Driver #1: Suquan Vega 23 of the 500 block of N. West Ave.

Driver # 1 stated he was south on the East Blvd when vehicle "X" was backing from a driveway, 2079 S. East Blvd.  Driver # 1 stated he swerved to avoid Vehicle "X" and struck a small section of concrete curbing on the west side of roadway.  He stated he then lost control of his vehicle and as he applied the brakes he slid into the front yard of 2079 Southeast Blvd.  Driver #1 told me his vehicle was unregistered, uninsured and he was an unlicensed driver.  

After a DMV look-up he was issued the following E-Tickets: 0614-E15-3184 for Unregistered Vehicle, 39:3-4; 0614-E15-3185 for Driving While Revoked/Suspended, 39:3-40; 0614-E15-3186 for Uninsured Vehicle, 39:6B-2 and 0614-E15-3187 for Fictitious Registration Plates.  The registration plates were seized and secured within the ID Section of the Vineland Police Department.  Vehicle # 1 was impounded by Olin's Towing.

Driver of Vehicle "X" stated she was backing from the driveway of 2079 Southeast Blvd when she saw Vehicle # 1 approaching, traveling southbound.  She stated she began to stop/slow until Vehicle # 1 passed however it lost control and came to a stop in the front yard.  She stated there was no contact between both vehicles.  

Vehicle "X" was a 2002 Subaru, New Jersey registration A88-ESV valid through 10/2016 operated by Shaina M. Capazzi, DOB 01/14/1888, New Jersey Driver's License C0513 70374 5182.

Driver #1: Joseta Seda 79 of the 1600 block of S. Orchard Road

Driver #2: Jose Gonzalez 54 of the 1000 block of Hendee Road

Driver # 1 advised that she was southbound on South Third Street, approaching West Elmer Street.  Driver # 1 said she stopped for a posted stop sign, and when she believed the roadway to be clear she proceeded forward.  Once in the intersection, Driver # 1 said her vehicle was struck by vehicle # 2, which was east bound on West Elmer St..

Driver # 2 stated that he was eastbound on West Elmer Street, approaching South Third Street.  Driver # 2 said as he approached the mentioned intersection, vehicle # 1 failed to stop for a posted stop sign in front of him, causing his vehicle to collide to vehicle # 1. 

Vineland Fire Department and Vineland EMS were contacted and responded to the scene due vehicle # 2 being overturned, and for the below listed complaints of pain from persons involved in this crash.  All occupants were transported to Inspira Hospital (1505 West Sherman Avenue).  

     1.  Driver # 1 - Complaint of pain to the chest and head.
     2.  Driver # 2 - Complaint of pain in the right shoulder and chest.
     3.  Driver # 3 - Complaint of pain in the right shoulder.  

Investigation revealed that Driver # 1 failure to stop for a posted traffic sign controlling her lane of travel was the cause of this crash.  Driver # 1 was issued summons # 0614 VC 004186 for failure to stop/yield (39:4-144).  Driver # 2 was issued a subpoena to appear in court for the summons issued against Driver # 1.

Several shrubs and a lawn was damaged on the property of 11 South Third Street.  The property owner, Carlos Rodriguez, was notified of the damage.

Driver 1 stated she was traveling North on Orchard rd. and a vehicle coming South on Orchard was passing someone in her lane so she swerved to the right to avoid a head on collision. She stated her vehicle left the roadway and struck a fire hydrant and then ran through a fence and came to rest inside the property of 775 N Orchard Rd.

Dispatch advised they attempted to make contact with the owner of the property with negative results at this time. A message was left on the machine advising of the damage.

Water utility was also notified by dispatch for the damage to the fire hydrant.

The gate to the property was opened by the fire department and secured by this officer after the car was removed.

The vehicle was towed from the scene by J&C auto.

The insurance company for the vehicle was Scott E. Sileo 1035 N. Black Horse Pike suite 6 in Williamstown, NJ 08094. No company number was listed on the insurance card.

No summons issued.

On this date, November 21, 2015, while traveling to Police Headquarters to start my tour of duty, I observed a green Kia Soul (Vehicle #1) traveling north on East Avenue in the area of the Knights of Columbus hall (1803 North East Avenue) with extensive front end damage.  As I continued passed the vehicle, I observed the driver was a white female and the last three digits of the license plate were GAG.  Behind the vehicle was a white SUV traveling in the same direction, but I wasn't able to capture any other information about the vehicle.

