Driver 1 stated she was waiting to pullout into traffic from the driveway of 20 West Park Avenue. She observed a vehicle traveling westbound on Park Avenue, with his right turn signal on to turn into the driveway of 20 West Park Avenue. She stated as the driver began his turn, his vehicle began sliding forward, instead of turning, and struck her driver side door area. Driver 1 and her passenger stated they did not have any injuries. Driver 2 stated he was driving westbound on Park Avenue and was attempting to turn right (north) into 20 West Park Avenue. He stated as he began his turn, his vehicle began sliding forward and struck the vehicle waiting to pull out of the driveway. Driver 2 stated he was not injured. Driver 2 was issued summons #0614-VC-5025 for Careless Driving.

Driver 2 stated he was backing out of a parking spot and did not see driver 1 and struck her in the rear driver side door. Driver 2 did not have any complaint or sign of injury. The vehicle was driven from the scene No summonses were issued.

Driver #1 stated he was heading east on West Oak Road when he approached the intersection at Northwest Boulevard. Driver #1 stated he stopped his vehicle behind vehicle #2. Driver #1 stated he looked at the railroad lights which were signaling and his vehicle slightly moved forward, striking vehicle #2. Driver #1 had no complaints of pain or injury. Driver #1 advised his vehicle suffered minor damage to the front license plate. Driver #2 stated she was heading east on West Oak Road when she approached the intersection at Northwest Boulevard. Driver #2 stated she was completely stopped and was the first vehicle at the stop sign. Driver #2 advised vehicle #1 then struck the rear of her vehicle. Driver #2 had no complaints of pain or injury. Driver #2 advised her vehicle suffered minor damage to the rear portion. Vineland Emergency Medical Services responded to the scene and both drivers refused medical treatment. No summons was issued. Investigation is complete.

Driver #2 stated he was traveling North on North Main Rd. Driver #2 said he didn’t see Driver#1 turning and rear-ended him. Driver#2’s vehicle had heavy front end damage and was towed from the scene by All-Tow.
Driver #2 had a complaint of wrist injury and was evaluated by VEMS on scene. Driver #2 signed a refusal form with VEMS.
Driver #2 was issued a summons for Carless Driving 39:4-97 0614VC-005201, he was given a court date of March 30, 2015 at 09:00am.

Driver#2 stated he was traveling east bound on Landis Avenue when he attempted to make a left turn onto North State Street when Driver #1 illegally passed him on his driver side striking his vehicle and causing the collision. No reported injuries. Moderate damage to both vehicles. Vehicle #2 was towed from the scene by Olins Towing. Driver#1 was issued Summons# VC-004099 for Improper Passing.

Driver 1 stated after the collision vehicle 2 suddenly fled the scene eastbound. Driver 1 was only able to describe vehicle 2 as a blue 4 door car (unknown make/model/registration/driver description). There were no summons issued.

Driver #2 stated as he was traveling south on North West Ave he came to the traffic light waiting for it to turn green and as he was moving forward slightly he felt vehicle #1 hit the back of his car. Driver #2 have minor damage to the rear bumper and to the passenger side brake light. No claims of injuries were reported at the scene.

Neither driver had complaint of pain nor suffered from any injuries. Vehicle #1 had minor damage to the rear bumper consisted of several scratches. Vehicle #2 had moderate damage to the front bumper consisted of several cracks, dents and scratches throughout the bumper. Neither vehicle required a tow truck and both vehicle were able to drive away from the scene. Driver #2 was issued summons number VC-005045 for 39:4-89, Following too Close.

Driver 1 stated he was traveling North on Union Rd. when a Deer stepped into the road. He stated he slowed down but the deer kept running and hit the front fender of his car and was thrown up onto the windshield and the rolled off breaking the driver’s side mirror. Driver 1 was not injured. The deer expired at the scene. County road department was notified to remove the carcass. The vehicle sustained damage to the front fender and mirror and was able to be driven from the scene. The point of impact was about 500' North of Hawthorne Crt. and about 8' East of the West curb of Union Rd. In front of 3184 Union Rd.

The area was checked but it was negative for the vehicle. No summonses were issued at this time.

