Driver#1: Jescenia Rosa-Gomez 29 of the 100 block of Axtell Ave.
On 29 September 2015 I was in the area of Third Street and Quince Street when I was informed a vehicle appeared to have been struck. I was informed the vehicle was parked near Third Street and Almond Street. Upon my arrival I observed a vehicle with driver’s side rear end damage. I informed dispatch of the incident at which time a family of the owner came out of his residence. He informed me the owner was on his way to see the damage as well as provide me with the documents needed for my report.
A neighbor who witnessed the incident but wanted to remain anonymous stated he observed a gold colored Dodge Neon strike the vehicle then flee the scene. The neighbor stated he knocked on the owner’s door but no one answered and he did not want to be involved.
Corey L. Carter, the registered owner of the vehicle arrived on scene to observe the damage. Corey provided me with the required documents and information for report purposes. I informed Corey a report would be taken and he would be able to obtain the report in 5-7 business days. (Note: Vehicle information is NJ registration L14DRM/1997 Lexus ES 300 White)
While speaking with Corey Police dispatch stated there was an accident called in no Third Street and Quince Street. Dispatch stated EMS was dispatched since there was a child in the car during the accident. I contacted dispatch letting them know I was in the area and it may be linked to my hit and run. Upon arriving at the scene a vehicle matching the description of the hit and run was parked on the side of the road. The vehicle was a gold colored neon with front passenger side damage consistent with the damage obtained by the hit and run collision near Third Street and Almond Street. (Note: vehicle information 2001 gold dodge neon, NJ registration M53EAR)
I spoke with the driver Jescenia Rosa-Gomez regarding her account of the accident which she stated the following:
Jescenia stated she was heading southbound on Third Street when her one year old daughter (Jesliany DeLaCruz DOB 12/03/2013), who was seated in the rear right passenger seat began unbuckling her car seat restraints. Jescenia stated she reached to the back seat to buckle her daughter in and stop her from undoing the restraints when she collided with the car. Jescenia stated she kept going because she was scared since it was her first accident. Jescenia stated she contacted her dad and told him what happened which he told her she needed to call the police.
After receiving all the facts involved in the accident, I determined Jescenia was at fault and would be issued the following summonses:

0614-VC-4578- Leaving scene of accident (39:4-129)
0614-VC-4579-Failure to maintain lane (39:4-88)

When I arrived on scene I found Driver #1 being cared for by Vineland EMS. Driver #1 complained of neck and back pain. Driver #1 stated she was traveling east on East Chestnut Avenue behind Vehicle #2. Driver #1 indicated that when Vehicle #2 slowed down she applied her vehicle brakes, however her vehicle continued to travel forward causing her to rear end Vehicle #2.
Driver #1 was transported to Vineland Inspira Medical Center by Vineland EMS Unit #628 to be evaluated for her complaint.
Vehicle #1 sustained heavy front end damage and was driven off of the roadway into a parking lot by Driver #1’s father to wait for a tow.
Vehicle #1 was left on scene in a parking lot to wait for a tow.
Driver #1 was cited for following too closely.

