Driver #1: Christian Perez 36 of the 1000 block of E. Landis Ave.

 

Driver #2: Joyce Harper 49 of Millville NJ

 

Driver #1 stated he was making a left turn from East Elmer Street onto South East Avenue.  Driver #1 stated he started his turn there was a vehicle (driver #2) stopped at the stop sign facing north on South East Avenue.  Driver #1 continued his turn as driver #2 was backing up to allow him to make the turn.  Driver #1 stated as he completed the turn the trailer wheels on the driver side collided with driver #2's vehicle.

Driver #1 stated she was stopped at the stop sign facing north on South East Avenue.  Driver #2 said as the truck was turning she started to blow her horn to let the driver know he was about to hit her vehicle.  Driver #2 stated she started to back up her vehicle but it was too late the driver #1 collided with her vehicle.
 
There were no injuries reported at the scene.

Driver #1's vehicle suffered no damage.  Driver #2's vehicle suffered minor damage to the lower left front of her vehicle.  Both vehicle were driven from the scene.

There were no summonses issued at this time.

Driver #2 was insured by IDS Property Casualty Insurance Company located at 3500 Packerland Drive in De Pere WI 54115.  NAIC number 29068 was located on the insurance card.

The trailer registration was 2089892 from Maine and it expires on 02/29/2020.

Driver #1 stated she was traveling north on North Delsea Drive behind Vehicle #2.  Driver #1 explained that Vehicle #2 suddenly came to a stop and she was unable to stop her vehicle in enough time to avoid a collision.  

Driver #1 and occupant did not report any injuries.
 

Vehicle #1 sustained front end damage. 
Vehicle #1 was pulled into a private parking lot to wait for a tow. 

 

Driver #2 stated he was in the north bound lane on North Delsea Drive stopped in traffic when Vehicle #1 rear ended him. 

Driver #2 did not report any injuries.
Vehicle #2 sustained rear end damage.
Vehicle #2 was driven from the scene. 

No summons was issued.

Driver# 1 stated at the scene, he was traveling south on Delsea Drive as he passed a vehicle that appeared to be stopped in traffic awaiting to turn into a driveway.  Driver# 1 said as he passed the vehicle, his vehicle driver side view mirror struck vehicle# 2's driver side view mirror.  Driver# 1 said he stopped his vehicle and backed up to speak with driver# 2 who was now pulled into her driveway at 3872 North Delsea Drive.  


Driver# 2 stated at the scene she was stopped in traffic facing north on Delsea Drive awaiting to turn into her driveway located at 3872 North Delsea Drive.  Driver# 2 said as vehicle# 1 passed her, going south bound, vehicle# 1's driver side view mirror struck her driver side view mirror.  Driver# 2 advised she pulled into her driveway and contacted this department.  Driver# 2 added there was a male subject walking south on the west side of Delsea Drive as Vehicle# 1 passed her.  Driver# 2 said she believed driver# 1 slightly swerved into her lane to avoid hitting the pedestrian, which is what driver# 2 said caused the crash.  


Vehicle# 1 did not sustain any damage and vehicle# 2 sustained apparent minor damage to the driver side view mirror.  No injuries were reported at the scene and both vehicles were able to be driven from the scene.  No summonses were issued as a result of the crash.

Driver #2: Dale Lagerholm 44 of the 2000 block of S. Lincoln Ave.

Driver #1 stated she was traveling in the north bound lane on South Delsea Drive behind Vehicle #2.  Driver #1 explained that as she was driving, she felt something brush against her leg so she quickly glanced down to see what it was and when she looked back up Vehicle #2 slammed the breaks.  Driver #1 indicated that she did not have enough time or space to avoid colliding with Vehicle #2. 

Driver #1 had an abrasion to the right hand with pain, Vineland EMS was requested and they responded.  Driver #1 refused treatment and transport from Vineland EMS. 

Vehicle #1 sustained heavy front end damage with multiple airbag deployments. 

