Driver #1: Louise Pernell 66 of the first block of Diane Court
On this date, November 18, 2015, I was dispatched to the area of East Chestnut Avenue and South Lincoln Avenue, in regards to a motor vehicle crash, involving a deer struck. When I arrived on scene, I located driver #1 stopped along the south side of the road, west of the intersection (Lincoln Avenue.) I positioned my Patrol car behind vehicle #1 and activated my emergency lights. I met with driver #1 and she related she was traveling east on Chestnut Avenue, approaching Lincoln Avenue when a deer ran out front of her vehicle from the south side of the road. She indicated the deer struck her front bumper/grille area and continued in a northern direction before collapsing on the edge of the road. Driver #1 indicated she wasn't injured and Vineland EMS was not needed on scene. I went and checked on the deer, and found that it was deceased. Police Dispatch was advised to contact the Cumberland County Road Department to remove the deer from in front of 3194 East Chestnut Avenue. Vehicle # 1 sustained minor damage to the front end, but was able to be driven from the scene.

On November 18, 2015, I was dispatched to Aldi's, located at 2651 South Delsea Drive, in regards to a motor vehicle collision. Upon my arrival, driver #1 advised that the collision occurred on Sherman Avenue, West of Delsea Drive. Driver #1 stated while traveling East on Sherman Avenue, in the left turning lane, driver #2 entered his lane from the right side and caused the collision. Driver #2 stated while stopped in traffic, facing in a North-East direction, after entering the left turning lane, driver #1 hit his vehicle. No injuries reported. Upon checking, the impact area was determined by debris on the road. Due to no witness(s) being located, and the driver's statements, both drivers were issued traffic summonses. Driver #1 was issued Careless Driving, summons #VC007518. Driver #2 was also issued Careless Driving, summons #VC007519. Traffic subpoenas were also completed for both driver's.

Driver # 1 stated he was south bound on North Delsea Drive, preparing to make a left turn onto Almond Road. Driver # 1 stated that he saw a vehicle stopped on the north bound lane of North Delsea drive, just prior to Almond Road. Driver # 1 stated that he believed the stopped car was waiting for him to make a left turn, and when he didn't see any other vehicles approaching from the south, he began to execute his turn. While in the middle of the turn, Driver # 1 said his vehicle was struck by vehicle # which appeared to pass the stopped car. Driver # 1 said that at no time did he see vehicle # 2 approach his location. Driver # 2 stated that she was north bound North Delsea Drive, approaching Almond Road when vehicle # 1 suddenly made a left turn from the south bound lane of North Delsea Drive in front of her, causing the two vehicles to collide. Driver # 2 never mentioned passing a stopped vehicle prior to the crash. Driver # 2 had no complaints of pain from the collision but was visibly shaken from the incident. Vineland EMS transported Driver # 2 from the scene to Inspira Hospital so she could be evaluated. Driver # 1 was issued a summons # 0614 VC 003196, for failing to yield right of way at an intersection (39:4-90). Driver # 2 was issued a subpoena to appear in court for the violation issued against Driver # 1.

On this date, 11/17/15 Ofc. K. SANTIAGO investigated a "hit & run" at N. West Ave. and W. Park Ave. at the time the accused vehicle described as a white Cadillac "Escalade" (pick-up truck style) with an "out of state tag" of "HI607". NOTE: The accused vehicle rear-ended the victim, and drove off. At the time the vehicle was unable to be located. On this date, 11/18/15 I was dispatched to 848 S. Delsea Dr. (Affordable Tire) in regards to the victim (Ishaun SOTO) locating the accused vehicle from yesterday's "hit & run". Upon arrival I spoke to SOTO who stated she was driving by the business, and saw a white in color Cadillac "Escalade" pick-up truck, so she went to the business to get a closer look at the vehicle, and noticed the vehicle had an out of state tag (South Carolina) and the registration was "HIL607". SOTO stated she then called the police to report she had found the "hit & run" vehicle from yesterday. The driver of the accused vehicle was located inside of Affordable Tire as an employee, and identified as Juan PEREZ-RUIZ. NOTE: PEREZ-RUIZ only speaks Spanish, and Ofc. C. ORTIZ responded for translation. Ofc. ORTIZ translated the following statement that PEREZ-RUIZ did not realize he had struck the other vehicle. NOTE: SOTO'S vehicle had minor damage, and PEREZ-RUIZ'S vehicle had white paint transfer to the front driver's side "tow hook". Insurance information for vehicle # 2 is: United Automobile Insurance Company (UAIC) P.O. Driver #1 stated he was stopped in traffic at the stop light (facing east) on W Landis Ave at the intersection of Mill Road in the left travel lane. Driver #2 said he realized he needed to get into the other lane and after he looked and did not see a vehicle, he attempted to change lanes into the right travel lane. Driver #1 said he then struck Vehicle #2 which he believes was located in his blind spot in the right travel lane. Driver #2 stated he was stopped in traffic, in the right travel lane, at the light (facing east) on W Landis Ave at the intersection of Mill Road when he saw Driver #1 slowly enter his lane from the left travel lane. Driver #2 stated he beeped his horn, but Vehicle #1 struck his vehicle. Driver #1 had no complaint of pain. Driver #2 had a complaint of pain to his left leg and was evaluated by Vineland EMS. Vehicle #1 suffered slight damage to its front passenger side fender and was driven from the scene. Vehicle #2 suffered damage to its driver side tire and front bumper. Vehicle #2 was parked in the parking lot of 1921 W Landis Ave to be picked up by Driver #2's towing company. No Summonses were issued.

