Driver #1 stated after stopping at the posted stop sign at SW Blvd and Butler Ave, he continued south bound. According to driver#1, as he continued through the intersection, vehicle #2, which was traveling east bound through the intersection, failed to stop and collided into the rear of his vehicle. Driver #2 stated she was traveling east bound on Butler Ave, and admitted fault of the collision by failing to stop at the posted stop sign and colliding into vehicle #1. As a result of the collision, both vehicles sustained minor damage. Vehicle #1 sustained minor damage to its passenger side rear quarter fender and tail lamp assembly. Vehicle #2 sustained minor damage to its front bumper and passenger side front quarter fender. Neither of the drivers were injured. A summons was issued to driver #2 for failure to stop / yield: VC-007734.

On this date, November 16, 2015, I was dispatched to Brewster Road and Chestnut Avenue, in regards to a motor vehicle crash. When I arrived on scene, I observed two vehicles pulled to the side of the road, just south of the intersection, both facing southbound. I positioned my Patrol vehicle behind the involved vehicles and activated my emergency lights. I approached vehicle #1 and inquired if driver # 1 was injured and she related that she wasn't. I then approached vehicle #2, and observed driver # 2 standing on the passenger side of the vehicle with two small children. I inquired if anyone was injured and driver #2 indicated she doesn't believe so, but they were covered in glass and would like the children evaluated. I requested Vineland EMS, and a short time later EMS Unit # 624 arrived on scene and began tending to the children. I then spoke with driver # 2 and she related she was traveling south on Brewster Road and had the green traffic signal. She stated as she was passing through the intersection, vehicle #1 ran the red light and struck her vehicle on the passenger side. I then met with driver #1 and she related she was traveling east on Chestnut Avenue and stopped for the red light at the Brewster Road intersection. Driver #1 indicated no one was coming and she started to turn, when she noticed vehicle #2 coming at a high rate of speed. Driver #1 indicated she then struck the side of vehicle #2. Vehicle #1 sustained moderate damage to the front end and was towed from the scene by J&C Towing. Vehicle # 2 sustained extensive damage to the passenger side, and was towed from the scene by All Tow. I then spoke with the witness, Will Henderson and he related, he just pulled out of the Wawa onto Brewster Road and was traveling north towards the intersection. Mr. Henderson indicated vehicle #2 was coming south on Brewster Road and had the green light. He then indicated vehicle #1 didn't stop for the traffic light and turned into the side of vehicle #2 Driver # 2 was issued summons # 0614-VC-8111 for failure to observe a traffic signal (39:4-81) and was given a court date set for December 1, 2015 at the Vineland Municipal Court. A subpoena was generated for driver #2 and the witness, in the event driver #1 pleads not guilty to the summons issued to her.

Driver #1 could not recall the crash. Driver #1 said she had not slept in over 24 hours and she appeared to be fatigued. Vineland EMS responded and transported driver #1 to Inspira Medical Center for further medical evaluation. Driver #2 stated at the scene she was traveling West on Chestnut Ave in the inside lane and was stopped at the Main Rd intersection which is controlled by a traffic signal. Driver #2 said the traffic signal turned green for westbound traffic on Chestnut Ave so she entered the intersection, when vehicle #1, which was traveling North on Main Rd, crashed into her vehicle. Driver #2 said vehicle #1 failed to stop at the red light. Driver #2 was not injured. Driver #2 was issued a summons for Failure to Observe Signal, 39:4-81, summons #0614 E15 003160

Driver 1 stated he was traveling north on N Main Rd and attempted to apply his brakes. He stated his brakes were not functioning properly and could not stop before striking vehicle 2. He struck vehicle 2 from behind. Damage to vehicle 1 was on the front passenger side front bumper area. Driver 2 stated he was stopped in traffic on N Main Rd, facing north. He stated, vehicle 1 came from behind and struck his vehicle. Damage to vehicle 2 was on the driver side rear bumper area. Driver 1 was issued Summons 0614-VC-007780 for following too closely, driver 2 was issued a subpoena for the set court date.

