Driver #2: Elizabeth Hamlyn 26 of Millville NJ
Driver #1 stated while coming off of Route 55 exit 27 he approached the Red traffic signal at Hennis Road. Once the traffic signal turned Green he began to make a left hand turn on to Hennis Road. As he began turning another vehicle, driver #2, began traveling through the intersection when she was not safe to do so. It was then that their vehicles collided Driver #2 stated she was traveling west bound on Hennis Road approaching the traffic signal just east of South Delsea Drive As she approached the traffic signal she thought she had the Green light so she continued to travel through the intersection. It was then that the two vehicles collided Investigation at the scene revealed point of impact to be approximately 150 feet east of the eastern curb edge South Delsea Drive and approximately 10 feet south of the northern curb edge of Hennis Road. Point of impact was determined by the statements of both drivers. There are no complaints of pain and or injury at the scene from either involved driver Vehicle #1 sustained damage to the front passenger side of the vehicle. The vehicle is driven from the scene by the operator Vehicle #2 sustained damage to the front driver side of the vehicle. The vehicle was left on private property and the driver advised she would have it removed as soon as possible

Driver #2: Allan Tubertini 64 of the 3000 block of S. Lincoln Ave.
Driver#1 stated she was stopped at the stop sign on S Brewster Rd, awaiting to turn south, on South Lincoln Ave, when she felt impact to the rear of her vehicle. Driver#1 stated Driver#2 rear ended her.
Driver#1’s vehicle sustained minor damage to the rear fender. Driver#1 had no complaint of pain or injuries on scene.
Driver#2 stated he was stopped at the stop sign on S Brewster Rd, awaiting to turn south, on South Lincoln Ave, when Driver #1 began to turn. Driver#2 stated Driver#1 suddenly stopped and he rear ended Driver #1.
Driver#2’s vehicle sustained minor damage to the front bumper. Driver#2 had no complaint of pain or injuries on scene.
Driver#2 was issued a motor vehicle summons for Careless Driving 39:4-97 0614VC-008473.

Driver #2: Sharday Lebron 19 of the 1000 block of E. Park Ave.
Driver #1 stated he was traveling south on North Main Rd, when Driver#2 suddenly pulled out of Walgreens in front of him. Driver #1 stated he was unable to avoid Driver#2, as she struck his vehicle on the driver’s side door, causing moderate damage.
Driver #1 and passengers were checked out on scene as a precautionary measure and all refused medical treatment from VEMS.
Driver #1 was issued a motor vehicle summons for not having a child properly restrained 39:3-76.2A.
Driver #2 stated she was turning out of Walgreens (1015 N Main Rd) south on N Main Rd and believed she had enough time to turn in front of oncoming Driver#1’s vehicle. Driver#2 collided with Driver #1’s vehicle, causing minor damage to the front passenger’s side quarter panel.
Driver #2 and passengers were checked out on scene as a precautionary measure and all refused medical treatment from VEMS.
Driver #2 was issued a motor vehicle summons for failing to yield to oncoming vehicle 39:4-90.

On 10/21/15 I was dispatched to the area of 75 Melrose Street for a possible collision. Upon arrival to the area I was advised by a neighboring resident that a small dark colored sedan struck a parked van on the east side of Melrose Street and fled traveling north bound on Melrose Street. While on-scene I observed a 2000 Chevrolet Venture (NJ Reg: D80 FXJ) on the east side of the road with minor front end damage. I located the registered owner of the vehicle, Terrell Bell, who stated the vehicle was unoccupied and that the damage I observed was new damage. Investigation at the scene revealed point of impact to be approximately 300 feet north of the north curb edge of West Chestnut Avenue and approximately 10 feet west of the eastern curb edge of Melrose Street. Point of impact was determined by information obtained at the scene. Vehicle #1 (Unknown Make / Model / Reg / Color) is believed to have sustained minor front end damage. No further information for the accused vehicle was obtained. Vehicle #2 (Parked) sustained minor front end damage and was left at the scene. THERE WERE NO INJURIES REPORTED AS THIS TIME

Driver #2: Gabrielle Friedman 22 of the 1300 block of S. Lincoln Ave.
Driver #1 stated at the scene that she had backed out of a parking stall, and checked for traffic but did not observe anything. Driver #1 advised while backing she was suddenly struck by vehicle #2. Driver #2 stated at the scene that she began backing out of her parking stall, and while doing so checked for traffic and did not observing anything. Driver #2 explained while backing out, she was suddenly struck by vehicle #1. Vehicle #1 had damage to the rear driver’s side quarter panel, and Vehicle #2 had minor damage to the rear driver side bumper. Driver #1 and Driver #2 did not complain of injuries at the scene, and EMS was not called to the scene. Both vehicles were driven by the scene by the drivers. No summonses issued regarding this investigation.

Pedalcyclist: Antonio Santiago 52 of the 200 block of N. Seventh Street
Driver #1 stated that he was traveling south on North East Avenue when a pedestrian on a bicycle drove off of the sidewalk and into the passenger side of his vehicle. Driver #1 indicated that pedestrian road into his vehicle without warning causing an unavoidable collision. Driver #1 claimed no injury.
Vehicle #1 sustained cracks to the passenger side windshield, scratches to the front passenger side door and the passenger side mirror was broken off.
On scene at the incident the pedestrian on the bicycle was conscious however he was not able to recall what occurred. The pedestrian was transported to the Inspira Medical Center of Vineland by Vineland EMS unit #630. The bicycle did not sustain any damage.
I secured the bicycle at Police Headquarters as property for safe keeping unit it can be recovered by the pedestrian, I then sent an email to Sergeant Riordan informing him of said property.
After securing the bicycle I proceeded to Vineland Inspira to issue the pedestrian his property receipt and a summons. Upon speaking with Emergency Room Registered Nurse Stacy Gregg, she advised me that the pedestrian is currently being transferred to Cooper Hospital due to internal bleeding. I immediately notified the shift supervisor Sergeant D’Arrigo of the pedestrian’s status and advised him that due to the circumstances I will be giving the summons and property receipt to his wife. I then spoke with the wife (Dolly Rodriguez) and gave her the summons that was issued and the property receipt. The pedestrian was issued summons number 0614-VC-08215 for “Bicycle Title 39 Violation” (39:4-14.1)
The bicycle involved in the collision is an Alpine Trac Bicycle that is gray in color.
I contacted Cooper Hospital and spoke with John Mason (Risk Management), he indicated that the pedestrian is there but has not been seen as of this time by. John provided me with his directly line (856-536-1312) and advised me to call him back in a few hours after he as the updated information.
