On 15 October 2015 I was dispatched to West Sherman Avenue and Southwest Boulevard for a rear end collision. The driver of vehicle 1 was identified as Shayla R. White. Shayla was driving NJ registration B30BXR which is a green 2003 Mazda Protégé. The driver of vehicle 2 was identified as Marc A. Ilarraza. Marc was driving NJ registration XCAT25 which is a grey 2015 Dodge Grand Caravan.
Upon my arrival I spoke with Shayla regarding her account of the incident which she stated the following:
Shayla stated she was heading Northbound on Southwest Boulevard, looking at the accident that occurred on Sherman Avenue and Southeast Boulevard. Shayla stated the traffic was moving slowly due to the accident that occurred on the other side. Shayla stated she was not paying attention and ran into the back of the vehicle in front of her. Shayla stated they pulled to the side of the road and waited for police arrival.
I then spoke with Marc regarding his which he stated the following:
Marc stated he was heading Northbound on Southwest Boulevard when he came to the intersection of Sherman Avenue and Southwest Boulevard. Marc stated he was waiting to turn left to head west on Sherman Avenue when he was rear ended. Marc stated he pulled to the side of the road and awaited for police to arrive.
After hearing the statements from both parties I determined I would issue a summons to Shayla for careless driving. Shayla was issued summons number 0614-VC-8143 for careless driving. Both drivers contacted their own tow trucks to have their vehicles towed.

Driver#1 stated he was traveling southbound on N Orchard Rd towards W Oak Rd. Driver #1 stated a deer ran out into the roadway in front of 1482 N Orchard Rd. Driver #1 struck the deer on the front passenger side front bumper causing minor damage. Driver #1 claimed no injuries. Driver #1 drove his vehicle from the scene. No summons issued.
I observed the deceased deer was off the roadway. I advised dispatch to leave a note for the road department to pick up the deer at their earliest convenience.
Point of impact was approximately 650ft S of W Wheat Rd and approximately 1ft West of North Orchard Rd.

Driver#1 stated he was traveling west bound on W Garden Rd approaching the intersection of N Mill Rd. As Vehicle #1 drove through the intersection he was struck by Vehicle #2 on the driver side rear door and quarter panel. Driver #1 stated he did not observe Vehicle #2 sitting at the stop sign on N Mill Rd.
He stated Vehicle #2 drove right through the intersection without stopping at the stop sign. Driver #1 claimed no injuries. At this time, Driver#1 was unable to provide his insurance documents. He advised it was at his residence and would respond to headquarters with proof of insurance.
Passenger #1 claimed no injuries.
Passenger #2 had complaint of pain to her arm but refused medical attention by Vineland EMS. Vehicle #1 was towed by Rulyns Towing. Driver #1 was issued a summons for unlicensed driver. 0614-VC-7046 for 39:3-10 (unlicensed driver)
Driver #2 stated he was traveling Northbound on North Mill Rd approaching the intersection of W Garden Rd. Driver #2 stated once he approached the intersection he stopped at the stop sign. Once he looked both ways he began to proceed through the intersection. He stated he did not observe Vehicle #1 traveling westbound on W Garden Rd. Driver #2 stated his vehicle was struck on the front bumper and front grill. I observed moderate damage to Vehicle #2. Driver #2 claimed no injuries. Vehicle #2 was towed by Good Fellas Towing.
Driver#2 was issued a summons for failure to stop. 0614-VC-7047 (39:4-144) Failure to stop.
Point of Impact was approximately 50ft south of the east curb edge of N Mill Rd and approximately 70ft west of the South curb edge of W Garden Rd.

