Driver 1 stated she was traveling in the south bound lane of N Delsea Drive and entered the left turn lane at the intersection of N Delsea Drive and W Almond Road. Driver 1 stated there was a vehicle in front of the North bound left turn lane to W Almond Road and could not see the north bound traffic. Driver 1 stated she then began to make a left turn when vehicle 2 came unexpectedly and struck her vehicle on the front bumper and spun her vehicle. The vehicle spun and came at a stop in the north bound lane of N Delsea Drive. 3 of the occupants including the driver complained of injury and were transported to Inspira for evaluation. Vehicle 1 had significant damage to the front bumper. Driver 2 stated she was traveling in the north bound lane of N Delsea drive when driver 1 pulled in front of her attempting to make a left turn. Vehicle 2 initial impact was the front driver side. Due to the impact of the collision, vehicle 2 was pushed into the Delsea Drive/ Almond Road sign which was knocked down as a result of this accident. Vehicle 2 sustained heavy damage to the front end. Driver 2 complained of Chest pain after EMS already cleared but she stated she would be going to the hospital for evaluation. Witness Domenique Diaz stated she was in the left turn lane of the north bound traffic of N Delsea Drive to go onto Almond Road. Diaz stated driver 1 was traveling south bound on N Delsea drive and entered the left turn lane without putting on her signal light. Diaz stated driver 1 then began to make a left turn and never stopped before turning at which time vehicle 1 struck vehicle 2. As a result of this accident the following summonses were issued to driver 1: - NJSA 39:4-90 (failure to yield at intersection) VC-008394 - NJSA 39:3-10 (Unlicensed Driver) VC-008395

Occupant of vehicle #2: Iran E. Lapaz-Dealvarez stated she was parked in front of 307 South 2nd Street, facing north bound. She stated vehicle #1 side swiped the vehicle #2 which she occupied sitting in the drivers’ seat. She claimed to have no injuries. She stated the school bus did not stop or slow down. She followed the school bus and obtained the New Jersey registration S1P283. I observed vehicle #2’s drivers’ side rearview mirror broken with yellow paint transfer, along with a small dent to the drivers’ side door.
The Vineland Board of Education was contacted and the Director, John Morris, along with the school bus (vehicle #1) and driver responded to the scene. I met with Driver #1 who stated she did not feel or see the collision and did not know she was involved. I also advised Mr. Morris of the collision. Driver #1 advised no one else was on the school bus and she was the only occupant.
I observed damage to vehicle #1’s passenger side revealing the sideswipe to vehicle #2’s drivers’ side rearview mirror, along with blue paint transfer. I also observed a scuff mark to the passenger side rear tire.

Driver 1 stated she was traveling south on S Delsea Dr and struck the rear of vehicle 2. Driver 1 stated she was not using her cellphone during the time of her collision. Damage to vehicle 1 was on the front bumper area. Driver 2 stated he was stopped in traffic traveling south on S Delsea Dr when vehicle 1 came at a high rate of speed and struck the rear of his vehicle. He believes driver 1 might have been on her cellphone because he does not understand how driver 1 did not see his vehicle stop. Damage to vehicle 2 was on the rear bumper area. Driver 2 was transported to Inspira Health Network by Vineland EMS for complaint of pain to the head.
Driver #1 was cited for following too closely.

While speaking with driver 1 he advised he was at the intersection of West Forest Grove Road and N Delsea Dr, facing west, waiting to turn south on N Delsea Dr. Driver 1 advised he was not sure what vehicle 1 was doing, he thought they were going to let him turn on the roadway. Driver 1 stated as he started making the left turn on N Delsea Dr he struck vehicle 2, who was going across the roadway. Driver 1 did not report any injuries at this time as a result of the collision. Vehicle 1 was unable to be driven due to front end damage sustained from the collision. Vehicle 1 was towed by JC towing. Upon my arrival I spoke to Driver 2 who stated he was driving east on West Forest Grove Road when he approached the intersection at N Delsea Dr and West Forest Grove Road. Driver 2 advised when he started to go across N Delsea Dr, vehicle 2 was turning south on N Delsea Dr when he struck vehicle 1. Driver 2 advised of having pain to his head from hitting it against the window. Driver 2 also advised his passenger was having pain from her left side and right leg as a result from the collision. I advised dispatch to contact Vineland EMS and have them respond to the scene to treat both occupants in the vehicle. Both occupants were transported to Inspira Health Network for further evaluation. Vehicle 2 sustained front end damage and was unable to be driven. Vehicle 2 was towed by JC towing. Driver 1 was issued a summons for failure to stop or yield 39:4-144 (0614VC007910)