On this date, 01 October 2015, this department was contacted by Millville PD and was informed that a subject was reporting a collision to them that occurred in our jurisdiction.
I was dispatched to, 1406 Wheaton Avenue Millville NJ 08332, to speak with the operator of the vehicle that was involved in a collision. Upon arrival I was met by Millville Officer P. Hand who stated he was dispatched to the scene after the driver contacted Millville PD. Hand stated the driver informed him he got stuck in the mud on Grant Avenue, in Vineland, and while attempting to drive his vehicle home it stopped working at that location.
The operator of the vehicle informed me that he was driving east on Grant Avenue when a deer jumped in front of his vehicle causing him to drive onto the side of the road. Driver1 stated he did not believe at that point he was involved in a collision so he left the scene to head home. Driver1 stated the collision happened approximately an hour prior to my arrival. Driver1 said the collision happened right past the bend on Grant Ave. Driver1 said he did not exit his vehicle to check on damage because he did not believe he caused any.
Vehicle one sustained moderate damage to the driver side door and moderate damage to the passenger side tire and fender. When asked why he did not call the police Driver 1 stated he did not believe he hit anything. All-Tow responded to tow the vehicle at the owner’s request.
At the scene of the collision I observed utility pole BT-27-VC was struck as well as a street sign (tractor crossing), both electric utility and road department were notified. There was approximately 12 feet of tire marks prior to the collision with the pole and then the yard and farm of 1532 E Grant was torn up from the vehicle attempting to get out of the dirt. A second set of tire marks were also on the dirt but no sign of a separate collision was present.
Driver 1 was supplied with the following summons:
(1) 0614-VC-007978 (leaving the scene of an accident)
(2) 0614-VC-007979 (failure to maintain lane)

Driver #1 stated at the scene she was traveling North on College Drive and had just stopped at a Red Light at Sherman Ave when she was struck from behind by vehicle #2. Driver #1 states that the driver of vehicle 2 was a young Spanish male. Driver #1 states that when she asked for his insurance information the subject got into his car quickly and sped off almost striking two other vehicles. Driver #1 stated the vehicle was white in color with PA registration FRE1188. Checking the plate showed an expired tag for a Carl Hall at 241 Back Street Montoursville PA. PA state Police were contacted and Trooper Quigley stated he would send someone out but to not expect much as this address is a trailer park with a lot of active criminal activity in the area. No summons issued at this time as it is unknown if the information from PA DMV is correct for the suspect vehicle.

Driver #1 stated at the scene that she was traveling North on Delsea Drive and was stopping in traffic before reaching Oak Road. Driver #1 states she was struck from behind by vehicle #2 but was not injured and her daughter was also uninjured. Driver #2 stated he was traveling North on Delsea Drive and did not realize traffic was not moving until he was at vehicle #1. Driver #2 states he attempted to swerve around but still struck vehicle #1. Driver #2 stated he was uninjured. Due to minor damage and no injuries no summons was issued for collision.

Driver #1 stated she was driving west bound in the parking lot of Shoprite (3600 East Landis Avenue) when she observed Vehicle #2 stop in front of the business in the fire lane (also facing west bound). Driver #1 then passed Vehicle #2 when she observed several pedestrians walking and not paying attention to vehicles. At that time, Vehicle #1 turned right abruptly and avoided the pedestrians only to strike Vehicle #2 located in fire lane. Vehicle #1’s front passenger side bumper struck the front driver side fender of Vehicle #2.
Driver #2 stated she briefly stopped in front Shoprite (3600 East Landis Avenue) to let her passenger out as the passenger is elderly and uses a walker. After dropping off the passenger in front of the business, Driver #2 proceeded to merge back into the traffic lane (going west bound) but, observed Vehicle #1 and did not merge right away. At that time, Driver #1 turned slightly right and sideswiped Vehicle #2. Vehicle #1’s front passenger side bumper struck the front driver side fender of Vehicle #2.
Both drivers did not have a complaint of pain.
Vehicle #1 sustained minor damage to the front passenger side bumper.
Vehicle #2 sustained minor damage to the driver side fender.
Driver #1 was issued summons #0614-VC-008310 (Unlicensed; 39:3-10).
Driver #2 was issued a subpoena with this report. Both drivers were advised of their options at Vineland Municipal Court.

Driver 2 stated she was traveling south on N Main Rd and was stopped in traffic at the red light at E Park Ave. Vehicle 1 came from behind and struck her vehicle. Damage to vehicle 2 was minimal to the rear bumper area. Driver 2 was advised of her option for complaint at the Vineland Municipal Court. Non-reportable approved by Sgt. Ruberti.