Driver #2 stated (owner of vehicle #2) stated he was parked on the north side of West Landis Avenue, in front of Colonial Electric (64 West Landis Avenue) waiting for his brother, Carlos Rivera, to come out of Vineland Check Cashing. Driver #2 stated as he was waiting, vehicle #1 backed into his vehicle. He stated a male driver exited the driver position of the vehicle and described him as a tall black male.
Driver #2 stated when he told driver #1 that he needed to make a police report, driver #1 told him not to call police that he would give him cash for his damage. Driver #2 stated when he insisted on contacting police driver #1 motioned to his female passenger walking towards the vehicle to hurry up.
The passenger entered the vehicle and fled west on West Landis Avenue and south on South Delsea Drive. Driver #2 stated the license plate was New Jersey “W57FRK”, on a Chevy Suburban. Driver #2 claimed to have no injuries.
Witness: Carlos Rivera
Rivera stated he observed vehicle #1 back into vehicle #2. He stated the driver of vehicle #1 was a tall black male and was in line in front of him at the Vineland Check Cashing. Rivera and I responded to Vineland Check Cashing and the teller provided video surveillance footage at which time Rivera identified driver #1 as Brian Baldwin (New Jersey driver’s license #B02620967401865).
Officer Montana and Officer T. Hall located the vehicle in the area of 334 Axtell Avenue, in front of apartment #9. Upon my arrival to the scene I spoke with the registered owner of the vehicle, Tameeka Johnson. Johnson stated she was sleeping before the police responded. Johnson stated she did not know who was driving her vehicle and the keys were inside of her residence. I then spoke with Kimberly Allen who advised she was the driver of the vehicle from Vineland Check Cashing. I asked Allen where Brian Baldwin was and she stated he wasn’t at the apartment. Allen refused to identify Baldwin as the driver and insisted she was the driver of the vehicle. Note: Allen advised she did not have a driver’s license and only had a New Jersey identification.

On October 16, 2015, I was dispatched to the area of West Chestnut Avenue and South West Boulevard, in regards to a motor vehicle collision. Upon my arrival on scene, driver #1 stated while traveling West on Chestnut, behind vehicle #2, driver #2 suddenly stopped ahead of him which he hit the back of vehicle #2. Driver #1 further said that the traffic light was green. Driver #2 stated while traveling West on Chestnut Avenue, he stopped for the red light and driver #1 ran into the back of his vehicle. No injuries were reported. Driver #1 was issued a traffic summons for Careless Driving, summons #VC007514. A traffic subpoena was completed for driver #2. No witness(s) were located.

Driver #1 stated while she was traveling east on Almond Road approaching the intersection of Almond Road and North Mill Road. Driver #1 stated she was stopped behind a vehicle at the stop sign when she was suddenly stuck in the rear by vehicle #2.
Driver #2 advised he was traveling east on Almond Road approaching the intersection of Almond Road and North Mill Road. Driver #2 advised he was behind vehicle #1 when approaching the intersection. Driver #2 said while approaching the intersection he looked away from the roadway, and when he looked back up he observed vehicle #1 was close to his vehicle. Driver #1 advised applied the vehicle breaks, but was unable to avoid hitting vehicle #1.
Vehicle #1 had minor damages to the rear bumper and rear driver side quarter panel of her vehicle as a result of the collision. Vehicle #2 had very minor scratch marks to the front bumper as a result of the collision. Driver #1 and the two passenger in vehicle #1 did not have a complaint of pain and EMS was not called to the scene. Driver #2 did not have a complaint of pain and EMS was not called to the scene.
No summons issued regarding the collision.

On October 16, 2015, I was dispatched to the area of East Chestnut Avenue and South Valley Avenue, in regards to a motor vehicle collision. Upon my arrival on scene, driver #1 stated while traveling East on Chestnut Avenue, driver #2 made a right turn onto Chestnut Avenue from Valley Avenue in front of him. Driver #2 stated after making a right turn onto Chestnut Avenue from Valley Avenue, driver #1 ran into the back of her vehicle while she was traveling East on Chestnut Avenue away from the intersection. Driver #1 sustained a minor cut to his face. Driver #2 complained of pain to her right arm. Vineland Emergency Medical Service responded which both subject's refused treatment. No additional injuries reported. Vehicle #1 was towed by Larcher’s. Driver #2 contacted a private tow and had her vehicle removed from the scene. Driver #1 was issued a traffic summons for Careless Driving, summons # VC007515. A traffic subpoena was completed for driver #2. Vehicle #2's insurance. Driver #2 stated she was traveling east bound on East Chestnut Avenue approaching South Seventh Street when she put her right turn signal on to change to the right lane. Driver #2 stated when she changed lanes she was rear ended by Vehicle #1 which was also traveling east bound. Driver #2 stated Vehicle #1 turned into the nearest business to turn around and flee west bound on East Chestnut Avenue. According to Driver #2, Vehicle #1’s front driver side struck Vehicle #2’s rear passenger side bumper.
Driver #2 provided the following description for the Vehicle #1: a tan colored four door older vehicle (possibly 90s) bearing NJ registration of the first two numeric being “BB” being driven by a Hispanic male in his 20s.
Driver #2 and her occupant did not have a complaint of pain.
Vehicle #1 should have damage on the front driver side (unknown amount).
Vehicle #2 sustained moderate damage to the rear passenger side bumper.
The area checked negative for the described vehicle. Investigation pending additional information