On this date, November 16, 2015, I was dispatched to the area of the main entrance of the Cumberland Mall (3849 South Delsea Drive) in regards to a motor vehicle crash. When I arrived in the area, I located both vehicles pulled into the parking lot of Arby's (3850 South Delsea Drive.) I pulled into the lot and met with both drivers and the following statements were made. Driver #1 indicated he was facing southbound on Delsea Drive stopped behind vehicle #2 at the intersection that enters the Cumberland Mall. Driver #1 indicated the traffic signal turned green and he took his foot off the brake and his vehicle "bucked, lunged forward, and stalled" after striking the rear of vehicle #2. He stated he restarted his vehicle and followed vehicle #2 into the parking lot. Driver # 2 stated she was stopped in traffic facing southbound on Delsea Drive, when her vehicle was struck from behind. Driver # 1 indicated he wasn't injured, but driver #2 indicated her lower back hurt as a result of the crash. Vineland EMS was requested at the scene, and arrived on scene a short time later. Driver #2 signed a refusal with EMS and indicated she would go to the hospital at a later time. Vehicle #1 had no visible damage and was able to be driven from the scene. Vehicle #2 had minor scuffs to the rear bumper cover and was able to be driven from the scene. No summonses were issued, the crash was deemed reportable, due to the fact driver #2 had a complaint of pain.

On 11/16/2015 at approximately 1719hrs, I was dispatched to police headquarters (111 North Sixth Street) in regards to collision. Upon my arrival, I met with Hector M. Reyes who related the following: Today (11/16/2015) at approximately 1630hrs, Reyes (Driver #2) was exiting the parking lot of Burger King (315 North Delsea Drive) onto North Delsea Drive when a vehicle (X) let him merge onto the roadway to make a left hand turn. When Reyes started merging onto the roadway a vehicle (#1) passed Vehicle X on its right side and sideswiped the front of Vehicle #2. Vehicle #1 continued on North Delsea Drive and cut through private property to travel east bound on West Park Avenue. Reyes did not report the incident right away as he thought he needed to go to the car dealer first to report the collision as it is a new vehicle. After Reyes responded to the car dealer, he responded to police headquarters to report the incident. Reyes described the accused vehicle (#1) as a red SUV bearing an unknown registration. It should be noted Officer M. Bonsall was dispatched to the area of North Delsea Drive/ West Park Avenue at approximately 1631hrs for a hit and run collision (Case #66283-15). The caller was a witness of the incident and followed the accused vehicle east bound on Park Avenue and observed the vehicle pull behind a residence across the street of 502 Lynn Avenue. The caller left the area and Officer M. Bonsall responded in. At the time, Officer M. Bonsall could not locate the victim of the hit and run collision so he cleared the area. At that time, the caller did provide a call back number but, did not pick up. The caller also described the accused vehicle (#1) as a red Ford Excursion. I contacted Police Dispatch and contacted the above witness. Speaking with the witness, he provided a statement that corroborated with Driver #2. Witness #1 stated the vehicle could be a Ford Excursion or Expedition and it had two Hispanic male occupants. Witness #1 indicated the vehicle (#1) pulled behind a residence across from 502 Lynn Avenue. The witness left the area as he did not want to walk onto the property and did not get a tag number. Driver #2 and his occupant did not have a complaint of pain. I provided Driver #2 with the case number to my investigation and sent him on his way. Prior to Driver #2 leaving, I observed the damage to Vehicle #2 and it sustained moderate damage to the front bumper. Vehicle #1 should have sideswipe damage to the passenger side. I responded to the area of 502 Lynn Avenue and located the residence across the street which is 501 Lynn Avenue. Knocking at the door, I met with a Kevin Ortiz. I advised Ortiz of the incident and described to him the vehicle (#1) which I was looking for. At that time, Ortiz stated the vehicle I am looking for belongs to his friend, Daniel Orlando, who also resides there but, is not there at the time I responded. I asked Ortiz if he could contact Orlando at which time he did. Shortly after speaking with Ortiz, Orlando arrived at the residence with a red Lincoln SUV. Observing the vehicle, I noticed it sustained white paint transfer damage on the passenger side (sideswipe). Orlando was seated in the passenger seat while I met with a driver by the name of Christian E. Soto-Salaberrios. I asked Soto where the damage came from at which time he admitted to being at fault for a collision in the area of North Delsea Drive/ West Park Avenue. Soto stated he was "nervous" and "afraid" as it was his first time in a collision which caused him to flee the scene. Soto (Driver #1) stated he was traveling north bound on North Delsea Drive approaching West Park Avenue when he observed Vehicle #2 exiting Burger King at the last second and sideswiped the front of the vehicle (#2). Driver #1 and his occupant did not have a complaint of pain. Vehicle #1 sustained moderate damage to the passenger side. Driver #1 was issued summons # 0614-VC-008318 (Careless Driving; 39:4-97) and # 0614-VC-008319 (Leaving the scene of an accident; 39:4-129b). Witness #1 was issued a subpoena with this report. Monday, November 16, 2015 I responded to West Landis Avenue and Mill Road for a two vehicle crash. Upon arriving I first verified that none of the occupants were claiming any injuries. I first spoke with driver two who explained that she was traveling East on Landis Avenue in the far right lane of travel. Driver two explained that driver one attempted to change lanes from the left lane to the right. According to driver two, as driver one began changing lanes, he struck her vehicle on the front driver’s side fender.
Upon speaking with driver one, he explained that he was also traveling East on Landis Avenue in the left lane of travel. Driver one explained that he wanted to change lanes and move into the far right lane because there was a vehicle turning up ahead in his lane. Driver one stated that prior to changing lanes he checked to be sure the right lane was clear and noticed vehicle two back in the distance. Thinking the lane was clear, driver one said he made his lane change and struck vehicle two with his rear passenger side tire and fender. Driver one told me that he believed that driver two sped up just prior to him making the lane change which caused him to collide with her.
Vehicle one sustained minor damage to the rear passenger side fender and was driven from the scene. Vehicle two sustained minor damage to the front driver’s side fender and was also driven from the scene.
No summonses were issued in this case.