Friday, December 18, 2015 I responded to the intersection of South Delsea Drive and West Walnut Road for a two vehicle crash. Upon arriving I was first determined that both drivers were complaining of pain. EMS was called to respond and treat each driver.
I first spoke with driver 2 who explained that she was traveling north on Delsea Drive. She stated she did not realize that driver 1 was stopped in the roadway waiting to make a left turn onto West Walnut Road. Driver 2 explained that once she realized that driver 1 was stopped she attempted to stop her vehicle however she was unable to avoid the collision with vehicle 1. After speaking with Driver 2, she was treated by EMTs and later taken to the hospital for further treatment.
I then spoke with driver 1 who stated he was stopped in traffic on Delsea Drive waiting to make a left turn onto West Walnut Road. Driver 1 said that suddenly he was hit from behind by driver 2. Driver 1 was checked by EMTs at the scene and refused any further medical treatment at the time.
Vehicle 1 sustained damage to the rear end and was driven from the scene by the owner. Vehicle 2 sustained damage to the front end and had front airbag deployment. Vehicle 2 was towed from the scene by Ayala Towing.
There was a witness to this crash who stated he was traveling north on Delsea Drive behind driver 2. The witness stated he observed driver 1 stopped in traffic waiting to turn onto Walnut Road with a turn signal on. The witness stated he watched as Vehicle 2 collided with vehicle 1, striking it from behind.
Due to injuries sustained and the amount of damage done, driver 2 was issued a summons for careless driving. 0614-VC-007302

On this date, 18 December 2015, I was dispatched to Fourth Street and Park Avenue for a two car collision. Upon arrival Bellmawr EMS, who was coincidentally passing by, was on scene and speaking with Driver 2 who had a complaint of chest pains. Driver 2 was transported to Vineland Inspira for medical screening. Driver 1 denied medical attention.
Vehicle 2 sustained heavy damage to the front end and driver side front causing it to be inoperable. Rock Towing responded to the scene to tow vehicle 2.
Driver 1 informed me he was driving South on N Fourth Street when he was driving through the intersection at Park Avenue. Driver 1 stated he looked both ways and did not see a vehicle but when he entered the intersection he was struck by Vehicle 2. Driver 1 stated Vehicle 2 did not have its headlights on causing him to not see the vehicle. Vehicle 1 sustained heavy damage to the Passenger side front end and door. Driver 1 was able to operate the vehicle and it was driven off scene by the owner.
I then responded to Inspira to speak with Driver 2. Driver 2 was being treated for chest pains. Driver 2 stated he was driving East on Park Avenue when Vehicle 1 came out of nowhere and struck his vehicle. Driver 2 stated his vehicles lights automatically turn on when the vehicle is started and he was sure they were on during the collision. Note: Upon my arrival the vehicle was off but when the tow truck operator turned Vehicle 2 on the lights did turn on.
A Subpoena to Testify form was completed for driver 2 and submitted with this report.
Driver 1 was later summoned for Failure to Stop/Yield 39:4-144 (0614-VC-008966)

When I arrived on scene, I immediately checked for injuries. Both driver’s reported no injuries at this time.
I spoke with driver 1 who stated she was traveling southbound on Lincoln Avenue, and was stopped at the red light. She reported when the light turned green, she proceeded to go, but advised that there was one vehicle in front of her. She further added the vehicle in front of her slammed on their brakes, and she in turn did the same. She told me vehicle 2 then rear ended her vehicle causing minor damage to the rear bumper of her vehicle.
I turned to driver 2 who reported as he was traveling southbound on Lincoln Avenue, he was stopped at the red light behind vehicle 1. He explained when the light turned green, he and vehicle 1 proceeded to go, then vehicle 1 slammed on the brakes, and he collided with her vehicle causing minor damage to the front license plate and bumper of his vehicle.
Both vehicles had minor damage and were driven from the scene by the drivers.

