Driver #1: Emanuel Rios 21 of the 200 block of S. Fourth Street

Driver #2: Grecia Rodriguez 30 of Bridgeton NJ

 Driver # 1 stated he was north on 7th Street behind Vehicle # 2.  He stated Vehicle # 1 had it's yellow flashing lights on and then the vehicle stopped. He stated he didn't stop in time and crashed into the rear of Vehicle # 2.  Driver # 1 was issued summons 0614-VC- for Following Too Closely, 39:4-89, with an appearance before the Vineland Municipal Court. 

    Driver # 2 stated she was north on 7th Street from Montrose Street and had the vehicle's warning lights flashing in anticipation of picking up children for school.  She stated as she was slowing to a stop, Vehicle # 1 crashed into the rear of her vehicle. Vehicle # 2 had insurance through New Jersey Schools Insurance Group, Company Number SBJIF-003, Policy Number PR383AG-01 valid through 07/01/2016.

Driver #1: Manohar Singh 46 of the first block of W. Chestnut Ave.

Driver #1 stated he was east bound on East Chestnut Avenue, just west of South Brewster Road. While traveling east bound he took his eyes off of the road in figure out where the Wawa entrance was located. Before he knew what was happening he struck a mailbox in front of 2557 East Chestnut Avenue,

Driver #1 stated he panicked and hit the gas pedal causing him to continue traveling east bound through the property and striking a sign in front of 2581 East Chestnut Avenue. It then that his vehicle came to a complete stop

Investigation at the scene revealed initial point of impact to be approximately 100 feet west of the eastern curb edge of South Brewster Road and approximately 2 feet south of the southern curb edge of East Chestnut Avenue. Point of impact was determined by information obtained at the scene.

THERE WERE NO INJURIES REPORTED AT THE SCENE

Vehicle #1 sustained damage to the front driver side of vehicle. The vehicle was towed from the scene by A1 Tow Service

Resident of 2557 East Chestnut Avenue (Thomas Masucci) was notified and made aware of the damaged mailbox.

I was unable to make contact with a representative for 2581 East Chestnut Avenue to advise them of the damaged sign.

No further information was made available at this time

Driver #1: Andrew Gouse 32 of the 1800 block of Wynnewood Drive

Pedalcyclist: Adonai Velasquez 23 of the 1300 block of S. Main Road

Driver 1 stated he was exiting the parking lot of 3419 South Delsea Drive, when he struck by a subject riding his bicycle on the sidewalk.  According to him, he did not see anyone while making his turn.

Next, I spoke to Adonai Velasquez, who stated he was riding his bike south bound on the sidewalk, when Driver 1 emerged from the driveway crossing the sidewalk, and the two of them collided.

While on scene, I had the opportunity to review surveillance footage from the business located at 3419 South Delsea Drive.  Surveillance revealed Driver 1 did not stop prior to crossing the sidewalk. Driver 1 crossed the sidewalk and was waiting for traffic on South Delsea Drive.  As he was waiting for traffic, Velasquez was riding his bike on the sidewalk southbound, and collided into the vehicle.  From the vantage point of the surveillance camera, it appeared Velasquez had enough time to avoid the collision.

Velasquez refused medical treatment on scene, after Vineland EMS arrived on scene.  Investigation revealed both parties had some type of fault in the collision due to the fact Driver 1 did not stop at the sidewalk, and Velasquez was riding his bicycle on the sidewalk against traffic. No summonses were issued to either party.

On 12/07/2015, I was dispatched to the area of South Delsea Drive and West butler Avenue in regards to a hit and run collision.

 

While en route to the scene, Vineland Dispatch reported that the accused vehicle was last observed traveling east on West Butler Avenue from South Delsea Drive.  The vehicle was described as an older model Mitsubishi sports car, white with a spoiler.  No registration could be provided by the caller.

 

Several patrol units began checking the area for the accused vehicle which later checked negative. 

 

Vineland Dispatch later advised that a witness (later identified as Joshua Kashow) called and stated that he observed the vehicle leaving the scene of the accident and was able to obtain a registration of NJ-J48FXJ.  The tag came back to a 1998 white Mitsubishi GS registered to a Hakeem L. Smith of 196 West Oak Road in Vineland, NJ.  The residence was checked by a patrol officer which was negative for the vehicle.

 

When I arrived I spoke with driver #1 who stated the following.  Driver #1 said she was traveling in the outside southbound lane of South Delsea Drive, approaching the intersection of West Butler Avenue.  Driver #1 said vehicle #2 passed her vehicle on the right when vehicle #2 struck the passenger front of her vehicle, which caused vehicle #2 to spin out in front of her vehicle.  Driver #1 said she pulled off to the west shoulder of South Delsea Drive when driver #2 drove off from the scene in his vehicle, traveling east on West Butler Avenue.