Pedestrian: Octavio Rosa 72 of the 100 block of W. Walnut Road
On 11/16/2015 at 1825hrs, I was dispatched to 189 West Walnut Road in regards to a hit and run collision. Police Dispatch relayed the collision occurred at approximately 1730hrs and the accused vehicle reportedly came from a neighboring residence. Upon my arrival, I met with Octavio Rosa who related the following: Today (11/16/2015) at approximately 1730hrs, Rosa was in the roadway (West Walnut Road) in front of his residence raking the leafs into a pile on the shoulder of the roadway. While Rosa was in the roadway, a vehicle (#1) backing out of the driveway of 202 West Walnut Road struck him. Rosa stated the vehicle did not strike him at a high rate of speed and was traveling slowly. Rosa described Vehicle #1 as a white or crème four door vehicle bearing an unknown registration and being driven by a black male. Rosa could not provide further description of the vehicle and after he was struck it fled east bound on West Walnut Road towards the Boulevard. Rosa waited for his wife to get home and tell her as he did not want any problems from his neighbors as they "are good people." Rosa only had a minor complaint of pain to his right leg. When Rosa's wife was notified, she contacted this police department. Due to Rosa having a complaint of pain, I requested Emergency Medical Service (EMS) to respond to the scene. EMS arrived on scene and tended to Rosa but, Rosa refused medical treatment. It should be noted the roadway is not illuminated brightly and dark where Rosa reportedly stood. Checking the area, I did not observe any video surveillance equipment in the area. I walked over to 202 West Walnut Road and met with Vivian Edwards (856-896-8749) who was advised of the incident. Edwards was apologetic and concerned for Rosa's wellbeing. Checking the driveway, I did observe a white new model vehicle but, it was parked with the front facing West Walnut Road. Edwards stated that is his vehicle and he never backs it out of the driveway due to how dangerous the area is with traffic. Speaking with Edwards, he stated two of his friends stopped over to check on his heater as it was malfunctioning. Edwards could not provide anything further regarding the two of his friends other than their first names, "Earl" and "Noel." Edwards indicated he met the two subjects while he was at work and they advised they could fix his heater. Edwards was unsure what vehicles they drive and stated both came in separate vehicles. Edwards advised it is possible they could have backed up into Rosa. Edwards stated, "It is dark here and difficult to see." Edwards also stated he will speak with them tomorrow (11/17/2015) while at work regarding the incident and attempt to get further. I advised Rosa of my findings and he was content. Rosa believes his neighbors are good people and believes they would not have done it purposely. Vehicle #1 has unknown damage on the rear bumper. No summons issued at this time. Investigation pending...