 

Driver #2 stated he was traveling in the north bound lane on South Delsea Drive behind a vehicle that was preparing to make a left turn.  Driver #2 indicated that he slowed down as the vehicle in front of him made a left turn.  Driver #2 explained that as he was slowing down Vehicle #1, slammed into the rear of his vehicle. 

Driver #2 and occupant claimed no injury. 

Vehicle #2 sustained heavy damage to the rear bumper and rear doors. 

 

Vehicle #1 was towed by M & R Auto Body, Vehicle #2 was driven from the scene.

Driver #1 was issued Summons #0614-VC-008826 for 39:4-97 (Careless Driving) and 0614-VC-008827 for 39:3-4 (Unregistered Vehicle)

Driver #2: Breeana Webb 24 of Pennsauken NJ

Driver #1 stated she was parked on the east side of the roadway on South Seventh Street.  Driver #1 explained that when she attempted to merge with traffic she did not see Vehicle #2 and struck the passenger side of that vehicle. 

Driver #1 and occupants did not report any injuries.  

Driver #2 stated she was on South Seventh Street stopped behind a vehicle that was turning into a driveway.  Driver #2 explained that Vehicle #1 drove into her vehicle while as it attempted to pass her on the right from where it was parked on along the curb.  

Driver #2 did not report any injuries. 

 

A witness on location indicated that he was stopped at the intersection of South Seventh Street and East Elmer Street at the red light.  The witness then explained that he observed Vehicle #1 drive into vehicle #2 from where it was parked. 

 

Driver #1 was issued summons #0614-VC-00828 for 39:3-10 (unlicensed driver) and summons #0614-VC-00829 for 39:4-97 (careless driving) 

 

Vehicle #1 was pulled into a parking lot to be picked up later by a licensed driver. 

Vehicle #2 was driven from the scene.

Driver: Cheryll Liberto 58 of the 800 block of N. Main Road

 

On 12/11/2015 I was dispatched to Rainbow Lane and North East Avenue in regards to a two vehicle collision with no reported injuries.   

 

Upon arrival I spoke with Driver # 2 who stated she was traveling east on Rainbow Lane approaching North East Avenue.  Driver #2 said there were two parked vehicles parked directly across from each other along the curb line.  Driver #2 advised she thought she had enough room to pass, and attempted to maneuver through the parked vehicles.  Driver #2 explained while passing the parked vehicles, the rear of vehicle #2 struck vehicle #1.   

Next, I spoke with Bob Paul’s (CTX Infrastructure Superintendent) who advised vehicle #1 was parked on Rainbow Lane on the north side curb.  Paul’s explained Vehicle #1 is registered to CTX Infrastructure, and no one was inside vehicle #1 when it was struck by vehicle #2.   

 

While on scene Driver #2 or the occupants in vehicle #2 did not have any complaints of pain, and EMS was not called to the scene.  Vehicle #2, Vineland Board of Education School Bus # 25 had minor scratches on the rear driver side quarter panel.  Vehicle # 1 had no reported damage.
The bus had the following students on the bus at the time of collision.

Driver 1 stated she was traveling south on N Eighth St and was stopped in traffic at the stop sign. Vehicle 2 struck her vehicle from behind. Driver 2 got out of the vehicle and checked his vehicle and vehicle 1. He got back in his car and pulled alongside driver 1 and stated that there was no damage on vehicle 1 and does not need to report the incident. She told him that she is calling the police because she needs the incident documented and driver 2 fled south on N Eighth St. Damage to vehicle 1 was very minimal to the rear bumper area.

A check of the area for vehicle 2 revealed negative results.

Description of vehicle 2 provided by driver 1 is: a 4 door Toyota, maroon in color. Driver 2 was described as a white male possibly 25 years of age.

A partial tag of vehicle 2 was supplied by driver 1 which was New Jersey registration (N15).

A check of the city tag reading cameras was negative for any vehicles that had that partial tag, going by the cameras.

Friday, December 11, 2015 I responded to the area of North Delsea Drive and West Park Avenue for a two vehicle crash. Upon arriving on scene, I first determined that there were no injuries.

 

I first spoke with driver one who explained that she was facing north on Delsea Drive, stopped in traffic, at a traffic light. Driver one explained that while stopped she was suddenly struck from behind by driver two.