 

Driver #1 stated the vehicle is an older model white Mitsubishi, unknown registration, with previous body damage and a spoiler.  Driver #1 said she could not get a good look at the driver, however she said there were several black occupants inside of the vehicle.

 

Both occupants of vehicle #1 stated they were not injured as a result of the collision.

 

Once I left the scene, I spoke with Vineland Dispatch who provided me with contact information for Joshua Kashow.  Upon speaking with Joshua, he stated he was traveling south on South Delsea Drive, approaching West Butler Avenue.  Joshua said he saw vehicle #2 spin out and stop in the middle of South Delsea Drive, however he was unsure of the circumstances which caused it.  He said as he approached the intersection, he noticed vehicle #1 off of the roadway with the hazard lights activated when vehicle #2 proceeded to drive away.  Joshua said he believed a collision occurred when he immediately followed vehicle #2 and was able to obtain the registration for the vehicle, which Joshua then provided to this department.  No description for the driver could be provided.

 

During this investigation, I was able to locate a contact number for the registered owner of vehicle #2 (Hakeem Smith) when I left a message for him to contact me.  After several minutes I was contacted by Hakeem, who confirmed he was operating the vehicle, on this date, at the time of the collision from South Delsea Drive and West Butler Avenue.  Hakeem advised he would respond to this department to speak with me.

 

At the station, Hakeem stated he was traveling south on South Delsea Drive, when a vehicle merged into his lane.  He said he swerved to avoid striking the vehicle when he had drove on the right side of vehicle #1.  Hakeem said his vehicle then struck the passenger front of vehicle #1 when his vehicle spun out and he stopped in the middle of South Delsea Drive.  Hakeem said he was “frightened” by the incident when he drove from the scene.

 

Hakeem confirmed he was not injured during the collision and he showed me his vehicle which was parked in parking lot at Wood Street and North Sixth Street.  I observed the recent damage to the driver side rear which he confirmed is from the collision.  While checking the vehicle, I also noticed that the driver rear tire of the vehicle was completely bald.  I asked Hakeem about the tire and he said he purchased the car with that tire.  I informed him that the tire was not safe to operate the vehicle and he stated he would change the tire with a usable spare.

 

Hakeem was issued summons for improper passing, leaving the scene of a collision, failure to report a collision and unsafe tires with a court date to appear for December 21st 2015 at 9:00am.  A subpoena was issued to driver #1 and Joshua Kashow.

Driver #1: Ryan Reed 38 of Muncie IN

 

 

Driver #1 stated he was traveling South in his tractor trailer and attempting to make a right turn onto W. Garden Rd from N. Delsea Dr. when his trailer side swiped the traffic signal pole located on the northwest corner of W. Garden Rd and N. Delsea Dr.

Driver #1 reported no injuries at the scene and his tractor trailer was able to be driven from the scene with minimal damage.

The NJ State DOT was contacted by Vineland Police dispatch and advised that the traffic light pole was struck.

After the traffic light pole was struck; it fell over and caused damage to two vehicles on the car lot of AG Auto Group located at 2332 N. Delsea Dr.  I spoke with the sales manager, Angel Marrero who gave me the below listed information for the vehicles.

1) 2014 silver 2 door Chevrolet Camaro bearing VIN# 2G1FB1E30E913509.  The Camaro sustained damage to the driver's side front and rear fenders as well as the front driver's side bumper.

2) 2007 gray 4 door Acura TSX bearing VIN# JH4CL959X7C015015. The TSX sustained damage to the passenger side from the front bumper to the rear bumper.

Trailer information:  White 2016 GTD ST bearing IN registration P496705 and VIN# 1GRAA0627GB706638 owned by Celadon Trucking Services Inc. located at 9503 East 33rd St. Indianapolis IN 46235.

Driver was cited for careless driving.

Driver #1 stated he was traveling south on N Delsea Dr (about 1000' north of Weymouth Rd) when he saw a deer standing on the west side of the road not moving.  Driver #1 stated he was driving past the deer when it suddenly ran towards his vehicle (hitting the front passenger side bumper slightly and the rear passenger side door).  Driver #1 pointed to where the deer hit and there was hair visible on the door.

There were no complaints of pain at the scene.

There was minor damage to the front bumper and passenger side rear door.  The vehicle was driven from the scene by the owner.

No summons was issued.