On Friday, October 16, 2015, I was dispatched to the intersection of North Orchard Road, and West Almond Road, for a two vehicle collision with multiple injuries, and occupants being trapped inside.
Upon arrival to that intersection, I observed Vineland Fire Department and Vineland EMS were already on scene. Vineland Fire Department had already removed the trapped occupant from Vehicle #1, and she was in the process of being transported to Cooper Trauma Center via helicopter. The occupant seated in the rear, driver’s side seat in Vehicle #2, was taken to Inspira Medical Center, with minor injuries to her head and neck. The occupants located in the front and rear passenger seats of Vehicle #2 were also transported to Inspira Medical Center for complaints of pain.
I spoke with Driver #1, who stated she was unsure of what direction she was traveling, but said she was approaching the intersection of Almond and Orchard. Driver #1 explained she was unsure of whether the light was red, but just remembers colliding with Vehicle #2, and ending up on the West side of Orchard Road, facing Southbound. Driver #1 went with EMS to Inspira Medical Center for complaints of pain.
After speaking to Driver #1, I went to speak with Driver #2, who stated he was driving Southbound on North Orchard Road. He continued to tell me as he was approaching the traffic signal at the intersection of Orchard and Almond, when Vehicle #1 collided with his vehicle. Driver #2 added when he was going through the intersection, the traffic signal was green for his direction of travel. He further added when Vehicle #1 struck his vehicle, it caused his vehicle to hit the curb located on the Southwest corner of the intersection. He told me after his vehicle hit the curb, his vehicle hit the light post on the same corner, causing it to fall over. He concluded by saying his vehicle rolled over one complete time, and he landed on all four tires in the yard located at 466 North Orchard Road, causing large gouges in the grass.
The damage to Vehicle #1 was major, and was located on the front, passenger side corner, and the passenger door of the vehicle. The damage caused the vehicle to be disabled, and the vehicle was towed from the scene by Rulyn’s Garage.
The damage to Vehicle #2 was also major, and was located on the front, driver’s side corner, the driver’s side door, and the rear driver’s side door. The roof of the vehicle was also damaged due to the rollover. Vehicle #2 was also disabled due to the collision, and the vehicle was towed from the scene by Goodfellow’s Towing.
Driver #1 was issued the following summons for Failure to Observe Signal (0614-VC-8127.) I attempted to contact Cooper Trauma Center to obtain information on the passenger of Vehicle #1 that was airlifted from the scene, but they were unable to locate any information on the victim other than her name being "Giselle." When her information is obtained it will be added to a supplement report for this case.