On 11/18/2015 at approximately 1422hrs, I was dispatched to 775 South Sixth Street, A6B in regards to a hit and run collision. Upon my arrival, I met with the owner, Carmen Santos, of the struck parked vehicle (#2). Speaking with Santos, she related she parked her vehicle in front of her residence on South Sixth Street facing north at approximately 1400hrs. Santos stated when she walked back to her vehicle at approximately 1420hrs, she noticed damage to the rear driver side quarter panel. When Santos noticed the damage she immediately contacted this police department. Observing the damage, it is consistent with a vehicle sideswiping it. Vehicle #2 sustained moderate damage to the rear driver side. I did not observe any paint transfer on the vehicle. Speaking with Santos, she could not provide an accused for the collision. There are no video surveillance equipment in the area and I could not locate any witnesses. My investigation revealed, an unknown vehicle bearing unknown registration struck Vehicle #2 while traveling northbound on South Sixth Street. Vehicle #1 should have damage (unknown amount) on the passenger side. Investigation pending...

On this date, November 18, 2015, I was dispatched to the intersection of Delsea Drive and West Landis Avenue, in regards to a motor vehicle crash. I arrived in the area and observed both vehicles in the Right Turn lane of Landis Avenue just west of the intersection. I approached both drivers and confirmed there were no injuries and had them move off the roadway. I then met with both drivers in the parking lot of Rite Aid (7 West Landis Avenue) and the following statements were made. Driver #1 indicated he was traveling east on Landis Avenue and entered the right turn lane, to travel south on Delsea Drive. Driver #1 indicated he observed vehicle #2 stopped in the lane and applied his brakes, but they were not functioning properly and he tapped the rear of vehicle #2. Driver #2 indicated she was stopped on Landis Avenue facing eastbound, waiting to turn left (south) onto Delsea Drive, when she was struck from behind. No injuries were reported and Vineland EMS was not needed at the scene. Vehicle #1 had no visible damage and was able to be driven from the scene. Vehicle #2 sustained minor damage (crack to the bumper cover) and was able to be driven from the scene. Note: Vehicle # 1 is registered to Leafy Green Landscaping LLC and was towing an enclosed trailer (NJ REG: TKU67S), which was not damaged as a result of the crash.

On Wednesday, November 18, 2015, I was dispatched to the intersection of Main Road and Chestnut Avenue, for a two vehicle collision.
When I arrived on scene, I immediately called for EMS to respond to my location, due to Driver 1 having a complaint of head and neck pain, as well as her being unclear as to what had happened during the collision and a small lump on her forehead. Driver 2 refused medical treatment at this time.
I spoke to Driver 1, who stated she was heading northbound on Main Road, attempting to make a left turn into the parking lot (TD Bank/CVS) on the West side of the road, just South of Chestnut Avenue. She stated she was in the turn lane waiting until opposing traffic was clear. Driver 1 added the traffic light showing northbound and southbound was yellow, when all the opposing traffic had passed. She then proceeded to make her left turn. Driver 1 concluded by saying she was turning when she was struck by Vehicle 2, causing major damage to the front corner of her vehicle (passenger side,) thus disabling her vehicle.
I looked inside Vehicle 1 and noticed that the driver’s seatbelt was locked to the rear, indicating that Driver 1 was not wearing her seatbelt at the time of the collision.
I turned to Driver 2, who stated as she was heading southbound on Main Road, she went through the yellow light showing northbound and southbound, and was struck by Vehicle 1 when she attempted to make a left turn into oncoming traffic, colliding with her vehicle. The damage from the collision was to the driver’s side front corner, and also disabled her vehicle in the roadway.
The driver’s seatbelt of Vehicle 2 was locked in a forward position, indicating the driver of Vehicle 2 was wearing her seatbelt at the time.
Vehicle 1 was towed from the scene by Olin’s, and Vehicle 2 was towed from the scene by All-Tow. Driver 1 was taken to Inspira Medical Center by Vineland EMS. Driver 1 was issued the following summonses: 0614-VC-8762 for Failure to Wear Seatbelt (39:3-76.2f) 0614-VC-8763 for Improper Left Turn (39:4-123B)