Driver: Mikhail Adamchak 35 of Egg Harbor Township NJ

Tuesday, December 08, 2015 I responded to Mays Landing Road approximately 1300 feet East of Cornucopia Avenue for a single vehicle crash involving a deer. Upon arriving I spoke with Driver one who explained that he was traveling West on Mays Landing Road when a deer suddenly ran out in front of his vehicle. Driver one explained that the deer ran out of a field located on the north side of Mays Landing Road. Driver one stated he observed three deer in the field and was not able to avoid hitting the deer that ran out in front of him.

 

Vehicle one sustained damage to the front passenger side corner of the front bumper and to the front passenger side head light. There was no air bag deployment and driver one claimed no injuries at the scene. The Deer was located and found to be deceased on the side of the road.
 

No summonses were issued in this case.

Driver#1 stated she was stopped in traffic south bound on Brewster Road from Chestnut when Driver#2 struck the rear of her vehicle causing the collision.

Driver#2 stated he was stopped in traffic on Brewster Road behind Driver#1 when he thought she had started in traffic.  Driver#2 stated he struck the rear of Driver#1 causing the collision.  

Minor damage to both vehicles.  No reported injuries.  No summons issued.

Driver#2 stated she was stopped behind Driver#1 when she dropped her chap stick causing her to take her foot off the break causing her to rear end Driver#1.

No reported injuries.  Minor damage to both vehicles.  No summons were issued.

Driver #1 stated she was in the north outside lane of North Lincoln Avenue near East Landis Avenue.  Driver #1 said vehicle #2 which was traveling parallel to her vehicle in the inside north lane of North Lincoln Avenue.  Driver #1 said a vehicle stopped in front of vehicle #2 to make a left turn when driver #2 proceeded to merge right, into her lane of travel, when vehicle #2 struck vehicle #1.  Driver #1 said as a result of the collision, her vehicle also struck the east curb of North Lincoln Avenue.

 

Driver #2 said he was traveling north in the inside lane of North Lincoln Avenue.  Driver #2 said a vehicle was stopped ahead of him to make a left turn when he activated his right turn signal and checked his passenger mirror for traffic.  Driver #2 said the outside lane appeared to be clear of traffic when he merged over and collided with vehicle #1.

 

No injuries were reported at the scene and no witnesses to the collision could be located.

 

Driver #2 was issued a summons for unsafe lane change with a court date to appear for December 22nd 2015 at 9:00am.  A subpoena was issued to driver #1.

Driver#2 stated she was traveling east bound on Chestnut Avenue in the inside lane when she did not see Driver#1 stopped in traffic rear ending his vehicle.

No reported injuries.  Moderate damage to vehicle#2 and minor damage to vehicle#1.  No summons were issued.

Driver#1 stated he was traveling south bound on SW Blvd when he stopped at the four way stop signs located at Butler Avenue.  Driver#1 stated after stopping he proceeded southbound when Driver#2 disregarded the stop sign east bound striking his vehicle.

Driver#2 stated she was traveling east bound on Butler Avenue when she thought she had stopped at the stop sign.  Driver#2 stated she proceeded through the intersection when she struck vehicle #1.

Driver#2 was transported to the hospital for complaint of pain to her neck.  Driver#2 was issued a summons for failure to stop/yield 39:4-144.  Minor damage to both vehicles.

Driver #2: Annette Massari 56 of the 100 block of W. Arbor Ave.

On this date, December 9, 2015, I was dispatched to the intersection of  Delsea Drive and West Landis Avenue, in regards to a motor vehicle crash, with injuries.  I arrived on scene, and observed two vehicles in the intersection with extensive front end damage. I exited my Patrol car (806) and observed a group of people standing on the southwest corner next to vehicle #1.  I observed a woman standing there with a rag on her face up against her nose. I inquired if she was one of the drivers and she replied yes. I then asked where the other driver was, and one of the bystanders indicated she had went into the Denny's Restaurant (1001 West Landis Avenue) and appeared to be intoxicated.

I went into the Denny's Restaurant and observed a female (later identified as driver #1) standing at the counter.  I asked her if she was involved in the crash and she turned to me and replied that she was.  When driver #1 turned towards me, I could detect an odor of alcohol coming from her person and her eyes were droopy. I advised driver #1 to step outside with me, and she asked if she could go to the bathroom first and I advised her that she couldn't. 

I asked driver #1 if she had been drinking and she replied that she was "earlier".  I asked her if she knew what time it was, and she replied it was almost 7:00am. It should be noted the actual time was 0736 hours.  I brought driver #1 back to the scene, where Field Sobriety Tests were completed.  Driver # 1 was placed under arrest for driving while intoxicated, after failing the tests.  Driver #1 had indicated as we were walking back to the scene, that she was driving home, and was attempting to turn left onto Landis Avenue. She stated she had the green arrow and attempted to turn, when the crash occurred. 