On 3/19/15 at approximately 2156hrs, I was dispatched to 330 E Walnut Road for a hit and run crash. Upon my arrival, I met with Lashan Gillam, Driver #2 who was being medically evaluated by Vineland EMS. Gillam told me she was driving her vehicle (NJ-X30DLX) northbound on S Delsea Drive towards Elmer Road. The traffic in front of her was stopped so she began slowing her vehicle. She looked in her rearview mirror and saw a black Acura SUV behind her that was not slowing down. Gillam said her vehicle was rear ended by the Black SUV. She and the SUV turned east onto Elmer Road and pulled over to the side of the road. The SUV pulled in front of her vehicle. Gillam said she saw the driver, a black male with a beard, exit his vehicle and walk to the front of his vehicle to look at the damage. Gillam tried to open her door to speak with the male, but it would not open. The male then got back into his vehicle and drove away. Gillam followed the vehicle for several blocks but lost sight of it and pulled into the driveway of 330 E Walnut Road and called police. Gillam gave me the license plate number from the SUV (NJ-SKZ79D). I ran the plate through NCIC and it returned to a Black Acura MDX SUV registered to Patricia Pulley of 103 W Cherry Street, Vineland. Gillam's vehicle (Vehicle #2) had severe damage to the rear bumper and trunklid. Gillam claimed no injuries at this time and signed a waiver from EMS.
I then responded to 103 W Cherry Street to attempt to locate the SUV and driver. I made contact with Patricia Pulley who told me she owns the SUV and her husband, Daryl Calloway is driving it. Patricia confirmed that her husband has a beard. I asked Patricia to call Calloway and have him respond to my location but she was unable to get him to answer after several attempts. Patricia told me Calloway was driving to an unknown store and had already received a text message from him at 2214hrs stating "on the way". Patricia told me she would notify police as soon as he gets home. A bolo was sent out to officers with a description of the vehicle, but at the time of this report, I have been unable to locate Calloway or the SUV.
No witnesses were located at the scene.

On this date, March 20, 2015, I was dispatched to the area of West Oak Road and the Northwest Boulevard, in regards to a hit and run motor vehicle crash. It was reported the accused vehicle, fled the scene traveling west on Oak Road and was a silver Honda Civic. When I received the call for the crash, I was at Delsea Drive and Garden Road. I traveled to the scene south on Delsea Drive, then east on Oak Road, but I was unable to locate the accused vehicle. I arrived at the scene and met with driver # 2 and he related the following information. Driver # 2 indicated he was traveling south on the Northwest Boulevard and stopped for the stop sign at the Oak Road intersection. Driver # 2 stated as he started to proceed forward, vehicle #1 came up from behind him failed to stop in time, and struck the rear of his vehicle. Driver # 2 stated he moved his vehicle out of the intersection, and he believed driver # 1 was going to stay at the scene, but he went around him and drove off heading west on Oak Road. Driver # 2 described vehicle #1 as a newer silver Honda Civic (2 door) and he believed it had Illinois license plates on it. He stated vehicle sustained extensive front end damage, that would be noticeable. He indicated driver #1 was a Hispanic male with a "clean cut" hair style. With the amount of vehicle debris on the roadway, vehicle #1 should have moderate damage to the front bumper and headlight assembly. The pieces of debris were collected and secured as evidence. After I cleared the scene, I began checking the area again for the accused vehicle, but had negative results. This investigation will continue, once the driver of vehicle #1 is identified, and/or the accused vehicle is located. Summonses are pending the identification of driver #1.

On this date, March 20, 2015, I was dispatched to the intersection of South West Avenue and West Landis Avenue, in regards to a motor vehicle crash. I arrived in the area and observed two vehicles in the parking lot of Tony Soprano's (West Avenue and Landis Avenue). I met with both drivers and the following information was provided. Driver # 1 indicated he was exiting the restaurant onto West Avenue, and he started to pull out after being waved on by another unknown driver and didn't see vehicle # 2 traveling on West Avenue. Driver # 1 stated as he started to pull out, he struck the side of vehicle #2. I then spoke with driver #2 and she related she was traveling north on West Avenue, approaching Landis Avenue, when vehicle #1 pulled out of the driveway and struck the side of her vehicle. No injuries were reported and Vineland EMS was not needed at the scene. Vehicle # 1 sustained moderate damage to the front end, but was able to be driven from the scene. Vehicle #2 sustained moderate damage to the passenger side doors, but was able to be driven from the scene. At this time, no summonses were issued.