Driver #1 stated she was stopped at the traffic light at the intersection of W Sherman Ave and S Delsea Dr. Drive #1 stated she was in the left turn only lane. Once the traffic light turned green for she was struck by Vehicle #2 on the rear bumper of Vehicle #1 causing minor damage. Driver #1 and Passenger#1 claimed no injuries.
Driver #2 stated he was stopped at the traffic light at the intersection of W Sherman Ave and S Delsea Dr. Drive #2 stated he was in the left turn only lane behind Vehicle #1. Once the traffic light turned green he believed Vehicle #1 proceeded into the intersection. Driver #2 then moved forward striking Vehicle #1 with his front bumper causing minor damage. Driver #2 claimed no injuries. Driver #2 was issued a summons for careless driving (39:4-97). 0614 VC 007051.
Both Vehicles were able to be driven from the scene.
Point of impact was approximately 25ft north of the south curb edge of W Sherman Ave and approximately 70ft south of the east curb edge of S Delsea Dr.

Driver # 1 stated she was Eastbound on W. Landis Ave. waiting to turn left (Northbound) onto N. Mill Rd. Driver # 1 stated as she went to turn left she struck vehicle # 2 which she stated "came out of nowhere". Driver # 2 stated he was also Eastbound on W. Landis Ave. and when he approached the intersection of Mill Rd. he stated that vehicle # 1 was "just sitting there" with no turn signal on. Driver # 2 stated that he thought vehicle # 1 was broken down or had "stalled out", so he turned left (Northbound) onto N. Mill Rd. Driver # 2 stated as he turned left, vehicle # 1 turned left, striking his vehicle. There were no injuries reported at this time. Vehicle # 1 was towed by J & C Towing. Due to the varying statements given, and no witness' located no Summons' were issued at this time. Both drivers were advised of their options.

Driver #1 stated she was traveling east bound on Mays Landing Rd, and approached a vehicle that was traveling slower than she was. Driver #1 indicated she then turned on her blinker to indicate she wanted to pass the vehicle, and turned into the west bound lane to attempt to pass the slow moving vehicle. As she entered the west bound lane she observed 2 other vehicles in front of the initial vehicle she wanted to pass. Driver #1 then stated she continued to try and pass all 3 vehicles, but then observed a vehicle approaching her quickly in the west bound lane (opposite direction). Driver #1 was unable to cut back into the east bound lane, and attempted to turn into the driveway of 6140 Mays Landing Rd. As she tried to turn, since the roadway was wet from a brief drizzle of rain prior to the accident, driver #1 lost control of her vehicle striking and going over top of the curb before coming to a stop about 50ft off the road way into the surrounding trees. Witness #1 stated she was traveling west bound on Mays Landing Rd, and observed multiple vehicles approaching her from the opposite direction. As she reached the vehicles approaching her from the opposite direction, she observed driver #1 pull out quickly from behind the last vehicle and come towards her head on. Witness #1 stated she then quickly pulled over towards the shoulder of the road to avoid any collision and allow driver #1 to pass her coming from the opposite direction, but driver #1 then turned quickly into the driveway of 6140 Mays Landing Rd. After driver #1 turned into the driveway, witness #1 observed driver #1 strike and go over the curb into the trees. Witness #1 then stopped her vehicle and rushed over to aid driver #1 and remained there until police arrived. Driver #1 was transported to INSPIRA Medical Center to be treated for back and chest pain. Her vehicle sustained major damage and was towed from the scene by J&C Auto. Driver #1 was issued a summons 0614-VC-008624, 39:4-85 Improper Passing.

Driver #2 stated at the scene she was traveling West on Walnut Rd activated her school bus lights and had stopped in front of Walnut Manor Apartments (794 E Walnut Rd) when vehicle #1 crashed into the rear of her vehicle. Driver #2 and the passengers in vehicle #2 were not injured. Driver #1 was issued a summons for Careless Driving, 39:4-97, summons #0614 E15 003198.