Dispatched to area of N. Delsea Drive and Oak Road in regards to a one vehicle collision. Upon arrival a black Ford F150 step side pickup was found just off the Western side of Delsea Drive unoccupied, with the keys in the ignition. Checking the area did not reveal the driver, the anonymous caller had reported the driver was a Mexican male who was last seen traveling North on Delsea Drive. South of the Vehicle on the East side of Delsea Drive a Verizon pole BT 604 was found hanging from the phone wires where it was sheared off at ground level. Electric Utility supervisor Chris Brodzik responded to the scene and advised he would contact Verizon. It appears vehicle #1 was traveling North on Delsea Drive and drifted off the Roadway to the East striking the Utility pole before overcorrecting to the West and striking the for sale signs and shrubbery of the old 7-11 lot on North Delsea Drive. The plate on the Ford truck PA Registration came back to a station wagon. Running the VIN 2FTDX0864VCA30964 came back as not on file. Further research thru inspection records revealed an owner of Michael St. John of Mullica Hill from 3 years prior. Contacting St John revealed he had sold the pickup approximately 2 years ago to an unknown Mexican Male. St. John had no further information on the subject. Sitting in the vehicle was a Corona 24 pack of 7oz bottles with 4 full bottles remaining. Vehicle was towed by Bee Brothers at this time. Thru VIN assist the vehicle was identified as 1997 Ford F150 Pickup truck. A white Samsung Galaxy Cell phone was tagged and placed in ID locker 79 with the registration which was from a 1997 Ford Station Wagon with VIN 1FMCU24X8VUB56819.

Marissa J. Mason, owner of vehicle 2, initially reported that someone caused damage to the driver's side mirror to her 2007 Nissan Altima. Upon further investigation, it was determined that the vehicle was actually struck by another vehicle, as her vehicle was parked on the street in front of her residence. Mason related that it occurred sometime between last evening and this morning. Upon looking at the mirror and the debris in the roadway, I found pieces to a mirror that did not match Mason's vehicle. I collected the pieces and placed them in an ID locker as evidence. There were no other indicators found that would determine what kind of vehicle struck Mason's.

Driver #1 stated at the scene he was traveling South on Delsea Drive and was making a left turn onto Landis Ave. Driver #1 states he did not see the subject walking a bicycle in the crosswalk crossing South across Landis Ave until he struck the bicycle the subject was walking. William Reed III stated at the scene he was walking his bicycle across Landis Ave when a Southbound vehicle made a left turn striking his bicycle that he was walking bending the back tire. Reed III was dressed all in dark clothing and was pushing a bicycle that did not have any lights at this time. Reed III signed a refusal for Vineland EMS at this time. Due to minor damage and no injuries no summons was issued at this time. Parsons loaded the damaged bicycle into his pickup truck and gave Reed a ride home from the collision at this time.

Driver #2 stated he was traveling northbound on North Delsea Drive when vehicle #3 struck his vehicle from behind at the same time he struck vehicle #1 in the rear. Driver #2 stated traffic was slowed down all of a sudden. Driver #2 claimed to have no injuries.
Driver #3 stated he was traveling northbound on North Delsea Drive when traffic all of a sudden slowed to a stop and he had no time to stop and struck vehicle #2 in the rear. Occupants in vehicle #3 claimed to have no injuries.
No summons were issued at this time.