Driver #1: Evelio Tobar-Peraza 39 of the 600 block of Joyce Lane
Driver of the parked vehicle that was struck stated at the scene, she parked her vehicle on Montrose Street just east of Sabater School on the south side of the roadway facing east. She advised she walked into the school, retrieved her children and returned to her vehicle where she was advised by an unknown crossing guard that her vehicle was struck by a Green Cab. She continued to say that she went home and then responded to Green Cab to advise them of the situation. That is when Joseph Feliciano contacted this department regarding the incident.
Furthermore, I asked Joseph how many cabs were running and he said 7 cabs were currently running. I asked Joseph to broadcast over there radio for all drivers that were clear to respond to their headquarters so I could look at each vehicle.
While at Green Cab, I observed minor damage to the front driver side quarter panel and driver side headlight. It appeared as if a side view mirror from the accused vehicle struck the quarter panel and due to the driver continuing to drive cut and ripped paint off all the way to the headlight. Vehicle# 2 had green paint transfer as well where the damage was located.
At that time, vehicle# 1 arrived to Green Cab and I looked at the passenger side view mirror. The mirror had white paint transfer which was consistent with vehicle# 2. The damage looked fresh and I observed thick pieces of white paint which was consistent with vehicle# 2's paint still stuck to the passenger door. The paint stayed on the passenger door due to it snowing and the door being wet. I showed driver# 1 what I had located and he said he did not know what happened. I asked him if he was in the area of 7th Street and Montrose Street around 2:15 PM and he said he was. A check with Green Cab dispatch revealed driver# 1 was in fact sent to the area of 7th Street and Montrose Street around the time of the collision. With all of this information gathered, it was evident that driver# 1 was at fault for the collision; however he refused to acknowledge he knew he struck the vehicle.
No injuries were reported as a result of the collision. Minor damage was sustained to vehicle # 1's passenger side view mirror. Vehicle# 2's vehicle sustained apparent minor damage to the front driver side quarter panel and driver side headlight lens cover. Driver # 1 was issued the following summonses.

Driver #2 stated he was stopped at the red light on E Landis Ave and Sixth Street facing west bound when Vehicle #1 hit his vehicle from behind, causing the collision. Driver #2 stated he observed Vehicle #1 pull away from the scene and make a left hand turn on to S. Sixth Street. Driver #2 stated after he realized he was involved in a collision he followed Vehicle #1 and observed it pull into a school parking lot where it stopped. Driver #2 stated the vehicle was occupied by two Hispanic males and was unsure which one was driving the vehicle as he did not see the driver. Driver #2 stated he approached the two males and Driver #1 repeatedly informed him that he was the driver of the vehicle. I spoke with Driver #1 who was now standing outside the vehicle with his relative, who was also in the vehicle when the incident occurred. Driver #1 was very uncooperative but, after several minutes of speaking with him he informed me that he was the driver of the vehicle. He stated he observed Vehicle #2 stop at the red light on E Landis Avenue and Sixth Street but, stated as he approached the traffic signal the light turned green and he continued to drive at his normal speed. He stated Vehicle #2 did not drive forward when the light turned green therefore Driver #1 crashed into Vehicle #2. Driver #1 stated he did not stay at the scene of the collision because he was fearful as the vehicle he was driving does not have insurance. Vehicle #1 sustained minor damage to the front bumper consisted of a several small cracks to the right side of the bumper. Also, there were several small red chips of glass lying on the front bumper. Vehicle #2 sustained extensive damage to the rear bumper consisted of several very large dents and deformity to the rear bumper. While speaking with Driver #1 I could smell a strong odor and an alcoholic beverage emanating from his breath. He was unable to keep his balance while speaking with me. His eyes were red and very glassy. Due to his demeanor Field Sobriety Tests were conducted and as a result Driver #1 was arrested for driving while intoxicated, see DWI report for more information. Vehicle #1 was towed by J&C Towing and placed on a 12 hour hold Neither driver or their occupants complained of any pain. Driver 1 stated he was travelling west on W Oak Rd approaching the intersection with N Delsea Dr. Driver 1 stated he was going to make a left onto N Delsea Drive. Driver 1 stated he saw vehicle 2 coming east on Delsea but believed the vehicle was stopping. Driver 2 stated he then began to turn at which time, vehicle 2 which never stopped, hit his vehicle. Driver 2 stated she was travelling east on W Oak Rd approaching N Delsea Dr. Driver 2 stated as she entered the intersection, vehicle 1 suddenly turned in front of her vehicle causing the collision. Driver 1 had complaint of pain to her right leg and right wrist. Driver 2 had complaint of pain to his head. One passenger in vehicle 2 had complaint of pain to his right elbow One passenger in vehicle 2 had complaint of pain in his right knee and chest. Sunday, March 22, 2015 I responded to the area of South Eighth Street and East Grape Street for the report of a collision involving a pickup truck and a tree. Upon arrival I first determined that the collision involved an unoccupied parked vehicle first, and then the tree. I spoke with the driver of vehicle #1 and determined that he was no claiming any injuries. Driver #1 exited his vehicle and began speaking to me in Spanish. He supplied me with registration and proof of insurance for the pickup truck. While speaking with Driver #1 I could smell a strong odor of alcoholic beverage coming from his breath and he was slurring his words. Driver #1 was not able to supply me with an accurate statement on what happened. It was believed that Driver #1 was under the influence of intoxicating liquor. See DWI report for more details.
I then spoke with Jeremiah Roberts who explained that he was not in his vehicle at the time of the crash. He stated his vehicle was parked on the west side of south Eighth Street facing south. Roberts explained that he did not witness the collision however his vehicle sustained heavy damage to the driver’s side door and front fender.
Vehicle #1 sustained heavy damage to the front end. It was towed from the scene and placed on a 12 hour hold for the DWI. Vehicle #2 sustained heavy damage to the driver’s side door and front fender and was left at the scene with the owner.