As I approached the intersection of East Avenue and Oak Road, I observed vehicle debris in the north bound lane of East Avenue.  I contacted Police Dispatch and inquired if they had any recent reported motor vehicle crashes in the area, and I was advised that they didn't. I advised Dispatch, once I start my tour of duty, I will return to the scene to investigate it further.

Once my tour started (0800 hours), I responded to the intersection of East Avenue and Oak Road, to investigate the incident further.  When I arrived on scene, I began checking the vehicle debris for any indication as to what make or model was involved, but I had negative results.  A local landscaping company drove past, and assisted with sweeping the lane of travel clear of the vehicle debris.  While I was on scene, I spoke with a resident who resides on the corner and she related the following information.  The woman identified herself as Ronda Gallo and she indicated she heard the crash occur, and went to the window to observe a green Kia had struck the rear of a older white SUV, possibly a Ford Explorer or a Nissan Pathfinder. Ms. Gallo indicated both vehicles pulled through the intersection, and stopped for a minute on the other side of the road, then they both pulled off traveling north on East Avenue.  She reiterated, that she didn't witness the actual crash, she observed the two vehicles stopped in the roadway and the damage to the two vehicles.  Ms. Gallo indicated the crash occurred at approximately 7:20 am, just after her alarm activate went off.  Ms. Gallo never provided a description of the drivers involved in the crash, nor did she see the license plates on the vehicles. 

Once the roadway, was cleared I began checking the area of the Cedar Crest Mobile Home Park (1976 North East Avenue) where I last seen the two vehicles, that were possibly involved.  As I was traveling past Cedar Crest, I observed a vehicle matching the description of vehicle #1 parked in one of the driveways.  I pulled into the park and confirmed vehicle #1 was parked at lot # 244, with several subjects standing around the vehicle.

I approached the subjects and inquired as to what occurred with the vehicle. The subjects indicated they weren't sure, but the owner of the vehicle was inside sleeping. As I looked at the vehicle, I noticed the fence surrounding the back yard was bent forward, as if the driver struck the fence when the pulled into the driveway. I asked one of the subjects standing there, if the driver struck the fence as well and he indicated that she did.  See Crash Report # 2015-67148, for further details regarding the crash at the residence. 

Due to the fact the vehicle had extensive front end damage, and the driver side airbag was deployed, I requested EMS at the residence.  I then entered the residence, located driver #1, who was an older white woman wearing pajamas and a bath robe. I asked her what happened to her vehicle and she indicated she didn't remember. I asked her where she was and she related this morning at 7:00 am, she went to the East Landis Motel and had two drinks with her son-in-law. She stated she left there and doesn't remember what happened.  As I was speaking with driver #1, I could not detect an odor of alcohol coming from her person, nor did she exhibit and signs of being intoxicated.


Driver #1 was issued the following summons as a result of the crash:
Summons # 0614-VC-8114 for careless driving
Summons # 0614-VC-8115 for leaving the scene of an accident
Summons # 0614-VC-9116 for failure to report an accident

A mandatory court date was set for December 7, 2015 at the Vineland Municipal Court for 9:00am.

Driver #1 stated she was driving through Boscov's parking lot, 3849 S. Delsea Dr., searching for a parking spot.  She stated she turned into a stall and drove through the stall to get to the connecting stall.  She stated as she reached the connecting stall another vehicle began to turn in.  She stated she swerved to avoid the second car and struck vehicle #2.

I spoke with the owner of vehicle #2, Willy Ramos, who advised he did not observe the collision.

Vehicle #1 sustained damage to the passenger side of the vehicle.  Vehicle #2 sustained damage to the front driver side corner of the vehicle.

No summons were issued due to the collision occurring on private property.

Driver #1 stated he was traveling west on Wheat Rd.  He stated as he passed vehicle #2 which was parked on the side north side of the roadway she attempted to make a U-turn.  He stated he attempted to swerve to avoid her but she struck the passenger side of his vehicle.

