ave in the inside lane when vehicle #1 pulled out in front of him and caused a crash. Driver #2 said he attempted to avoid the crash but was unable to. Initially, driver #1 said he was not injured and then said he had back pain but he did not want EMS to respond.

No summonses issued at this time.

Driver #1: Mary Lou Roix 59 of Troveland MA

Driver # 1 advised me she was operating her vehicle on 3401 South East Boulevard, AJM Packing Corp., when the trailer she was hauling collided into a parked vehicle.  Driver # 1 said she was making a left turn on the above mentioned property when she failed to judge the distance between her vehicle and vehicle # 2 properly.  While turning, Driver # 1 said the trailer of her vehicle struck the driver's side cab of vehicle # 2.

Driver # 2 stated that he was parked on the above mentioned property when his vehicle was struck by vehicle # 1 trailer. 

Investigation revealed that Driver # 1 actions was the cause of this crash.  Due to no injuries being reported from this incident, and the fact this collision occurred on private property, no summonses were issued.  Vehicle # 2 was disabled at the scene and needed to be removed by tow service.  Driver # 2 arranged for his own tow.

Driver #1, said she was traveling eastbound on Chestnut Avenue, when she was hit on the driver side middle portion of her vehicle. Driver #1, advised once her vehicle was hit it caused her vehicle to flip over on its roof and rest on the driver side portion of the vehicle.   

 

Driver #2, said while she was traveling southbound on South Third Street approaching Chestnut Avenue, she did not see vehicle #1 and pulled out hitting the drive side portion of vehicle #1, causing it to flip over.     

 

Damage report;

Vehicle #1 sustained damage to the driver side middle portion of her vehicle, as well roof damage. Driver #1's vehicle was towed by M&R towing.

 

Vehicle #2 sustained damage to the front portion of her vehicle. Driver #2's vehicle was towed by A1 towing.

 

Injure report;

Driver #1 had a complaint of pain to her neck and was transported to Inspira Health Network (IHN) for further medical treatment.

 Driver #2 refused any and all medical treatment at the scene.


Summons issued;

Driver #2 was issued E-Ticket number 0614E16000251, for improper emerging.

Driver #1 was issued a Subpoena to testify.

Driver #1 stated he was backing out of the driveway of 748 South East Avenue (west side) when Vehicle #2 backed into his vehicle (#1). Driver #1 indicated Driver #2 was backing out from the driveway of 747 South East Avenue (east side). Driver #1 stated, “If he saw me than why didn’t he honk the horn.”

 

Driver #2 stated he was backing out of the driveway of 747 South East Avenue (east side) when Vehicle #1 backed into his vehicle (#2). Driver #2 indicated Driver #1 was backing out from the driveway of 748 South East Avenue (west side). Driver #2 advised Vehicle #1’s rear passenger side bumper struck the rear driver side corner of Vehicle #2. Driver #2 claims Driver #1 is at fault due to the positioning of vehicles as Vehicle #1 backed up farther into the roadway.

 

Both drivers did not have a complaint of pain.

 

Vehicle #1 sustained minor damage to the rear passenger side bumper.

 

Vehicle #2 sustained moderate damage to the rear driver side corner.

 

No summons issued due to conflicting statements as both drivers were backing out into the roadway. No witnesses were located.

Driver #2: Felix Ramirez 70 of Hammonton NJ

Driver #1 stated he was traveling west on West Park Avenue, approaching North Delsea Drive when he entered the left turn lane to make a left turn (south) onto North Delsea Drive.  Driver #1 said when the traffic light turned green he proceeded forward but then stopped for oncoming traffic to clear.  Driver #1 said while stopped his vehicle was struck from behind by vehicle #2.

 

Driver #2 stated he was stopped behind vehicle #1 attempting to make a left turn (south) onto North Delsea Drive.  Driver #2 said vehicle #1 proceeded forward but then made a sudden stop when his vehicle struck the rear of vehicle #1. 

 

No injuries were reported at the scene and damage sustained to both vehicles were minor. 

 

No witnesses to the collision were located at the scene and driver #2 was issued a summons (#VC-008407) for operating an unregistered vehicle.

Driver 1 stated he was traveling West on Mays Landing Road when a deer came out of the woods from the area of Panther Road. Driver 1 attempted to not hit the deer but the deer struck his vehicle on the passenger side rear and front door. Damage to vehicle 1 passenger side door was minimal.