Driver #1 stated he was traveling south bound on South Fourth Street approaching West Elmer Street when he failed to stop at the stop sign and proceeded through. Driver #1 stated he “ran the stop sign” and struck Vehicle #2 at a right angle. Driver #1 was apologetic and forgot there was a stop sign at that intersection and he did not have the right of way.
Driver #2 stated she was traveling east bound on West Elmer Street approaching South Fourth Street when Vehicle #1 ran the stop sign causing her vehicle to strike it at a right angle. Vehicle #1’s front end struck Vehicle #2’s front passenger side. Driver #2 stated, “He (Driver #1) did not stop. I couldn’t do anything, it was too late.”
Both drivers had no complaint of pain. The front seat passenger of Vehicle #2 had a complaint of pain to her head and chest. Emergency Medical Service (EMS) responded to the scene but, medical assistance was declined.
Vehicle #1 sustained major damage to the front passenger side area and was towed by Rock Towing.
Vehicle #2 sustained major damage to the front end and was towed by Ayala.
Driver #1 was issued summons # 0614-VC-007006 (Failure to stop; 39:4-144). Driver #2 was issued a subpoena with this report.

On this date, 20 December 2015, I was dispatched to the intersection of Wood Street and Fourth Street for a 2 car collision with no reported injuries.
Upon arrival I asked all parties involved if medical attention was needed and they all replied that it was not.
Driver 1 said he was starting from the stop signs at N Fourth Street, heading north, when he did not see Vehicle 2 coming and collided with it. Vehicle 2 sustained minor damage to the front passenger side fender and was driven off seen by the owner.
Driver 2 stated he was driving West on Wood Street when Vehicle 1 came through the stop sign and struck his vehicle. Vehicle 2 sustained minor damage to the front driver side fender and door. Vehicle 2 was driven off scene by the owner. Note: Vehicle 2 had the right of way.
No Summonsed issued.

Driver #1 stated she was traveling west bound on East Oak Road approaching Chapel Drive when a deer came from the north side of the road heading in a southern direction at which time it struck Vehicle #1 at a right angle. Driver #1 attempted to apply her brakes to avoid the deer but, it was not possible.
On scene, I checked the area for the struck deer and it was on the south side of the road (on the shoulder) deceased. I notified Police Dispatch of the deceased deer.
Driver #1 did not have any complaint of pain.
Vehicle #1 sustained major damage to the front bumper area (grill area) and was towed by Bee Bros. There was what appeared to be deer hair stuck in the front grill of the vehicle.
No summons issued.

On this date, 20 December 2015, I was dispatched to South Delsea Drive in the area of the Mall entrance for a collision with unknown injuries.
Upon arrival I observed 2 heavily damaged vehicles blocking the roadway. Officers on scene already checked the medical status of both occupants and position their vehicle so that oncoming traffic was no longer a danger. Stacy King the operator of Vehicle 2 had a complaint of pain to her neck so EMS was contacted and responded (she later signed off and did not want to be transported to the hospital). Stephanie Mcveigh was sitting a short distance away from the vehicles and when asked if she needed medical attention she declined.
The description of both vehicles were relayed to Vineland Police Dispatch. Vehicle 1 (Mcveigh owner) 2011, Mercedes-Benz, 4 Door, Black, VIN# WDCGG8HB0BF529599, NJ Reg YUS83F. Vehicle 1 was heavily damaged to the front end of the vehicle was inoperable and later towed by Rock Towing (12 Hour). Vehicle 2 (King owner) 2010, Nissan, 4 Door, Red, VIN# 1N4AL2AP6AN552101, NJ Reg Z18DYL. Vehicle 2 was heavenly damage to the front driver side and front end and also was inoperable it was later towed by Ayala Towing.
King advised me she was driving North on Delsea Drive when Mcveigh turned in front of her vehicle at the mall entrance. King said she was unable to avoid the collision and was struck. King stated her light was green.
On scene were two women who stated they were driving by when the collision occurred. Both Amy Krise and Tammi Garrison said they observed Mcveigh disobey the red turn signal and caused the accident with King.
I asked Mcveigh how the accident happened and she said she turned left at the light trying to enter the Mall entrance when King and her collided. Mcveigh said she believed the arrow was green when she turned. After subsequent investigation, Mcveigh was taken into custody and charged with DWI. (See DWI report for further)
Mcveigh was processed and issued the traffic summonses listed below.