Driver #2 stated she was traveling west on Wheat Rd. at which time she pulled partially into the driveway of 402 E. Wheat Rd.  She stated she attempted to turn around to travel the opposite direction and did not see vehicle #1 at which time they collided.

Vehicle #1 sustained damage to the passenger side of the vehicle.  Vehicle #2 sustained damage to the front portion of the vehicle and was towed from the scene by All Tow.

Driver #2 was issued the following summons.

A subpoena was issued in regards to the above summons

Driver #2: Terry Mayes 39 of the 200 block of W. Laurel Street

Driver#1 stated she was in line at the McDonald's drive-thru and Driver#2 was in line in front of her. Driver#1 stated Driver#2 suddenly backed his vehicle up and struck the front of her vehicle with his trailer hitch.

Driver#1's vehicle sustained moderate front end damage. Driver#1 had no complaint of pain or injuries on scene.

Driver#2 stated he was in line at the McDonald's and decided he was going to leave. Driver#2 stated he backed up to leave the drive-thru line and backed into Driver#1's vehicle.

Driver#2's vehicle did not have any damage. Driver#2 had no complaints of pain or injuries on scene.

Driver#2 was issued the following motor vehicle summonses:

Driver 1 stated he was driving East on Vine Road when a deer ran through the road and struck the vehicle in the center of the front bumper. The driver was not injured and refused any medical attention.

Vehicle damage was on the front bumper. The bumper was fully dented in, onto the frame and collapsed in.  Driver estimates the damage to be approximately $6,000.

The road department was contacted for the deer which died as a result of this accident. The deer was moved out of the roadway in the wooded area.

Driver 2 stated he was traveling south on S Main Rd, approximately 100 feet north of E Grant Ave, when he heard 2 loud banging sounds. He looked in his rear view mirror and noticed vehicle 1 had struck the rear end of his vehicle (vehicle 1 was also traveling south on S Main Rd).

He immediately stopped his vehicle and vehicle 1 stopped as well. Driver 1 got out of her vehicle and driver 2 told her to call the police as well as provide him with her insurance information. Driver 1 stated that her phone was in her vehicle and she went back in her vehicle. Driver 1 then took off in her vehicle without providing her insurance information or contacting the police. Damage to vehicle 2 was on the rear bumper area.

Driver 1 was described as a white or Hispanic female, possibly between the ages of 40-50.

Vehicle 1 was described as a newer model black sedan, with a registration that possibly starts with "G" (unknown which state). Vehicle 1 should have front end damage from this collision.

Driver 2 stated she was stopped at the red light at W Park Ave / N Delsea Dr, facing south. Vehicle 1 struck her vehicle from behind. Driver 1 got out of her vehicle and started yelling at her. Damage to vehicle 2 was on the rear bumper area.

Driver 1 stated she was traveling south on N Delsea Dr and was distracted looking to the left at a building. She did not see how close she was to vehicle 2 that was directly in front of her. She struck vehicle 2 from behind. She stated it was completely her fault and that she caused the collision. Damage to vehicle 1 was minimal to the front bumper area.

Driver 1 was issued a Summons 0614-VC-008261 for following too closely and driver 2 was issued a subpoena for the set court date.

Driver 1 stated he was traveling south on S East Ave behind vehicle #1.  Driver 1 stated vehicle 2 began to stop so he began to go around vehicle 1, but was unable to stop when he realized it was turning into a driveway causing the collision. 

Driver 2 stated she was traveling south on S East Ave.  Driver 2 stated she put on her blinker and then began to turn into the driveway of 747 S East Ave when her vehicle was struck on the driver side by vehicle 2.

There were no injuries as a result of this collision.

Damage to both vehicles was moderate.

There were no summons issued.

Driver#1 stated she was traveling west on West Landis Ave, when Driver#2's vehicle came into her lane of travel, hitting her vehicle on the driver’s side door. Driver#1's vehicle sustained moderate damage to the driver’s side and was legally parked on private property.

Driver#1 had no complaint of pain or injury on scene.

Driver#2 stated he was traveling west on West Landis Ave, when he stated his steering wheel locked up. Driver#2 stated he hit Driver#1's vehicle on the driver’s side door, then ran the curb of the gas station located at 1402 W Landis Ave, striking a utility pole (BT23.5). The pole had minor damage and electric utility was notified.