On 10/04/2015, driver #1 failed to stop when I attempted to stop his vehicle. After driver #1 failed to stop the vehicle he continued to travel eastbound on Oak Road, where it collided with vehicle #2 at the intersection of Oak Road and the Northwest Boulevard.
Driver #2, said she was stopped at the stop sign at the intersection of Oak Road and the Northwest Boulevard, when she was hit from behind by vehicle #1.
Damage report;
Vehicle #1, sustained heavy damage to the front portion of his vehicle. The vehicle was towed by all towing.
Vehicle #2, sustained heavy damage to the rear portion of her vehicle. The vehicle was towed by J&C auto.
Injury report;
Driver #1 had no injuries to report.
Driver #2 said after she was hit she exited her vehicle and went into the Oak Tavern to call for an ambulance. Driver #2 said once she saw police arriving on the scene she did not call for an ambulance, she went back outside to her vehicle to tell police what injuries she sustained.
Once Driver #2 approached me she said her back, neck and left knee hurt her form the collision. She was asked if she wanted EMS to respond to the scene to treat her and stated, “No, I’m fine Motor Vehicle traffic summons: Driver #1: 0614-VC-006237 Failure to maintain lane, 39:4-88B. 0614-VC-006238 Suspended driver, 39:3-40. 0614-VC-006239 Uninsured Motor Vehicle, 39:6B-2. 0614-VC-006241 Reckless driving, 39:4-96. Driver #1 stated on scene she was traveling north on Northeast Boulevard approaching the intersection of Northeast Boulevard and Park Drive. Driver #1 advised when she approached the intersection she came to a complete stop at the stop sign. Driver #1 said after making a complete stop, she began to turn right on to Park Drive. Driver #1 stated while making the right turn, she was suddenly struck by Pedalcyclist #1. Driver #1 stated she did not see Pedalcyclist #1 prior to the collision.
Driver #1 explained after the collision she exited her vehicle, and could the smell the odor of alcohol coming from Pedalcyclist #1 person. Driver #1 said Pedalcyclist #1 told her he was ok, and suddenly left the scene on the bicycle.
After speaking to Driver #1 I spoke to witness #1 who stated the following: Witness #1 stated Driver #1 was at a complete stop at the intersection of Northeast Boulevard and Park Drive. Witness #1 advised driver #1 began to make a right turn on to Park Drive when Pedalcyclist #1 collided in to the passenger side of vehicle #1. Witness #1 stated Pedalcyclist #1 never used the breaks, or stopped traveling prior to colliding in to vehicle #1. Witness #1 said after observing the collision he approached Pedalcyclist #1, and could immediately smell alcohol coming from Pedalcyclist #1 breath. Witness #1 advised Pedalcyclist #1 stated he was ok, and left the area on his bicycle despite him telling Pedalcyclist #1 to stay on scene.
After speaking to Witness #1, I spoke to Witness #2 who stated the following: Witness #2 stated he observed Pedalcyclist #1 collide in to the passenger side of the vehicle #1. Witness #2 said Pedalcyclist #1 did not stop at the intersection, and did not apply the brakes prior to colliding in to vehicle #1. Witness #2 stated he approached Pedalcyclist #1 after the collision, and could immediately detect the odor of alcohol coming from Pedalcyclist #1 breath. Witness #2 advised he told Pedalcyclist #1 to stay on scene and to wait for Police arrival, however Pedalcyclist #1 left the scene on his bicycle.
Vehicle #1 sustained damage to the rear passenger side door, and scratched on the front passenger side door. Vehicle #1 was driven from the scene by Driver #1.
Pedalcyclist#1 was described as a light skin possibly Hispanic male with short gray hair and approximately 50 years of age. Pedalcyclist #1 was also described as wearing a thick flannel shirt, and black jeans. Pedalcyclist #1 bicycle is described as a black or purple ten speed bike, no other description was provided. Driver #1 did not have a complaint of pain and EMS was not called to the scene. After speaking to all parties, I checked Landis Park and the surrounding area for Pedalcyclist #1, however the check was negative.
Pedalcyclist #2 made no statements because he left the scene. Investigation will continue upon receiving further information about Pedalcyclist #1.

Pedalcyclist: Nelson Perez 61 of the 400 block of N. East Ave.
On this date, 10/18/15 I was dispatched to Inspira Medical Center (1505 W. Sherman Ave, Bed #13) in regards to a bicyclist who was injured in a collision two days prior. As I was dispatched, I was advised that there was a case prior on 10/16/15 from Park Dr/NE Blvd that was matching the description of a bicyclist who left the scene of a collision.
Upon my arrival, I spoke to Nelson Perez (Bicyclist.) He related that he was riding his bicycle on 10/16/15, Friday night and it was dark, around 7:30-8:00 pm. At first he wasn't too sure where the incident took place but he believed it was at 7th/Wood. He believed it was definitely in that area because that is his normal route he rides every day. He went on with the fact that his residence is 414 N. East Ave but due to a TRO, he is staying with a friend at 404 Rosewood Dr temporarily. I asked Perez that because he is staying on Rosewood Drive, it was possible that he was involved in the collision at NE Blvd/Park Drive? He stated, "No, I never ride my bike in those areas. I stay away from busy streets." I then asked him for details about the collision. He said that everything happened so quick that he didn't pay much attention to things.
According to Perez he was going Westbound and the vehicle involved was traveling Northbound. Perez believed this to be at 7th/Wood. He said he didn't see the vehicle and suddenly "It was there." He quickly turned his bicycle (Burgundy Schwinn 10 speed) and struck the vehicle on the passenger’s side with the left side of his body. At the time he didn't feel any pain, remembered telling someone he was okay and then got on his bicycle and left. Perez stated there was no damage to the bicycle because it was his body that struck the car. Perez did mention that he saw that the driver of the vehicle was a female and that it was "either a white Honda or Toyota." I again asked him about the location and he insisted that it was 7th/Wood.
When Perez woke up on 10/17/15, he felt some discomfort but was able to function and walked around Vineland all day. As he woke up this morning (10/18/15) and felt "a bad pain in his left side." He also said he could barely bend down to put his boots on and had difficulty breathing because of the pain. He then walked over to a friend’s residence of 510 Park Drive where he made a phone call to go to the hospital. This phone call was verified by dispatch and that is how Perez arrived at Inspira Medical Center. Perez was given something for the pain and was waiting to go for a cat scan to check on any injuries to his left side. He also had some minor scrapes on his head which led him to believe he may have struck his head when he hit the ground. During the time I spoke to him, Perez said several times "It was my fault. I ran into her."
Prior to leaving the hospital, I asked Perez that it made sense that if he lived on Rosewood Ave temporarily and because he had a friend that lived on Park Drive that it seems it could be possible that NE Blvd/Park Drive was where the collision may have occurred. He did agree that he does go that way to get to where he is presently staying.
After returning to headquarters, I reviewed Ptl Garvey's report. I believe there are a number of factors that match up with the original report and by what Nelson told me. First that he was traveling Westbound and the car was traveling Northbound. Secondly, that the car was a "white Honda or Toyota" as stated by Nelson and the fact that he stated that he saw the driver of the car was a female. Nelson also fits the description given to Ptl Garvey by the Driver #1 by him being a light skinned Hispanic Male with short gray hair.