Driver # 1 stated she had stopped her vehicle in the intersection of South East Avenue and East Landis Avenue, purposely blocking the path of a second vehicle stopped at the intersection. Driver # 1 said she was exiting her vehicle to confront her boyfriend, who was seated in the passenger seat of the second vehicle. Driver # 1 said she forgot to put her vehicle in park. Vehicle # 1 rolled forward, unoccupied, drove overtop the curb, and struck the north east corner of East Landis Motel (827 East Landis Avenue). Vehicle # 1 sustained minor damages to the front of the vehicle. The building sustained minor damages to the aluminum molding around the corner. The owner of the building, Ray Marquez, was advised of the damages and estimated repair cost at under $50.00. Driver # 1 stated she was driving through the Village Apartment Complex (890 East Walnut Road) and was preparing to make a left turn when the driver side of her vehicle was struck by pedalcyclist # 1. Driver # 1 was uninjured during the collision and her vehicle sustained a small dent and chipped paint to the driver’s door.
Pedalcyclist # 1 said he was riding his bicycle through the Village Apartment Complex (890 East Walnut Road) when he drove into the side of vehicle # 1. Pedalcyclist # 1 sustained a fractured left wrist and abrasions to the left side of his face. Pedalcyclist # 1 was not wearing a helmet and his mother was made aware of the incident (Karen L. Silcott-Palmer / 890 East Walnut Road # 96 / DOB 07-06-1976).
I met with Village Apartment Manager, Angie Becker. Becker was able to locate video surveillance of the collision. After viewing the surveillance videos, I observed driver # 1 traveling east through the apartment complex, preparing to make a left turn. I then observed the pedalcyclist approach the intersection traveling south, striking the side of vehicle # 1. Driver # 1 was properly operating her vehicle through the apartment complex and she was unable to avoid the pedalcyclist striking her vehicle. Pedalcyclist # 1 failed to properly navigate on the apartment complex roads.

I responded to Collage Drive and West Sherman Avenue for a minor two vehicle crash. Upon arriving, I first determined that neither of the two occupants were claiming any injuries. I then spoke with driver #2 who explained that he was stopped in traffic in the lane to proceed straight through the traffic light at Sherman and Collage Drive. He explained that while stopped waiting for the light, driver #1 attempted to switch lanes. Driver #2 explained that driver #1 was behind him in the straight lane and attempted to move to the left turn lane. According to driver #2, Driver #1 struck his vehicle while changing lanes.
Driver #1 explained that she was attempting to switch lanes from the straight lane to the left turn lane on West Sherman Avenue. She explained that she struck vehicle #2 while doing so and that the crash was her fault.
Vehicle #1 sustained damage to the front passenger side fender and mirror; the vehicle was driven from the scene. Vehicle #2 sustained minor damage to the rear driver’s side fender and tail light. Vehicle #2 was also driven from the scene.
No summonses have been issued in this case.

Driver 2 stated that he was traveling east on W Landis Ave. He was stopped in traffic at the red light. Vehicle 1 struck his vehicle from behind. Damage to vehicle 2 was on the rear passenger side bumper area.

Driver #1 was issued a summons for following too closely.

NOTE: Driver # 1 stated that she believed that driver # 2 was speeding. Driver # 2 stated that he was Southbound on S. Main Rd. when vehicle # 1 turned left into his vehicle. There were no reported injuries at this time. Driver # 1 was issued a Summons for Improper Turn (left) 39:4-123, Summons # VC-004678.