Driver#2's vehicle had heavy front end damage and passenger side damage. Driver#2's vehicle had front airbag deployment. The vehicle was towed from the scene by All-Tow.

Driver#2 refused medical treatment by VEMS on scene and signed a refusal of treatment form.

Driver#2 was issued a motor vehicle summons for Failure to Maintain Lane 39:4-88B. He was issued a court date of 12/07/15 at 09:00am.

On 11/22/2015 while I was on patrol I was driving on Axtell Ave when I observed a fire hydrant (364 Axtell Ave) that was taken out of the ground. There was no water coming out of the ground from where the fire hydrant was disconnected to. The cause of how the fire hydrant was taken out is unknown but assuming it was struck by a vehicle or possibly a commercial vehicle. There was no debris around the area of the fire hydrant or and tire marks leading into the dirt around it. Water Utility was notified of the incident and would have someone respond to scene.

Crofton stated he was parked parallel to parked vehicle #2, just ahead of it, in the rear parking lot of Wawa, 2802 South Delsea Drive, Vineland, facing northbound.  He stated vehicle #1, a Wawa tractor trailer was traveling north through the parking lot attempting to turn right through the parking lot on the north side of the building.  He stated vehicle #1 had to back up to make the right turn.  When vehicle #1 then pulled forward it sideswiped vehicle #2.  He stated vehicle #1 should have damage to the left rear corner; however he was unsure of the extent of the damage.   

 

Vehicle #2 was parked at time of collision.  Driver was identified as Abel Cuevasbeteta (NJ DL # C91210020007772).  Vehicle #2 had trailer – NJ TJZ69F.   

 

Vehicle #1 was contacted by Wawa and returned to the scene.  Driver #1 stated he did not know he was involved in a collision as he did not feel the impact.  Driver #1 and his passenger claimed to have no injuries.

   Driver # 1, Shelby Flores stated that she was traveling west bound on E. Chestnut Ave at approximately 40 mph when the vehicle in front of her made an abrupt left turn onto Allen Ave. Flores stated that she quickly pressed her vehicle brakes and was narrowly able to avoid hitting the vehicle in front of her, however her vehicle was struck from behind by vehicle #2. Flores stated that she believed that Vehicle # 2 was traveling at her same rate of speed, but may not have had an ample amount of space to avoid the collision. Neither Flores nor her passenger, Fletcher Bennet complained of any pain or injuries while at the scene. Vehicle # 1 sustained damage to the rear driver’s side bumper area and was subsequently towed from the scene by Caldero's Towing.

     Driver #2, Dianne Johnson stated that prior to the collision she was traveling westbound on E. Chestnut Ave directly behind vehicle #1. Johnson advised that she observed a vehicle abruptly make a turn, causing vehicle # 1 to make a sudden stop to avoid a collision. Johnson advised that she also tried to stop, and avoid the collision as she pressed her brakes and veered into the oncoming traffic, however she did not have ample time as she struck vehicle # 1 from behind. Johnson complained of no pain or injury while at the scene, however passenger, Charles Bennet complained of a minor injury to his left knee. Bennet refused treatment at the scene and advised that he would respond on his own to the Hospital. Vehicle #2 sustained damage to the front passenger side fender, wheel, door and hood. Johnson advised that she would be calling AAA to later tow the vehicle. 

     Point of impact was on E. Chestnut Ave approximately 50 feet east of the east curb edge of Allen Ave and approximately 15 feet south of the north curb edge of E. Chestnut Ave. 

     Johnson was issued a summons # 0614-VC-002791 in violation of NJS 39:4-97. Flores was issued a subpoena for the set court date.

Driver 2 stated the light at the intersection of Grant Ave and Main road just turned green and the traffic was slowly moving when his vehicle was struck by driver 1. Driver 2 had no complaint of injury. Vehicle 2 did not sustain any damage.

Driver 1 stated he was stopped at the stop sign at the intersection of N Mill Rd/W Garden Rd (facing north). He thought it was his turn to proceed, so he attempted to make a left turn onto W Garden Rd. While making the turn, vehicle 2 struck his vehicle from the passenger side. Damage to vehicle 1 was on the passenger side area.

