Driver 1 Orlando Garcia Ridgewood Dr  Vineland age 39

On 11/27/2014 at 0037hrs I was dispatched in the area of East Sherman Avenue and South Main Road for a collision. Upon arrival to the scene I observed a black pickup truck on the south side curb of East Sherman Avenue. As I approached the vehicle I observed the driver, identified as Orlando Garcia-Lopez, sitting inside of the vehicle.

After ensuring he was not injured in anyway, I asked Orlando what happened. Orlando stated he was traveling west bound on East Sherman Avenue approaching South Main Road. When he began to slow down, he lost control of his vehicle due to the wet roads.

Investigation at the scene revealed point of impact to be approximately 25 feet east of the eastern curb edge of South Main Road and approximately 8 feet south of the southern curb edge of East Sherman Avenue. Point of impact was determined by information obtained at the scene.

It was determined while traveling west bound on East Sherman Avenue, Orlando left the roadway, just east of South Main Road, striking a utility pole (VE 1168 S) on the south side of the roadway with the front passenger side of his vehicle.

Orlando had no reports or any pain and/or injury while at the scene

Vehicle #1 sustained damage to the front and passenger side of the vehicle rendering it inoperable. Orlando was issued a summons (VC 4025) for Careless Driving in reference to this incident.

During the investigation on scene, an odor of an alcoholic beverage was detected emanating from Orlando. Field sobriety test were conducted and ultimately Orlando was arrested for Driving While Intoxicated. (Reference DWI Report for further information) BAC .15%

The vehicle was towed from the scene by Giancola Tow Service and placed on a 12 hour hold.

Vineland Electric was advised to respond and evaluate the damage to the utility pole.

Location: 3600 East Landis Avenue, Vineland, ShopRite

Date: November 29, 2014, at approximately 2:04PM

Driver 1 Jose Almanzar 3rd St age 34

On November 29, 2014, at approximately 2:06PM I was dispatched to 3600 East Landis Avenue, Vineland, ShopRite parking lot for a one vehicle collision with EMS responding. Upon my arrival EMS was on scene and vehicle #1 was parked in front of a light post, at the north west corner of the parking lot in front of the ShopRite building. Only driver #1 was present and stated the occupants went into ShopRite to use the facilities.

Driver #1 stated he was talking to his wife who was seated in the front passenger seat. He stated he was traveling west bound through the parking lot and attempted to conduct a left turn traveling south when he didn’t see the light post and struck it with his vehicle.

Driver #1 claimed to have no injuries and refused medical treatment with EMS. Both occupants of the vehicle also claimed no injuries. I observed damage to the front drivers’ side panel to the vehicle where the front drivers tire was pushed into the vehicle making unable to be turned.

Driver #1 along with the owner of the vehicle contacted their Insurance Company (Gieco) who later recommended a tow company. The vehicle was left on private property to await a tow.

Point of impact was approximately 235’ feet north of the north curb line of East Landis Avenue and approximately 75’ feet east of the west edge of the property line to 3600 East Landis Avenue. Point of impact was determined by position of vehicle #1.

I spoke with the Store Manage of ShopRite, Joe McCluskey and advised him of the collision to the light post. I observed scrap marks to the concrete barrier which supported a metal light post. The metal on the light post was damaged and bent. McCluskey advised he would attempt to locate the property owner to have the damage estimated.

Driver # 1 advised he was north bound on South Lincoln Avenue, north of Dante Avenue, following vehicle # 2. Both vehicles had just entered a Construction zone. Driver # 1 said he observed vehicle # 2 activate its left turn signal and began to slow down to execute a turn onto the property of 1748 South Lincoln Avenue. When Driver # 1 saw vehicle # 2's action, he stated that he applied his vehicle's brakes and the pedal went to the floor, but his vehicle failed to stop. Driver # 1 explained that due the crash occurring in a construction zone and Construction personnel and equipment was on either side of the roadway, he had no other alternative but to strike vehicle # 2 in the rear. Driver # 1 said that his vehicle was serviced several months earlier. No specific time given.

Driver # 2 said he was north bound on South Lincoln Avenue, south of Dante Avenue, when he entered a Construction zone. Driver # 2 said that he activated his vehicle's left turn signal and slowed his vehicle down to nearly a stop, preparing to execute a turn onto the property of 1748 South Lincoln Avenue. Driver # 2 said that while he was preparing to make his turn, his vehicle was struck in the rear by vehicle # 1. Driver # 1 complained of pain in his lower back from this crash. Vineland EMS responded to the scene and treated Driver # 1 for his reported injury, and then transported to Inspira Hospital.

An employee of the Vineland City Water Department, Dave Ricci, said he witnessed the crash. Mr. Ricci said he observed vehicle # 2 activate his left turn signal, slow down and prepare to execute a left turn onto 1748 South Lincoln Avenue. Mr. Ricci then said he observed vehicle # 1 collide into the rear of vehicle # 2.

Investigation revealed Driver # 1 was unable to maintain control of his vehicle due to a possible mechanical defect, specifically the vehicle's brakes being the problem. Driver # 1 contacted his own tow service to remove his vehicle from the scene. As a result of this investigation Driver # 1 was issued a summons for careless driving, summons # 0614 VC 003195. Driver # 2 was issued a subpoena to appear in court for the charge signed against Driver # 1.