 

Upon speaking with driver two, he explained that he had just exited the driveway to Wal-Mart and began traveling North on Delsea Drive. Driver two explained that he did not see that driver one was stopped in traffic until he was too close. Driver two explained that he hit his brakes and swerved to the right in an attempt to avoid the crash, however he was not able to and struck the rear passenger side corner of vehicle one.

 

Vehicle one sustained damage to the rear passenger side corner and was driven from the scene by the driver. Vehicle two sustained damage to the front driver’s side corner and was also driven from the scene. There was no airbag deployment in the crash and no summonses were issued

Driver #1 stated at the scene she was traveling South on Delsea Dr approaching the Chestnut Ave intersection when she was distracted by her children in the rear seat, so she took her attention away from the road and crashed into vehicle #2. Driver #1 and the passengers in vehicle #1 were not injured.

Driver #1 stated at the scene she was traveling South on Delsea Dr and was stopped in traffic at the Chestnut Ave intersection when vehicle #2 crashed into her. Driver #1 said she felt some discomfort in her neck but she did not want any medical attention at this time. She said she would seek medical attention at a later time.

Driver #1 was issued a summons for Careless Driving.

Driver #1: Victoria Togno 69 of the 1600 block of N. East Ave.

Driver #2: Amy Stenberg 37 of Millville NJ

Driver #2 stated that she was stop at the intersection of W Park Ave and Northwest Blvd (Red Light) facing east bound when her vehicle was struck from the rear by Driver #1. Driver #2 claimed minor neck pain at the scene but refused medical treatment. Driver #2's vehicle sustained extensive damage to the rear driver side bumper and was towed from the scene by A-1 Towing.

Driver #1 stated that she was traveling East on W Park Ave towards the intersection of Northwest Blvd when she attempted to change lanes. As she merged into the southern lane of travel she collided with Driver #2 who was stopped in traffic. Driver #1 stated that she was unaware that Driver #2 was stopped in traffic due to a blind spot in her vehicle caused by the front window frame.

Driver #1 sustained a minor laceration to her head and was treated by EMS at the scene. She refused further medical treatment. Driver #1's vehicle sustained extensive damage to the front passenger side bumper and towed from the scene by Johnson's Towing.  No summons was issued at this time.

Location: N.W. Boulevard and Rosewood Ave.

On 12/22/2015 I was dispatched to the area of North West Boulevard and Rosewood Avenue in regards to a vehicle on the railroad tracks.  Prior to my arrival on scene dispatch contacted Conrail regarding the incident.   

 

Upon arrival I spoke to Driver #1 who stated he was traveling south on North West Boulevard approaching Rosewood Avenue.  Driver #1 explained while traveling he changed the vehicle clutch, and the vehicle apparently went in to a stall.  Driver #1 said at this time he lost control of the vehicle, and the vehicle came to a complete stop on the rail road tracks.   

 

Driver #1 stated he has been residing in New Jersey for about five years, and does not have a valid New Jersey driver’s license. Driver #1 advised vehicle #1 is currently registered to his friend who he identified as Roque Garcia. Note: Driver #1 was unable to provide detailed information regarding Roque Garcia, but did provide registration information at the scene which indicated vehicle #1 is currently registered to “Roque Garcia”.   Dispatch was provided the tag for vehicle #1 (Temporary Pennsylvania Registration 8896675), however dispatch was unable to access the registration information for vehicle #1.  Dispatch also attempted to contact Roque Garcia, however the call was negative.   

 

While at the scene Driver #1 did not have a complaint of pain, and EMS was not called to the scene.  Vehicle #1 was towed from the scene by M&R Towing.

Driver #1: Michelle Duven 26 of the 1000 block of Weymouth Road

Driver #2: Hilton Shostak 67 of the 1000 block of Riviera Boulevard

Driver #1 Michelle Duven, made the following statements regarding this incident.  Duven stated she was traveling south bound on S Main Rd, approaching the intersection of E Butler Ave.  Duven indicated the light at the intersection was green, and as she approached closer to the intersection she was unable to decipher if the traffic signal had turned red or not due to the sunglare.  Just prior to entering the intersection she observed driver #2, Hilton Shostak, entering the intersection traveling west bound on E Butler Ave.  At that time, Duven slammed on her breaks and slid forward, striking the passenger rear door of Shostak’s vehicle.   