Driver 2 stated she was stopped at the stop sign at the intersection of N Mill Rd/W Garden Rd (facing south). She thought it was her turn to proceed, so she attempted to make a right turn onto W Garden Rd. While making the turn, vehicle 1 struck her vehicle from the driver side front bumper area. Damage to vehicle 2 was on the driver side front bumper area.

Both drivers were unsure on whose fault it actually was. Both drivers were advised of their options.

Monday, November 23, 2015 I responded to the intersection of North Seventh Street and East Park Avenue for a two vehicle crash. Upon arriving I first determined that none of the occupants involved in the crash were claiming any injuries.

 

I first spoke with driver one who explained that he was on Seventh Street attempting to make a left turn onto Park Avenue to begin traveling west on Park. Driver one explained that as he began making his turn he noticed driver two traveling East at a high speed. Driver one explained that he struck vehicle two as he pull out onto Park Avenue to make his turn.

 

Driver two explained that she was traveling east on Park Avenue when driver one suddenly pulled out and struck her vehicle on the passenger side.

 

Vehicle one sustained damage to the front right corner of the front bumper, hood, and light fixture. Vehicle one was driven from the scene by the driver. Vehicle two sustained damage to the front passenger fender, wheel, and door. Vehicle two was towed from the scene by Speedy’s Towing.

 

No summonses were issued in this case.

Driver #1: Suzanne Pavone 36 of the 2000 block of Musterel Lane

Driver #2: Galina Nechay 55 of the 2000 block of Cavallo Drive

 On Monday, November 23, 2015, I was dispatched to the location of East Sherman Avenue and Southeast Boulevard, for a two vehicle collision without injuries. 

 

    When I arrived on scene, I immediately confirmed that there were no injuries at this time. 

 

    I spoke to Driver 1, who stated as she was traveling eastbound on Sherman Avenue, she was driving in the inside lane. She reported as she was approaching the traffic light at the intersection of Sherman Avenue and the Boulevard, there was a vehicle at rest in the inside lane, waiting to make a left turn to head north on Southeast Boulevard. Driver 1 told me she made the lane change to the outside lane, and was in the lane for approximately two seconds when she was sideswiped by vehicle 2. The collision caused minor damage to the passenger side front quarter panel, and passenger side front and rear doors of Vehicle 1. 

 

    When I turned to Driver 2, she told me she was also traveling eastbound on Sherman Avenue, but in the outside lane. She added as she was crossing the intersection of Sherman Avenue and the Boulevard, Vehicle 1 changed lanes into her vehicle, causing minor damage to the driver’s side of Vehicle 2. The damage to Vehicle 2 was consistent with a sideswipe, and the damage traveled the entire length of the driver’s side of the vehicle. 

 

    Both vehicles were driven from the scene by the drivers. No summons were issued.  No injuries were reported.

Driver #1: Jeffrey Warrington 40 of the 100 block of Doren Terrace

Driver #2: Kelly Auerbach 49 of the 3000 block of Burnt Mill Drive

Driver #1 stated he was traveling east bound on W Oak Rd approaching the intersection of NW Blvd.  As he came up to the four way stop, he looked both ways before crossing over onto the railroad tracks, and up to the next set of four way stops at NE Blvd.  Driver #1 then indicated as he came up to four way stop at NE Blvd, he failed to look both ways before entering the intersection and struck driver #2's vehicle in the intersection.

Driver #2 stated she was traveling south bound on NE Blvd, approaching the intersection of E Oak Rd.  Driver #2 indicated she came to a stop and scanned in all directions for any vehicles, and observed driver #1 quickly crossing NW Blvd, and coming up to the railroad tracks before stopping.  Driver #1 then stated she proceeded to cross the intersection since she had the right of way, when her vehicle was suddenly struck on the passenger side of her vehicle. 

While gathering information from both drivers, driver #1 admitted to being at fault for not checking both ways for any vehicles prior to crossing.

Driver #1's vehicle sustained minor damage to the front driver side bumper, and was issued summons: 0614-VC-008625, 39:4-144 for failure to stop or yield.

Driver #2's vehicles sustained minor damage to the passenger front bumper and front quarter panel over the tire.

Neither driver complained of any injuries at this time.