Driver 1 stated while he was driving south on South Lincoln Ave he approached the traffic that was stopped due to a red traffic light when he hit the rear of vehicle 2. Driver 1 stated he was trying to make a left hand turn on Dante Ave but did not see the vehicle in front of him stopped due to the rain. Driver stated he was at fault involving the collision. Vehicle 1 had minor damage to the front bumper of the vehicle. Driver 2 stated while she was stopped at the red traffic light at South Lincoln Ave and Dante Ave a vehicle struck the rear of her vehicle. Vehicle 2 had minor damage to the rear bumper of her vehicle. There were no injuries reported at the scene as a result of the collision. Both vehicle were able to be driven from the scene.

Driver 1 stated while she was driving east on East Park Ave the vehicle in front of her (vehicle 2) slammed on their brakes for an unknown reason. Driver 1 stated she did not have enough time to react due to the roadway being wet causing her vehicle to hit vehicle 2. Vehicle 1 had a moderate amount of damage to the front bumper of her vehicle. Vehicle 1 was able to be driven from the scene. Driver 2 stated while she was driving east on East Park Ave she noticed the vehicle in front of her vehicle stop instantly for an unknown reason. Driver 2 stated she was unable to stop in time to avoid hitting vehicle 3. Vehicle 2 had minor damage to the rear and front of the vehicle. Vehicle 2 was also able to be driven. Driver 3 stated while she was driving east on East Park Ave a truck pulled out of the driveway causing the without regard of the vehicles driving east on East Park Ave. Driver 3 stated she then slowed down and stopped in time to let the vehicle drive away without hitting the vehicle pulling out of the driveway. Driver 3 stated while she was stopped in the roadway waiting for the unknown vehicle to drive away vehicle 2 struck the rear of her vehicle. Vehicle 3 had minor damage to the rear of the vehicle and was also able to be driven. No injuries were reported at the scene as a result of the incident.

On this date I and Officer Connelly, members of the Vineland Police Department Street Crimes Unit, were patrolling the area of Delsea and Wheat when we came upon a collision near that intersection. I observed a white in color pick-up truck and a red in color Honda in the middle of the roadway. A black male driver got out of the white in color pick-up and I observed a Hispanic male with a bloody face. I advised dispatch to send EMS and I approached the hurt subject. He stated his name was Francisco Ortiz and that he just left his residence when the white truck pulled out of a driveway of the property on the northwest corner of the intersection. Ortiz stated he attempted to avoid the collision by swerving but could not. I advised him to sit down while EMS responded. I observed numerous small cuts to his face that seemed to be done by his head striking the windshield of his vehicle. Officer Connelly stood by with the driver to the truck who I recognized as Lewis Brailsford. Lewis had the front (crotch) area of his pants wet. I asked him if he urinated on himself. Lewis stated, "Yes". Lewis stumbled as I walked him to the east side curb edge away from traffic. I smelled a strong odor of alcohol coming from his person. I asked Lewis if he had been drinking alcohol. He stated, "yes". At that moment Officer Judice arrived on scene and Lewis was turned over to him for sobriety tests to be conducted. Ortiz was transported to Inspira Health Network for treatment to his injuries. Lewis was issued the following summons by me: VC-007927 (Improper Emerging from Driveway), VC-007928 (Uninsured M.V). Lewis was also issued several other summons by Officer Judice. (See DWI report)