 

Driver #2, Hilton Shostak, made the following statements regarding this incident.  Shostak stated he was traveling west bound on E Butler Ave and approached the red stop light at S Main Rd, and came to a stop.  After waiting for the light to change, it finally changed to green and he proceeded to cross the intersection.  As he entered the intersection, Shostak stated his vehicle was suddenly struck on the passenger side causing his vehicle to turn north and south.  After his vehicle turned north and south the momentum of the collision propelled his vehicle over the northwest curb, causing him to strike the decorative hedges of 590 E Butler Ave (Marilyn Chappius).  All the air bags on the passenger side of the vehicle were activated during this collision. 

 

Witness #1 Allison Mccray, made the following statements regarding this incident.  Mccray stated she was traveling south bound on S Main Rd behind Duven.  Mccray stated as they approached the light she observed the light turn red.  As Mccray applied the brakes, she observed Duven continue to pull away from her and enter the intersection.  Mccray stated Duven entered the intersection and struck Shostak’s vehicle.  Mccray indicated she never saw the brake lights of Duven’s vehicle. 

 

Duven’s vehicle sustained moderate damage but was still drivable.  Duven had no complaint of pain at this time. 

 

Duven was issued summons 0614-VC-008631, 39:4-81 Failure to observe Signal 

 

Shostak’s vehicle sustained moderate damage and was unable to be driven, therefore it was towed from the scene.  Shostak’s passenger, Susan Shostak, had complaint of pain to her face, but refused medical treatment on scene by EMS.

On Saturday, December 12, 2015, I was dispatched to the intersection of Sherman Avenue and Delsea Drive, for a two vehicle collision in front of 2802 South Delsea Drive (Wawa,) with no reported injuries. 

 

When I arrived on scene, Vineland Fire Personnel were already on scene directing traffic, as vehicle 1 was disabled in the two way left turn lane and disrupting the flow of traffic. I spoke to both drivers, and the passenger of vehicle 2 for injuries. None of the occupants involved reported any injuries at this time. 

 

I spoke to driver 1 who stated as he was travelling southbound on Delsea Drive, he was driving in the inside lane, when driver 2 pulled out of the parking lot of Wawa, when he collided with vehicle 2, causing moderate damage to the front driver’s side corner of his vehicle, disabling him in the two way turn lane facing southbound. 

 

I turned to driver 2 who stated as she was attempting to make a left turn out of Wawa’s parking lot in order to head north on Delsea Drive, she misjudged the speed of driver 1 and she was struck on the driver’s side of her vehicle, causing moderate damage to the front and rear driver’s side doors. Vehicle 2 came to rest on the east side of Delsea Drive, in the grass, and knocking over a white directional sign in the process. 

 

Police Dispatch notified the Department of Transportation and advised them of the damaged sign. 

 

M&R Towing towed vehicle 1 from the roadway, and vehicle 2 was driven from the scene by the driver. 

 

Driver 2 was issued the following summons for failing to yield the right of way when leaving a private driveway: 0614-VC-8769

Location: Chestnut Ave. and S.E. Boulevard

Driver #1: Jose Ramos-Perez 26 of the 200 block of S. Seventh Street

Driver #2: Jacob Negron 27 of Millville NJ

Driver #1 stated as he traveled east  on E. Chestnut Ave. and upon entering the  intersection of S.E Blvd  and E. Chestnut Ave. he was struck by vehicle # 2 on the passenger side.

 

Driver #2 stated as he was traveling north on S.E. Blvd and upon entering the intersection of S.E Blvd  and E. Chestnut Ave., vehicle #1 entered the intersection without stopping at the red light when it was required to do so, causing the collision.

 

A witness to the collision stated she observed vehicle #1 go through the red light without stopping when it was required to do so causing both vehicles to collided.