Driver #2 was being cared for by Vineland EMS for complaint of pain to her face and left arm. Driver #2 was transported to the Inspira Health Network for further treatment.  Driver #1 had a complaint of pain to her hand, but refused medical treatment at the scene.

As I was conducting investigation, I was approached by William Holbrook and he related he witnessed the crash.  Mr. Holbrook indicated he was stopped facing north on Delsea Drive behind vehicle #1, waiting to make a left turn on to Landis Avenue.  Mr. Holbrook indicated the traffic light for north and south bound traffic was green, but the green left turn arrow was not lit, and driver # 1 turned left in front of vehicle #2, causing the crash.  He stated after the crash, vehicle #1 jumped the curb at the southwest corner and struck the traffic sign. 

A representative from the New Jersey Department of Transportation was notified and indicated the sign will be replaced.

Vehicle # 1 sustained extensive front end damage and was towed from the scene by A-1 Towing. Vehicle #2 sustained extensive front end damage to the front end and was towed from the scene by Johnson's Auto Body.

After driver #1 was processed and released to her mother, Diana Horn, I responded to the residence of driver #2, to return her paperwork and collect her statement.  Driver #2 indicated she was traveling south on Delsea Drive in the outside lane, going through the intersection. Driver #2 indicated she had a green traffic signal, and the arrow lights were not illuminated. She stated as she was going through the intersection, driver #1 turned in front of her and their vehicles collided. 

After clearing driver #2's residence, I placed a telephone call to Christopher Hawk, regarding the crash. Mr. Hawk's business card was given to me by Sgt. D'Arrigo at the scene, and he related that Mr. Hawk witnessed the crash.  When I spoke to Mr. Hawk he related he was stopped at the intersection facing east on Landis Avenue, when the crash occurred right in front of him.  Mr. Hawk stated he observed the beige Lexus (vehicle #2) going through the intersection and then the vehicles collided.  I asked if observed vehicle #1 turned in front of vehicle #2, and he stated that it appeared that is how the crash occurred. 

Driver #1 was issued the following summonses:

Summons # 0614-VC-8843 for reckless driving
Summons # 0614-VC-8844 for failure to yield at an intersection
Summons # 0614-VC-8845 for driving while intoxicated

A court date was set for December 22, 2015 at the Vineland Municipal Court, with a time set for 9:00am. Subpoenas were generated for both witnesses and driver #2, to appear in Court.

On 12/9/2015 I was dispatched to the intersection of W. Landis Ave and N. Delsea Drive in regard to a Motor Vehicle Collision.

On scene I spoke with Driver 1, Melinda Cruz who advised that prior to the collision she was traveling east bound on W. Landis Ave at approximately 30 mph. Cruz explained that as she approached the intersection of Coney Ave, numerous vehicle began to slow down due to heavy traffic ahead. Cruz stated that she also slowed down, and while doing so her vehicle was struck from behind by vehicle 2. Neither Cruz nor her passenger, Rosita Rodriguez complained of any pain or injuries while at the scene. Vehicle 1 sustained extensive damage to the rear bumper area.

Driver 2, Diana Thach stated that she was traveling east bound on W. Landis Ave as she observed vehicle 1 coming to a stop. Thach advised that she also attempted to slow down and come to a stop, however her vehicle was struck from behind by vehicle 3. Thach advised that as vehicle 3 struck her from behind, this caused her vehicle to move forward and collide with vehicle 1. Thach complained of no pain or injuries while at the scene. Vehicle 2 sustained damage to both the front and rear bumpers.  

Driver 3, Charnea Briggs stated that prior to the collision she was traveling east bound on W. Landis Ave behind vehicle 2. Briggs explained that she failed to observe traffic slowing down ahead, due to the sunlight which impaired her view. Briggs stated that she was blinded by the bright sun and this was the reason for the collision. Briggs complained of no pain or injuries while at the scene. Vehicle 3 sustained extensive damage to the front bumper area.

Briggs was issued a summons # 0614-VC-002792 for Careless Driving in violation of NJS 39:4-97. Both driver 1, Melinda Cruz and Driver 2, Diana Thach were issued subpoenas for the set court date.

Point of impact was determined to be on W. Landis Ave approximately 300 feet east of the east road edge of N. Delsea Drive and approximately 25 feet north of the south curb edge of W. Landis Ave.

Driver # 1 was traveling North on South Delsea Drive. Vehicle # 2 abruptly stopped in front of his vehicle leaving him no time to avoid the collision striking vehicle # 2 IN THE REAR OF THE VEHICLE.