Driver # 1 said she was traveling South on South Delsea Drive, in the fog lane (closest to the road edge) when a vehicle (X) pulled onto South Delsea Drive from the driveway of Friendly's Restaurant, 3655 South Delsea Drive to travel in a Southern direction. Driver # 1 said that vehicle X appeared as if the vehicle was going to merge into his lane of travel. Driver # 1 said she applied her brakes and began to slow down, at which time vehicle # 2, which was traveling South behind her struck her vehicle in the rear. Driver # 1 was unable to provide me with a description of vehicle or driver X. Driver # 2 said he was traveling South on South Delsea Drive, behind vehicle # 1. Driver # 2 told me that a red van, unknown make, model or registration pulled onto South Delsea Drive from the driveway of Friendly's Restaurant, 3655 South Delsea Drive. Driver # 2 told me that vehicle X pulled into the lane closest to center to travel South on South Delsea Drive. Driver # 2 said that it appeared that vehicle X was going to enter their lane of travel when entering South Delsea Drive. Driver # 2 told me that driver # 1 then applied her brakes abruptly, at which time he attempted to stop his vehicle. Driver # 2 said his vehicle slid on the wet roadway and struck the rear of vehicle # 1. Damage to vehicle # 1 was minor to the rear bumper and damage to vehicle # 2 was moderate to the front bumper and hood. Driver # 2, who is also the registered owner of vehicle # 2 left his vehicle parked on the property of Doctor Ashok Pilly located at 3662 South Delsea Drive. Driver # 2 told me that he would make arrangements to get his vehicle at a later date and time. No injuries were reported at the scene by either driver and no summonses were issued at this time. A check of the area for vehicle X was negative.

Driver #1 stated he was pulling out (making left turn) of the Main Discount Liquors (108 S Main Rd) parking lot when he suddenly saw vehicle #2 (traveling south bound) and attempted to accelerate but couldn't get across the roadway "fast enough" (NOTE: Roadways are wet from consistent rain). Driver #2 stated she was traveling south on S Main Rd when vehicle #1 pulled out in front of her. Driver #2 stated she hit her brakes but was unable to stop due to the wet roadway. Vehicle #1 sustained moderate damage to the driver's side rear bumper area and was later driven from the scene. Vehicle #2 sustained moderate front end damage and was towed from the scene by A-1 Towing (per Registered Owner's request). Passenger #3 in vehicle #1 complain of left shoulder pain and was transported to Inspira by her father (who did not want an ambulance to respond to the scene. Driver #1 was issued summons 0614-VC-006272 in violation of 39:4-66A (Improper entering roadway). Driver #1 stated he could have gotten out of the way (pulling from the parking lot) if Driver #2 wasn't driving "fast" and was advised of his complaint options with VMC. A Subpoena was written for Driver #2.

Driver # 1 said she was traveling Northbound on South Mill Road in the area of West Chestnut Avenue when a deer ran from the Eastern side of South Mill Road, directly into her lane of travel. Driver # 1 said she then struck the deer and then contacted this department to report the collision. There was no visible damage to vehicle # 1 except what appeared to be hair from an animal, possibly a deer around the driver side headlight. No summonses were issued to driver # 1.

Driver # 1 stated he was attempting to make a left hand turn onto N. Orchard road from W. Landis Avenue. He advised me that he had the green light at the traffic signal, and believed it was clear to turn, so he proceeded. Driver # 1 said at that time he was struck on the front portion of his vehicle by vehicle # 2. I observed heavy damage to the front potion of vehicle # 1. Driver # 1 was transported to Inspira Network by EMS for complaint of pain to both arms. Vehicle # 1 was towed by Olins Towing. Driver # 2 advised me he was traveling west on W. Landis Avenue. He said as he crossed the intersection of N. Orchard Road and W. Landis Avenue, vehicle # 1 attempted to turn in front of him. Driver # 2 said he attempted to avoid the collision by applying his brakes but was unable too. At that time he swerved to the right, in attempt to further avoid the collision. Driver # 2 said he struck the front portion of vehicle # 1 and then struck the traffic signal, which was located at the northwest corner of W. Landis Avenue. Driver # 2 complained of pain to his back and was transported to Inspira Network by EMS. Vehicle # 2 was towed from the scene by J&C Auto. Driver # 1 was issued summons # 0614-VC-4512 for Improper turning. Driver # 2 was issued summons # 0614-VC-4513 for Unlicensed Driver.