 

The passenger of vehicle #1 complained of pain to his left side and his back and was transported to Inspira Health Network by Vineland EMS. All others involved claimed no injuries and refused medical attention.

 

Vehicle # 1 sustained major damage to the passenger’s side and was towed always by Johnson Body Shop. Vehicle # 2 sustained moderate damage to the front and was driven away.

 

Driver #1 was issued summons numbers 0614-VC-8527 and 0614-VC-8528 for driving without a license and for failing to stop.

Location: Melrose Street and Almond Street

Driver #1: Jose Perez 39 of the 400 block of S. 2nd Street

Driver #2: Alexis Cardona 45 of the 400 block of Ithaca Street

Driver# 1 stated at the scene, he was traveling east on Almond Street as he neared a new development, he noticed a vehicle began to pull out of this roadway that was under construction.  Driver# 1 said he did not have time to swerve or stop and he struck vehicle# 2 as it pulled out in front of him. 


Driver# 2 stated at the scene, he was pulling out of the construction area and began inching forward as he noticed a vehicle was coming from the east.  Driver# 2 said he waited for the vehicle from the east to pass and inched forward again, now realizing vehicle# 1 was coming from the west.  Driver# 2 said he tried to reverse to avoid being struck by vehicle# 1; however he was struck anyway.  That was when he contacted the police and I arrived a short time later. 

NOTE:  It should be known that the street that vehicle# 2 was pulling out of is Melrose Court; however I was not able to change the crash location in the CAD due to it not being updated. 


No injuries were reported as a result of the crash.  Vehicle# 1 sustained apparent minor damage to the front bumper and vehicle# 2 sustained apparent moderate damage to the driver side front wheel that was canted.  Vehicle# 1 was able to be driven from the scene and driver# 2 contacted his tow for vehicle# 2.  Vehicle# 2 was not a hazard and was off the roadway.  No summonses were issued as a result of this crash.

Driver #1: Rita Rivera-Worley 44 of the 2000 block of Greenwillows Drive

Driver #2: Roman Berzin 26 of the 1000 block of S. Spring Road

Driver # 1 stated she came to a complete stop at the intersection of Greenwillows Drive and South Spring Road, facing west on Greenwillows Drive. Driver # 1 said she looked both ways, traffic was clear, and she pulled into the intersection. Driver # 1 said she was then struck by vehicle # 2 on the driver side of her vehicle. Driver # 1 had a complaint of pain to her left side and was transported top Inspira Medical Center by Vineland EMS for evaluation. Vehicle # 1 was towed by Bee Brother Towing after sustaining extensive damage to the driver side of the vehicle.    

Driver # 2 stated he was traveling north on Spring Road, approaching Greenwillows Drive. Driver # 2 said driver # 1 pulled in front of his vehicle and he was unable to avoid the collision. Driver # 1 was uninjured and his vehicle sustained front end damage.

Driver: Marie Fisher 68 of the 200 block of E. Oak Road

Pedestrian: Juan Aleqeyn 28 of the 400 block of N. East Ave.

 

Driver #1 stated she was traveling east on Fenimore St.  She stated she observed a garbage truck pulled to the side of the road and being loaded with trash.  She stated she slowly passed the truck in the opposite lane of travel.  She stated Pedestrian #2 turned around after loading the trash and walked into the opposite lane as she passed at which time she ran over his foot.

Pedestrian #2 stated he was loading trash into the trash truck and was unaware the vehicle #1 was passing.  He stated he turned around at the exact time the vehicle #1 was passing at which time his left foot was ran over causing him to fall to the ground.

I then spoke with Daniel Serrano who was the operator of the South Jersey Sanitation garbage truck.  Serrano related he observed the incident.  Serrano related Driver #1 passed the garbage truck without him waving her through.  He stated Pedestrian #2 was not looking at which time he turned around and had his foot ran over.

Vehicle #1 had no visible damage to the vehicle.  Pedestrian #2 had complaint of pain to his left foot and elbow at was transported to IHN for treatment.

No summons were issued in regards to this collision.