Driver #1 stated that he was stopped in traffic on North Delsea Drive facing South bound when Driver #1 collided in to the rear bumper of his vehicle. Driver #1 and the passenger in vehicle #1 claimed no injuries at the scene and refused medical treatment. Driver #1’s vehicle sustained damage to the rear bumper.
Driver #2 stated he was traveling south bound on North Delsea Drive when he looked away from the roadway at his GPS. Driver #2 advised after looking at his GPS, he looked back up at the roadway and he observed vehicle #1. Driver #2 advised he was unable to stop in time, and he collided in to the rear of vehicle #1. Driver #2’s vehicle sustained damage to the front bumper and front passenger side quarter panel portion of the vehicle. Driver #2 declined EMS, and EMS was not called to the scene. Vehicle #2 was towed from the scene by Nissan of Vineland.

Driver #1 stated he was traveling north on Delsea Drive when he stopped in traffic to make a left in to USA gas station. Driver #1 stated he had his left turn signal on, and was suddenly struck in the rear by vehicle #2. Driver #2 stated he was traveling north on Delsea Drive behind vehicle #1. Driver #2 stated he looked away from the roadway towards Sanctuary (3926 N Delsea Drive) because of an incident at the business. Driver #2 said when he looked back at the roadway, vehicle #1 was right in front of his vehicle. Driver #2 advised he hit the brakes and attempted to swerve out of the way, but was unable to avoid colliding with vehicle #1.
Both drivers and the passenger in vehicle #1 did not have a complaint of pain, and EMS was not called to the scene. Vehicle #1 sustained damage to the rear bumper of the vehicle. Vehicle #2 sustained moderate damage to the front bumper and driver side quarter panel portion of the vehicle.

On 10/17/2015 I responded to the intersection of North Brewster Road and East Oak Road for a two vehicle crash with no reported injuries. Upon arrival I spoke with driver #1 who explained she was stopped in traffic facing south on Brewster Road. Driver #1 stated she was at a complete at the intersection (4way stop flashing red). Driver #1 advised while she was stopped, she was hit from behind by driver #2. While speaking with driver #1 she had a complaint of pain and EMS was called to the scene. Driver #1 refused medical treatment at the scene.
Upon speaking with driver #2, she explained she was driving south on Brewster Road approaching the four way stop at the intersection of Oak Road and Brewster Road. Driver #2 stated a bottle from her cup holder fell on to the driver side floor, and went underneath the brake pedal. Driver #2 said while attempting to grab the bottle she could not stop in time, and struck vehicle #1 in the rear.
Vehicle #1 sustained minor damage to the rear bumper area. Vehicle #1 was driven from the scene by the owner. Driver #2 reported no apparent damage to vehicle #2, and vehicle #2 was driven from the scene by the owner.
No summonses were issued in this case.