The deer was not injured and fled into the woods prior to my arrival.

Monday, February 01, 2016 I responded to South Seventh Street and East Landis Avenue for a two vehicle crash. Upon arriving, I first spoke with driver one who explained that she was stopped in traffic at a red traffic light on South Seventh Street, pointed north. Driver one explained that while she was stopped, she was suddenly struck from behind by driver two. It should be noted that at the scene, driver one wasn’t claiming any injuries and refused any medical services.

 

After speaking with driver one, I then spoke with driver two who explained that he was traveling north on South Seventh Street, approaching vehicle one from behind. Driver two explained that he noticed vehicle on stopped in traffic and began braking to stop his vehicle. Driver two explained that his foot slipped off of the brake pedal because the bottom of his boot was wet; thus causing him to collide with vehicle one. Driver two was also claiming no injuries at the scene and refused medical services at the time.

 

Vehicle one sustained damage to the rear bumper cover and the trunk lid. Vehicle one was driven from the scene by the driver. Vehicle two sustained minor damage to the front bumper and was also driven from the scene.

 

No summonses were issued in this case.

Driver #1: Charles Neely 74 of Warrington NJ

Driver #2: Hector Rivera 20 of the 700 block of S. State Street

Driver # 1 stated at the scene he was exiting the driveway of the Exxon Gas station (located at 345 N. Delsea Drive) and made a left hand turn onto W. Park Avenue.  He stated he believed it was clear to exit the driveway, onto the roadway and proceeded.  At that time he struck the left front portion of vehicle # 2.  I observed minor damage to the front right portion of vehicle # 1.

Driver # 2 stated at the scene he was traveling west on W. Park Avenue.  He stated Driver # 1 exited the driveway of 345 N. Delsea Drive in front of him.  Driver # 2 attempted to avoid the collision and applied his brakes, but was unable too.  At that time he was struck by vehicle #1.  I observed minor damage to the front left portion of vehicle # 2. 

Both drivers did not complain of pain and refused medical treatment.

No summonses were issued.

Driver # 1 stated as she was parking she struck vehicle # 2 accidentally.  She stated she was the driver of the vehicle at the time of the crash and had driven the vehicle from her residence to pick up her children from school. 

Driver # 1 presented an insurance card through Bankers Independent Insurance Company, NAIC 13455 under Policy # 4420766.  She was issued citation # 0614-VC-9015 for Unlicensed Driver, 39:3-10, and 0614-VC-9016 for No Vehicle Insurance, card expired 10/19/2015, 39:6B-2 with an appearance before the Vineland Municipal Court. 

Driver # 2 was not present at the time of the crash.  Occupant # 3 stated he was seated in the vehicle when Vehicle # 1 struck his vehicle in the parking lot of 411 S. 6th Street.  He stated he saw Driver # 1 as the operator of the vehicle.

Witness # 1, Linda Ruiz, 03/12/76, stated she witnessed the crash and saw Driver # 1 as the operator of the vehicle.

Driver #2: Francine Alfano 46 of the 1700 block of Tomahawk Court

On February 1, 2016 I responded to 2802 South Delsea Drive, Vineland (Wawa) for a two vehicle collision (Reportable/No Injuries) in the parking lot.  Upon arrival, I located both vehicles in the parking lot on the north side just south of Sherman Avenue. 

I spoke with both drivers who stated they were fine and not injured from the collision.  

Driver#2 then stated she was exiting Wawa attempting to enter onto Sherman Avenue but was slowing due to traffic.  Driver#2 stated she was then suddenly rear ended by Driver#1 prior to entering the roadway.  

I then spoke with Driver#1 who stated she was following behind driver#2 and thought that she was exiting the parking lot but then realized she was stopping due to traffic.  Driver#1 stated she was not able to stop and rear ended driver#2.

On February 1, 2016 I responded to 3849 South Delsea Drive, Vineland (Chick-Fil-A) for a two vehicle collision (Reportable/No Injuries) on the parking lot. 

Upon my arrival, I spoke with driver#1 who stated that she was backing out of her stall when she suddenly backed into driver#2. Driver#1 stated she thought it was clear to back up but realized it wasn't when she struck driver#2's vehicle.  

Driver#2 stated that he was traveling west in the parking lot looking for a parking stall when driver#1 suddenly backed into his vehicle on the rear passenger side. 

Both drivers and all occupants stated they were fine and declined medical attention.