Driver # 2 was traveling North on South Delsea Drive. A truck in front of vehicle # 2 hit the brake which caused her to slow down. Driver # 2 OBSERVED VEHICLE # 1 travel towards the back of her vehicle and was not slowing down. She believed driver # 1 was not paying attention when he struck the rear of her vehicle.

Front RIGHT occupant of vehicle # 2 complained of pain to her lower back and the child that was seated in a car seat behind the driver had neck pain. Vineland EMS responded on scene where both patients declined further treatment.

Vehicle # 1 had front end damage to the front bumper area and hood. Vehicle # 2 had damage to rear bumper area. Both vehicle were driven from the scene.

Driver # 1 was issued a motor vehicle summons #VC-004709 for following too close; 39:4-89.

On this date, December 9, 2015, I was dispatched to the Wawa (2802 South Delsea Drive) in regards to a motor vehicle crash on the lot.  When I arrived on scene, I met with both drivers and the following statements were made. 

Driver #1 indicated he was backing out of the stall and didn't see vehicle #2 and struck the driver side of the vehicle.  Driver #2 indicated she was stopped behind vehicle #1, waiting to pull up to the gas pump, when vehicle #1 started backing up.  Driver #2 stated she blew her horn, but vehicle #1 didn't stop and struck her vehicle. 

No injuries were reported and Vineland EMS was not needed at the scene. 

Vehicle #1 had no visible damage and was left on the property.
Vehicle #2 had a minor dent in the driver's side front door, but was able to be driven from the scene. 

After checking driver #1's credentials, it was learned that his vehicle registration expired (September 2014) and the vehicle wasn't insured.

Driver #2: Amie Boccelli 32 of Minotola NJ

Driver #1, stated at the scene she was traveling north on South Delsea Drive when an unknown vehicle also traveling north in front of her stopped on the roadway and made a left turn into 1668 S. Delsea Drive (Vineland Discounts Liquor).  Driver # 1 indicated she waited for a few seconds behind the unknown vehicle prior to her deciding to pass the vehicle on the right shoulder (east side of the road). Driver # 1 stated while she was passing the unknown vehicle on the right, her vehicle and vehicle # 2 collided.


Driver # 2, stated she was traveling north on South Delsea Drive when vehicle # 1 stopped on the roadway at which time she decided to pass this vehicle on the right hand shoulder. Vehicle # 1 suddenly turned right and it began to travel on the right shoulder at which time both vehicles collided. 

Both drivers claimed they did not suffer any injuries during the crash.

Vehicle # 1 had minor damage on the rear bumper, vehicle # 2 had moderate damage on the front passenger's side end. Vehicle # 2 was towed from scene by M&R Towing. 


Point of Impact (P.O.I) 40 feet north of the north curb line of Michael Avenue and 4 feet west of the east curb line of South Delsea Drive.

No summons were issued at this time.

Driver #2: Monica Brent 48 of the 700 block of Broadway

On 12/9/15 I responded to the above address for a motor vehicle collision in the parking.  On location I met with involved persons and they indicated the following. 

 

Driver #1 stated he was traveling along side of the curb to on the east side of the lane to make a left turn when Vehicle #2 attempted to cut him off and drove into him as he approached the stop sign. Driver #1 did not report any injury. 

 

Vehicle #1 sustained minor damage to the passenger side front and rear door. 

 

Driver #2 stated she just exited her parking stall and as she drove to the east side of the lane, Driver #1 drove around her from behind cutting her off and he then pulled in front of her causing their vehicles to strike each other. 

Driver #2 did not report any injury 

Vehicle #2 sustained minor damage to the front driver side bumper. 

 

Both vehicles were driven from the scene, no summons were issued.

Driver #2: Theresa Gould 58 of Bridgeton NJ

Driver #1 stated he was traveling north bound on South Orchard Road approaching West Sherman Avenue when he stopped at the intersection as the traffic signal was red. Driver #1 indicated his “foot slipped off the break” and he struck the rear of Vehicle #2. Driver #1 admitted for being at fault for the collision.

Driver #2 stated she was stopped at the intersection of South Orchard Road/ West Sherman Avenue facing north bound as the traffic signal was red. Driver #2 indicated shortly after being stopped at the intersection, Vehicle #1 “bumped” the rear of her vehicle (#2). Vehicle #1 rear ended vehicle #2.

Both drivers had no complaint of pain.

Vehicle #1 sustained minor damage to the front end.  

Vehicle #2 sustained minor damage to the rear end.  

No summons issued.
